The licensor observed the following: Toddler 3- paint on the wall, chipping, and peeling Preschool 3 -2 holes in the wall, peeling and chipping paint within the classroom. Preschool 1- chipping paint Toddler 2- hole in the wall by the children's bathroom Toddler 1- the changing pad on the changing table is torn, and chipping and peeling paint in the classroom Infant/Toddler classroom-changing pad on changing table, torn, small hole in the wall, and peeling and chipping paint.

The licensor observed whiteout on a shelf in the Preschool 1 and Preschool 4 classrooms that was accessible to children.
The licensor noted that the multiple IHCP Individual Health Care Plans did not indicate who was trained and by whom. Additionally, one child's IHCP was missing from the program.
The licensor noted that in multiple classrooms, the children's bottles and cups were not labeled.
The licensor observed the following: Preschool 4 classroom- White-out on a shelf accessible to children Listerine mouthwash and oral rinse in a drawer accessible to children Preschool 1 classroom- white-out accessible to children Infant 2 classroom- Lysol was accessible to children under the unlocked handwashing sink.
Staff meeting documentation did not note the times of the meetings.

Observed in Infant/toddler classroom- paint peeling at base of wall; duct tape around cabinet doors; sink full up to faucet with dirty dishes; minimal amount of toys and materials visible as options for children to choose.
2 children not signed in on the attendance for classroom Infant 1; those 2 children were still signed in on the attendance for Infant 2 even though they were combined with Infant 1 children.

At the time of the visit the following items were observed: the ceiling vents in P4 had dust hanging out of them, in P4 and electrical outlet was not covered, there were water damaged ceiling tiles in a number of places throughout the program and in T1 the wall behind the trash can had food splatter on it.
At the time of the visit the P2 classroom had 5 children in attendance however only 4 were signed in.

At the time of the visit there were numerous resting mats in T2 that had tears on them and were in need of replacing.

At the time of the visit 3 children with emergency medications were reviewed and the following was found: one child's medication had an expired "discard by" date
At the time of the visit 3 children with emergency medical conditions were reviewed and the following was found: one child had an IHCP that was missing EEC required information and one child did not have an IHCP for their condition.
At the time of the visit an educator's purse was on a child table in the T2 classroom, accessible to children.
