A review of three staff records indicated the following: 1) One did not have evidence of a staff information form. 2) One did not have evidence of their application/resume or interview date. 3) Two did not have evidence of reference checks. 4) One did not have evidence of a PQR. 5) Two did not have evidence of a current physical examination and MMR immunization records. 6) Two did not have evidence of a program orientation completed.
