The Allergy/Medical Condition List included a medication for a child for a mild reaction that was not noted in the child's IHC Plan.

A sampling of children's files was reviewed. Two children's files did not include evidence of a current Lead Test and results.
The door to the kitchen was open into the Lady Bugs classroom. There must be a barrier between children's activity space and the kitchen.

One exit in the Sensory and Lady Bug classrooms did not have a written evacuation plan posted at the exit.
One child's IHC Plan included administration of a medication should a child have a minor allergic reaction. The parent has not provided this medication and required paperwork to the program.
1) Two children's Individual Health Care (IHC) Plans were not signed and dated by a parent. 2) One child's IHC Plan included a medication dosage of 8 ML. The program did not have a medication dispenser with 8 ML noted.
This follow-up visit inspection recorded no violations or advisories.
Physcial Facilities
All practice evacuation drills were conducted between 9:25 AM and 12 PM. The program must practice evacuation drills during all hours of operation.

One child's Individual Health Care Plan was not signed/dated by the child's doctor.
A sampling of children's files was reviewed. 1) One child's enrollment forms were dated over one year old. 2) Two children's Children's Enrollment Forms (Face Sheet) were signed/dated by the parent over a year ago. 3) One child's Transportation Departure Plan was blank. 4) One child's Medication Consent form included 2.5ML medication dosage. However, the most recent IHC included 5 ML dosage. Dosage information needs to be consistent.
A sampling of staff records was reviewed. One staff file did not include a current annual evaluation.
In the Lady Bug classroom, there was a tri-side easel located at the egress to the playground. This was partially blocking the evacuation route. Exits and evacuation routes must be kept clear of obstructions.
The program has City Fresh as its meal provider. The program did not have a current 2024 Health Permit for City Fresh. If the program provides food that is prepared off-site, the food must be prepared at a facility that has a Food Service permit. Please email EEC when the permit is uploaded to the document library.
One child's physical form included that the child had a chronic condition and medication. The parent has not provided this medication or required paperwork to the program.

1) One child's physical included that the child has severe allergies and has 2 emergency medications. The child's parent has not provided these medications to the program along with Medication Consent forms (one for each medication) and an Individual Health Care Plan. 2) The dosage to be administered was unclear for one child's emergency medication. 3) One child's emergency medication did not have a dispenser with the specific dosage to be administered.
In the Bumble Bee classroom, written evacuation plans were not posted at the exit.
