The ground surface under and around the large slide in the outdoor space was hard packed and tree roots were protruding. There was a gap under a section of the fence that was accessible to children playing in the area.
There was no gate at the bottom of the back stairs adjacent to the outdoor play area. The back door entry was in need of cleaning.
