The Medical Consent Form for the current child with medications included 2-4 puffs of the medication as needed. However, the prescription label included 2 puffs of medication. Documentation needs to be consistent.

1) In the Infant/Toddler classroom, there was an air freshener spray bottle accessible to children. 2) In the Infant/Toddler classroom, there was a drawer accessible to children that included hazards that may cause injury to children. The safety lock on the drawer was broken.

Three snack items had been transferred from their original box to clean and covered containers in the kitchen area. The program did not have documentation of the expiration dates for the 3 food items. EEC was unable to determine the expiration dates for these 3 foods.

A plastic can of disinfectant wipes were stored on a shelf accessible to children. This was corrected at the time of the visit.
1) The parent provided training for the administration of one child's medication. The IHC Plan needed to include this information with the doctors approval. 2) One child's IHC, medication consent form and prescription label included different dosage information. All information must be consistent.

In the Infant Classroom, EEC observed a crib and bouncy seat obstructing the far classroom exit. Exits and evacuation routes must be kept clear of obstructions.

The evacuation drill log was reviewed. It was noted that the effectiveness of the drill (time to evacuate) and the exit route used was not included in documentation.

At the time of the visit, the Assistant Director had locked their keys in the Program Director's office. Due to this, staff files were unavailable for review at the time of the visit, therefore, a sampling of First Aid certification was unable to be reviewed.

In the Pre-K (UPK classroom), the attendance presented to EEC had 2 children marked present. There was one child present. The program must have a method of knowing which children are present at all times during the day.
Due to the following, the staff did not ensure a child's health and safety at all times. A child in the program has a life-saving medication. According to the Assistant Director, the only staff member trained on how to administer the medication was the Program Director, who was on vacation at the time of the visit. The child was present at the program at the time of the visit.
There were three Individual Health Care Plans (IHC) that did not include staff who were trained and who trained them.
1) One child's file had a Medication Consent form that was not signed/dated by the parent. 2) One child had a medication, and the child's file did not include a Medication Consent Form.

The lock on both of the playground gates was broken. Please submit a written purchase order or work order to EEC confirming that the locks have been repaired and are locking properly OR that new locks have been installed in working order.
There were dirty toys stored in the Infant 2 handwashing sink.

A sampling of staff files was reviewed. 1) One staff file was missing a bi-monthly observation which was due on 7/23 and evidence of completion of Strong Start 12 Essential Trainings. 2) One staff file did not include a 2nf MMR or proof of immunity. 3) One staff file was missing the bi-monthly observation that was due 8/23.
In the Infant/Toddler classroom, 3 sippy cups did not include the child's name. In the Infant classroom, there was one bottle that did not include a child's name. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, and sleeping materials, must be labeled with the name of the child for whom they are intended.
In the Infant/Toddler classroom, there was an open box of Cheerios. This opened box/bag was not being stored in a covered container.
The Allergy/Medical Condition List did not include one child's condition, their medication and to Call 911 should the child show signs of their condition.
