A review of one IHCP indicated that it did not identify which program staff were designated and trained to administer medication in an emergency.
Two of four staff records were available for review onsite. Of the two, one did not contain evidence of a staff information form, application/resume, documentation of their interview, or a current physical examination. The second record did not contain evidence of a completed program orientation. The other two staff records were requested electronically.

A review of two staff records indicated that one staff completed an online training only. First aid/CPR training certificates for two staff were sent electronically.

At the time of the visit, the program hadn't applied for license renewal at least than 30 days before the current license's expiration date.
A review of two staff records indicated that both did not have evidence of a program orientation completed.

There was no evidence of an ongoing record of injuries or incident log.
There was no evidence of staff monthly staff meetings documented.
A review of staff records indicated the following: Two did not have evidence of a current physical examination, one did not have evidence of a current EEC registration, and two did not have evidence of current First Aid and CPR training completed.
The Site Coordinator stated during the visit that they have not conducted an evacuation drill since they opened.
