The evacuation log for October did not include the time of the drill, exit routes used or effectiveness of the drill The months from January - May was not an evacuation log but was written in the form of notes.

A review of staff records, Licensor noted that one educator was missing staff observations, date of interview.
A review of attendance, Licensor noted that one child was not signed out in September and one child was not signed out on 10/29/24 in the toddler room.
Three children were missing the parental consent forms for the medication.

A review of children’s files was conducted and the licensor noted the following: 1. Two children’s face sheet enrollment forms were not signed and dated by the parent/guardian. 2. A child with an IHCP form did not have a doctor signature and date. 3. A child’s first aid medical consent form did not have a parent signature and date. 4. Photos of children are not in files. 5. Off site permission form did not list the specific locations the children will visit or walk to. 6. One child did not have [REDACATED] results documented on their [REDACTED] exam.

The program is using plastic serving dishes and cups without the proper ability wash and sanitize these items. The program does not have a dedicated sink these items or dishwasher on the premises.
Brooms and dust pan were accessible to children. The infant room eating area was next to diapering station.
