It was observed that there was chipped and flaking paint throughout the program on classroom walls and window sills accessible to children. (This was previously cited on 5/15/25 and 4/15/24).

The following was observed and accessible to children: In the PRE-K classroom upstairs, there were rips in the red vinyl seat cushion and a blue plastic seat was cracked.

A review of three children's records indicated that two did not have evidence of a current physical examination.
The following was observed and accessible to children: 1) In the INFANT classroom, the A/C electrical cord was not secured. 2) In the PRE-K classroom, there were brooms and a dustpan near the children's lunchboxes.
During the visit, the following was observed and accessible to children: In TODDLER 1, the shelf on the stage was unsecured and tippable. In TODDLER 2, the floor grate under the windows was unsecured and rusted over. In PRESCHOOL, the floor grate next to the handwashing sink was rusted over, the garbage container was uncovered and contained discarded food items, and the baseboard panel was peeling in the circle area. In PRE-K UPSTAIRS, there was chipped and peeling paint on the two windowsills and the garbage container was uncovered and contained discarded food items.

During the visit, the following was observed and accessible to children: There were two containers of Clorox wipes on the changing table between INFANTS and TODDLERS. At the back stairwell leading up to PRE-K and the secondary exit, there was an open bag of rock salt.
A review of the evacuation drill log indicated that a drill was not conducted in March.
It was observed that on the Preschool outdoor play area, that the pink play kitchen was not secured and tippable.
It was observed that in the PRESCHOOL classroom, not all of the children's water cups were labeled with their name.
It was observed that the unlocked changing table cabinet in TODDLER 2 contained diaper creams, ointments and wipes. These were accessible to children.
The following was observed and accessible to children: In the TODDLER 1 bathroom, the baseboard was peeling. In TODDLER 2, a window screen had a small hole. In PRESCHOOL, the dollhouse bookshelf and plastic tool bench were not secured and tippable.

The cabinet in the Pre-K bathroom was not locked, leaving hazardous items accessible to the children.

The following hazards were observed indoors and were accessible to children: Plastic bags were accessible to the children in the Preschool bathroom and rear lobby. In the Toddler classroom, one electrical outlet did not include a safety cover. A plastic kitchen set was not secured, the L bracket was pulled from the wall. There were several floor tiles missing in the Preschool bathroom and pose a tripping hazard. The kitchen floor was in need of cleaning.
In the Toddler 1 classroom, a can of Lysol and COVID tests were accessible to the children. A bucket of rock salt was located in the second floor hallway.
It was learned during the visit that the Program Director is regularly working in the Preschool/School-Age classroom. The program's licensed capacity requires the Program Director to be 100% non-teaching administrative time.
This renewal - monitoring visit inspection recorded no violations or advisories.

The outdoor first aid kits do not include a CPR mouth guard as required.
Two children's Individual Health Care Plans did not include all of the required information. This was previously cited on 10/2/2019.
There is peeling paint on the window sills in the Toddler and Preschool classrooms. After reviewing the program's Lead Paint Report and Compliance Letter, these areas are noted as having tested negative for lead and do not pose a lead hazard to the children.

The following issues were observed on the Preschool Playground: unknown vegetation and weeds growing on the fence and in the sand. One plastic play kitchen must be secured. Two play kitchens and the climbing structure need to be cleaned. Two toy garages have broken parts. The following issues were observed on the Toddler Playground: there is an air conditioner protruding from a low window onto the playground where children are playing. This poses a safety hazard to the children. A piece of rope approximately two feet long was hanging on the fence accessible to the children.
The first aid kit taken to the playground does not include all of the required items.

The following was observed in the Infant Classroom: the electrical cord to the CD player was not secured. An area rug is frayed. Several foam floor blocks are in need of cleaning. The following was observed in the Infant/Toddler classroom: One bookshelf was not secured. The gray carpet is stained. There were fingerprints and smudges on the windows. The staff bathroom door was left open, allowing children access to a vacuum, mop and toilet brush. The following was observed in the Preschool Classroom: a screw was protruding from the brown bench. There were fingerprints, smudges and tape residue on the windows. The toilet paper holder in the children’s bathroom is not in working order.

A review of four staff records indicated the following: Two did not have evidence of a current EEC PQR registration and one did not have evidence of a current physical examination.
It was observed that the attendance record for the TODDLER 1 classroom was inaccurate with one child not signed in. An Educator immediately corrected it.
The building and fire inspections were expired.
It was observed that there was chipped and flaking paint in the following classrooms: TODDLER 1, TODDLER 2, and PRESCHOOL.

The licensor requested three staff files for review. One file was provided for review. Staff files must be made available for review upon request. The staff file reviewed did not include a current physical and evidence of immunity to Measles, Mumps and Rubella.
At the time of this visit, one educator's Background Record Checks was not current. Each person employed by the licensee, who has the potential for unsupervised contact with children, shall have a completed BRC and deemed suitable.
The staff schedule was not posted. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
The following hazards were observed outdoors and were accessible to children: The plastic Little Tykes house was not in good working order. Three plastic kitchen sets were not secured to prevent them from tipping over. The small Tykes slide was not placed 6 feet away from the railroad tie. One railroad tie was rotting and splintering. There were tree roots protruding through the sand and pose a tripping hazard. The back of the air conditioning unit was exposed in a lower-level window.
There is peeling paint on the windows sills in the Preschool and Preschool/School Age Classrooms.
A toilet training seat was observed sitting in the children's handwashing sink.

A teacher's cup of hot coffee was observed on the children's play kitchen. The licensor removed the coffee from the classroom. Hot liquids that could burn a child must be kept out of the classroom.

The attendance was not accurate in the Infant Classroom. There were five children present but only four children were signed in.
The Pack N Play cribs were not spaces two feet apart as required.
In the Infant/Toddler classroom, a toilet training chair was not emptied of urine and then washed and disinfected after use.
The water temperature was above 120 degrees.
The gate to the second floor kitchen was not closed.
The Toddler playground, does not have an adequate depth of impact absorbing material around the fall zones of the two climbing structures.
The location of the first aid kit was not posted in the Preschool classroom.
Blankets and Boppy pillows were stored in the evacuation crib. Evacuation cribs must only be used for their intended purpose.
In the first floor kitchen, there were two open packages of food not stored in covered containers as required.
Five children in the Pre-K classroom are not brushing their teeth after eating a meal. Documentation noting their parents have opted out of toothbrushing while in care was not provided during the visit.
There is peeling paint on the widow sills in the Toddler and Preschool classrooms.
The program does not have a current Building Inspection or Fire Inspection.
