It was observed in the INDOOR GYM that there was chipped paint along two walls. The Program Director was aware and had a pending work order.
A review of eight IHCPs indicated that four did not identify which staff were designated and trained to administer medication in case of an emergency.
A review of the staff meeting log indicated that there was no documentation of a meeting held in July 2024.

A review of the evacuation drill log indicated that one was not conducted last month in September.
A review of eight medication consents indicated that one did not have a parent's signature.
It was observed throughout the OUTDOOR play areas that there were entrapment hazards underneath the fencing and between the gates. The Program Director was aware and had a pending work order.

The building inspection was expired.
The following was observed: There were cracked floor tiles in Preschool 1 and PreK. The baseboard trim near the sink was peeling off the wall and an outlet was uncovered in the Preschool hallway. The large rug had frayed and ripped edges and the play structure had stained carpeting in the Indoor Gym.
It was observed that there was an expired whole milk jug in the Preschool 1 classroom and an expired oat milk carton in the School Age classroom.

A review of four children's records indicated that one did not have evidence of an updated enrollment packet and current physical examination.
A review of five staff records indicated that two did not have evidence of a current PQ registration with the Department.
It was observed that there was chipped paint in the following classrooms: TODDLER 2, PRESCHOOL 2, and PRE-K. The program is in compliance with their lead inspection.

The playground with the small house had very minimal shade when assessed at 10:30 AM. The outdoor play area must provide for both direct sunlight and shade.
The file for a child with an IHCP and medication did not include a medication consent signed by the parent, authorizing medication administration.
The following were observed indoors: -There was chipped paint and plaster in T1 and P2. There was chipped paint in several rooms throughout the building. -T2-- Stained ceiling tiles. -P3- A curling rug, creating a trip hazard. A cabinet that one bolt is not secure in the wall. -P1- There was a cabinet that was tippable. -GMR- A piece of baseboard trim was missing, exposing plaster.
One child's IHCP was not signed by the parent.

The playground was not completely assessed due to poor weather conditions and rain. However, one slide was on grass and did not have adequate ground covering. In addition, on the infant playground, the green rug/ ground covering was ripped and buckled, creating a trip hazard.
In Tod 2: the diaper table was not directly beside water.

The following were observed: -Waddler: One shelf was tippable. -Toddler 2: One shelf was tippable and a sink cabinet drawer was off the track, causing it to protrude at an angle. The trash can was soiled. -PS 1: There was exposed plaster and gloves accessible in the bathroom.

A review of three staff records indicated the following: one had an expired EEC PQ registration, and no evidence of a staff orientation completed, one had an incomplete staff information form, one did not have evidence of immunizations, and all did not have evidence of consistent bi-monthly observations.
A review of three children’s records indicated that one did not have evidence of a current physical examination.
Chipped paint was observed and accessible to children throughout the classrooms.
It was observed that a thermometer was not located in the Preschool 1 refrigerator.
In the Preschool 1 classroom, it was observed that there were cleaning solution bottles located underneath the bathroom sink in an unlocked cabinet.

INFANT/ TODDLER PLAYGROUND: The outdoor infant and toddler room has significant issues with the green outdoor covering. There are many rips, holes and the fabric is buckled, creating a trip hazard. This was previously cited on Visit # 113245 on 9/16, 2021. The compliance plan for that visit stated "I have made a request to facilites to address the problems with the infant playground. They will send someone out to access and make repairs." Also noted today on the I/T playground: there is a place where the fence is buckled and pulled away from the fence. There is vegetation growing into the play area, that would be accessible to children. There was also a folded over section of green rug on the playground that could be a hazard for children. PRESCHOOL PLAYGROUNDS: There was trash stuck into the top of the fence. There was a broken white water pipe, which had a sharp edge. *** PLEASE SUBMIT A FULL CORRECTION PLAN, INCLUDING DATES OF ANTICIPATED CORRECTION FOR ALL AREAS. Please submit pictures when all areas are corrected.

A review of progress reports for one child indicated that they did not consistently have any date on the report. It could not be determined how often the progress reports were completed.
A review of 3 staff files indicated: -One did not have evidence of a current PQ Registry. -Bi-monthly observations were not consistently reviewed with the employee observed. In addition, observations in staff files were not always individualized for each staff.
On one area outdoors that was used for play space, the weed barrier was protruding and presented a trip hazard. Please submit pictures to show this is resolved.
