The licensor reviewed 4 children's files with the following findings: 3/4 children have outdated permissions and development histories on file; 3/4 children do not have a current progress report on file; 2/4 children do not have a physical, record of immunizations, and lead test results on file; 1/4 children have an outdated physical, record of immunizations, and lead test results on file.

A current building inspection certificate is not posted or available at the visit.
The licensor made the following observations: the Preschool 1 classroom first aid backpack is accessible to children on the top of a low bookshelf; the Preschool 2 teacher's personal bag is accessible to children in the unlocked classroom cabinet; 2 electric outlets do not have safety covers in the Preschool 1 classroom; the preschool bathroom has a potty chair filled with urine; a long cord taped to the floor of the gross motor room is exposed in several areas posing a tripping hazard; there was no first aid kit in the gross motor room when 2 classrooms were present; electric outlets in the gross motor room do not have safety covers.
The licensor observed the following: a labeled spray bottle of sanitizer and an unlabeled spray bottle of clear liquid accessible to children on the top of the Preschool 1 microwave; a Mrs. Meyers spray bottle accessible to children in the preschool bathroom.

The heating system is not functioning properly, and the temperature of the licensed space is 54 degrees Fahrenheit.

Administration did not follow the program's emergency preparedness plan; specifically, the documented procedures to implement during the loss of heat experienced on 2/18/2026 and 2/19/2026 (the day of this visit).
The files of 2 staff members were reviewed with the following findings: 1/2 staff do not have documentation of interview; 2/2 staff have an expired PQR form on file; 1/2 staff do not have documentation of immunity to MMR; 1/2 does not have an EEC Essentials 2.0 completion certificate on file; 1/2 staff do not have an EEC Program Orientation form on file.

While observing the Preschool 2 classroom the licensor noted that there were 3 children in the Preschool 1 classroom awake and on rest mats unattended while their teacher was in the bathroom outside the classroom with the other children for approximately 5 minutes; when the licensor entered the Toddler 1 classroom there was 1 teacher with 6 toddlers for approximately 5 minutes when another teacher entered the classroom.

2 structures that have slides are tilted and on an angle; both structures do not have a 6' fall zone in all directions. Due to the amount of snow on the ground the licensor was not able to determine if there is an adequate amount of impact absorbing materials in the fall zone of both structures.
The licensor made the following observations: a portable heater in use in the office during the visit; Toddler 1 classroom has an uncovered electrical outlet and an accessible drawer with adult scissors; Toddler 3 classroom has a drawer that is accessible with adult scissors; Toddler 2 classroom has drawers accessible with a stapler and adult scissors; one bathroom has plastic bags accessible in the green drawer and a spray bottle not clearly labeled; Preschool 1 classroom has a toy kitchen that is not secured and poses a tipping hazard, 1 uncovered electrical plug next to the shelf unit; Preschool 2 classroom cubbies are outside the room and a closet in the hallway is accessible to children that has a container with a lighter on the bottom shelf.

The first aid emergency backpacks in the following classrooms do not have all EEC required items: Toddler 1, Toddler 2, Toddler 3, Preschool 1. Preschool 2 classroom backpack should also be checked to ensure all required items are there. 1st Aid/Emergency backpacks was accessible to children in the Toddler 1 classroom.
There were a number of places throughout the program that had peeling paint or exposed/crumbling plaster (Preschool 1, Preschool 2 and Pre-K).

At the time of the visit three staff files were reviewed and the following was found: Staff #1 was missing observations, 2022 evaluation, documentation of 2022 professional development, and BRC suitability letter. Staff #2 was missing observations. Staff #3 was missing first aid and CPR certificate, observations, 2022 evaluation, and BRC suitability letter.
At the time of the visit there were numerous napping mats in PS1 and PS2 in need of replacing due to tearing.
This investigation inspection recorded no violations or advisories.

Proper evacuations procedures were not posted at every exit throughout the program.
One child's emergency medication was not in its original packaging with the prescription label.

Supervision of Children
The licensor made the following observations: push pins are being used on the Toddler 1 classroom bulletin board; a teacher's purse was accessible to children on the sink counter; a cubby unit outside the preschool 2 classroom is not secured and poses a tipping hazard.
The licensor observed that a spray bottle with pink liquid in the Toddler 1 classroom is not labeled and a spray bottle with purple liquid in the Preschool 2 classroom is not labeled.
The licensor observed a toddler 1 staff member diaper 2 children; the staff member washed the children's hands and changed her gloves after each diaper change but did not wash her own hands after each diaper change.

3 staff files were reviewed with the following findings: 2/3 staff do not have evidence of immunity to MMR on file; 1/3 staff only have evidence of 1 dose of MMR on file; 1/3 staff do not have a current physical on file; 1/3 staff do not have documentation of interview on file; 1/3 staff do not have verification of 2 references on file.
Licensor noted that rest mats in 2 classrooms are ripped. All rest mats in the program should be examined for rips or tears and replaced as needed.
Building certificate expired 10/1/2023.
Location of first aid kits is not posted in the Toddler 1 and Toddler 2 classrooms; location of the program's health care policy was posted during the visit.
Licensor observed a staff member apply sunblock to 2 children's faces in the Toddler 2 classroom without the staff member washing their hands between applications.
The Licensor observed the following: the door to the supply closet next to the kitchen is open during the visit and cleaning supplies on the bottom shelf are accessible to children; Spray bottles of cleaning products are not labeled in the following areas: Toddler 3 classroom, bathroom with the changing table, Preschool 1 classroom.
EEC IHCPs for 2 children are not on file; 1 medical action plan for a child with a documented allergy/ medical condition is not on medical practice letterhead and does not have a medical professional stamp.
Door to kitchen is open during the visit and licensor observed plastic bags and knives accessible to children in a cabinet and a drawer.
At the time of visit two children with emergency medical conditions were reviewed and the following was found: both IHCP forms were missing EEC required information.
