It was observed in the INFANT 2 classroom that one milk bottle and two water bottles were not labeled with the child's name for whom they are intended.
During the visit, it was observed that a child received 1:1 support services by two specialists separated from their peers. A background record check was not completed for the specialists to have unsupervised contact with the child.
A review of three children's records indicated that one did not have evidence of a recent progress report.

A review of three staff records indicated that one did not have evidence of completing the Essentials 2.0 training.
It was observed OUTDOORS on the Infant/Toddler side that there were four groups present at the same time with 29 children present. This exceeds the number of children allowed in the space.
The following was observed and accessible to children: 1) In the INFANT 2 classroom, there was an Alexa cord unsecured. 2) In the TODDLER 1 classroom, the zigzag soft mat was torn. 3) In PRE-K upstairs, there was chipped paint on the green wall.
It was observed throughout the program that the list of medications, allergies, and medical conditions was not accurate and up to date.
It was observed that one over-the-counter medication was not stored with its original manufacturer's packaging.

A review of medication documentation indicated that three did not have evidence of a parent medication consent for staff to administer medication in case of an emergency.
A review of three staff records indicated that all three did not have evidence of required professional development training hours completed per year.
During the visit, it was observed in PRE-K that the trash container was uncovered and contained discarded food items. Garbage must be kept in lined and covered containers.
During the visit, it was observed in the TODDLER 1, TODDLER 2, and PRE-K classrooms that not all of the children's bottles or cups were labeled. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.

It was observed that there was chipped paint in the PRESCHOOL and PRE-K classrooms.
In the TODDLER 1 and 2 classrooms, the refrigerator temperatures were not maintained between 32 to 40 degrees.
The following was observed: both play areas did not have any equipment to provide shade and the chain link fence gates had protruding and rusted screws; a potential entanglement hazard.

The following was observed: The Infant classroom was out of ratio when each Educator left to use the restroom, leaving one Educator alone to supervise five infants.
It was observed in the Preschool classroom that two children were climbing and jumping off of furniture and running around the classroom. The Educator in the classroom made minimal effort to redirect the children.
A current staff schedule was not posted.
It was observed that children in the PRESCHOOL classroom were not accompanied by an Educator when using the bathrooms located in the hallway.
It was observed in the PRE-K classroom, that children's lunches were left uncovered on the tables for approximately up to ten minutes while the group was outside.
The following was observed and accessible to children: In the TODDLER 2 classroom, the play nursery was not secured and tippable. In the PRESCHOOL classroom, there were tears in the rest mats. In the PRE-K classroom, the play kitchen was not secured and tippable and their secondary exit route outdoors was obstructed by a playset, play materials, and rest cots.
The current building inspection did not include the inspector's name or signature.
The following was observed and accessible to children: In the PRE-K CHILDREN'S BATHROOM, there was an open box of gloves. In the TODDLER 1 classroom, there was a roll of plastic bags located in an unlocked cabinet.
A review of three Individual Health Care Plans indicated the following: One was not fully completed to indicate which staff were designated and trained to administer the medication and did not include a parent's signature. Two did not have evidence of parental medication consent.
