Provider uses a mixture of progress report types (guild of st Agnes, eec, etc.). Licensor reviewed 3 random children’s files and found 1 to have no progress reports. Provider was not aware school age children required progress reports once a year. Please begin conducting progress reports once a year for school age, every 6 months for preschool and every 3 months for infant/toddler/special needs children.

The program was operating above its licensed capacity. There were seven children present. All under five years old including two under two years old (one infant).
Outdoor space: cinder block accessible, uncovered sandbox with water accumulation. Playroom: missing two outlet covers Bathroom: shampoo, conditioner on the tub Kitchen: blades in two unlocked drawers.
Back yard: wooden tippable shelving unit on grass not secure.

The smoke detector at the bottom of the stairs was not working at the time of the visit.

This follow-up visit inspection recorded no violations or advisories.
The educator has a daily schedule that includes a variety of activities. **The educator stated that she is not taking the children outside because of the hazards present in the back yard. ** ** Regulations require at least 60 minutes of physical activity in full day programs; and daily indoor and outdoor time periods, weather permitting, which include both small and large muscle activities**
The following hazards were observed on the outdoor play space ( back yard). Missing fence section near the gate closer to the parking spaces. The wooden gate that leads to the side yard is loose, it is not installed. Wood panels leaning on the fence Snow blower, rolled rug, bricks, concrete tiles accessible The ground in the back yard was covered with snow and ice, but the licensor observed pea gravel under a little tikes structure accessible to the young children enrolled in the program.
Two little tikes structures that are above 18 inches from the ground with no fall zone. Discussed requirements with educator. Emailed the Playground booklet and policy for reference. Please remove the structures or install a fall zone.

Attendance is tracked in a notebook with the daily arrival and departure times of the children. School age children were not signed out on the day of the visit. Discussed attendance being accurate at any given point in the day.
The home is located on the corner of a busy intersection. Fenced in yard is missing entrance gates in the front and the side of the yard. These gates must be in place to prevent children access to the road. Educator will need to send licensor photos of the gates when installed.

The gate was not secure at the bottom of the stairs at the time of the visit.
The emergency card information that was in the first aid kit did not contain the permission for emergency treatment and transportation to a hospital. Also missing doctor's information
Front and back doors. The back door was obstructed by a gate and dog items on the floor. Please make sure all exits are accessible and safe at all times. Deck used as an exit had trash bags and miscellaneous items to be disposed.

Pack and plays. Sleep mats for older children. Blankets are stored touching each other. Discussed storing napping items separately in extra large Ziploc bags that can be labeled for each child. Educator washes all blankets every Friday.
