1. Two staff files did not have a current physical. 2. Two staff files did not have a current PQ Registry.
There was not a sufficient number of materials accessible in the Infant Classrooms.
An infant child was not being held while being fed a bottle. The child was drinking their bottle while laying on a soft mat.

Individual Health Care Plan (IHCP)
1. One child's first aid/emergency consent form was not current. 2. One child's off-site permission form was not current. 3. One child's transportation plan was not current. 4. One child's developmental history form was not current. 5. One child's file did not have a current physical, immunizations and lead screening.
The attendance was not accurate in the Toddler A Classroom. A child transitioning into the classroom was not signed in.
Evacuation procedures and diagrams were not posted by each exit.
A child's emergency lifesaving medication was not immediately available.
The licensor noted that the program’s daily attendance method was electronic. The program was using an IPad/software to record the children’s arrival and departure time. EEC does not have a policy at this time that allow the use of electronic software.
The licensor noted wooden cylinders, acorns, and small loose beans in the toddler classroom that were not age appropriate and posed a potential choking hazard to toddler age children.
At the time of the visit, the licensor observed the following: Toddler Room: Electrical wires for the fish tank were not secured and out of reach to children. Adult size sicssors were easily acceptable to children. Preschool bathroom: A diaper changing mat was not stored in a sanitary manner.

The licensor noted two gates that flanked the infant rooms entrance and exit doorways. The gate had a bar that spanned across the bottom which prohibited the emergency crib to move freely through the doorway.
