Provider did not adequately supervise a [REDACTED] when she disclosed during the visit that she had rested the [REDACTED] in the port a crib inside the house while she was outside with the other children due to the [REDACTED] crying.
The provider was not holding infants while they were drinking a bottle.
There was a power strip and cords accessible in the living room area.
There was no gate on one side of the stairs and the stairs were accessible while children under 3 were present.
