There are some water damaged ceiling tiles. 4. There was not a lock on the cabinets underneath the changing table, in the Infant Classroom, and there were cleaning solutions in there. 5. The cabinet between the Toddler 1 and Toddler Classroom is loose and there is a metal piece sticking out at the bottom.

A review of 5 staff files, Licensor noted the following: 1. One educator had an expired PQR dated 5/17/24. 2. One educator missing all EEC pertinent information on the EEC staff records checklist. 3. One educator in ratio hired on 7/24 did not have date of interview, had a blank program orientation form, no evidence of a physical examination, or MMR immunizations. 4. One educator was missing documentation of interview, date of hire, had a blank program orientation form, and no evidence of a physical examination, or MMR immunizations. 5. One educator in ratio had no evidence of date of interview, teacher qualifications, EEC 12 hours Essentials, PQR, physical examination, or MMR immunizations.
