One child’s [REDACTED] had expired resulting in the emergency medication not being readily available.
The licensor noted that all three Building Inspections were expired. Upon receipt of the inspections, please upload the current inspections into the portal library.
1) A sampling of past attendance sheets were reviewed and one school age child’s departure time was not recorded. Classroom attendance must be accurate at all times. 1) At the time of the visit, the school age classroom attendance did not accurately reflect the number of children present. 2) At the time of the visit, in the Preschool B classroom, a child's departure time was not recorded.

The school age classroom was observed and the following was found: 1) A telephone cord longer then 6 inches was easily accessible to children. 2) The main classroom door was not in working order. It required multiple forceful tugs to open which causes a potential safety hazard. 3) A metal strip from the front of the heater was missing, therefore, exposing pipes. 4) A trash bin containing food was missing a lid. The Preschool B classroom was observed and the following was found: 1) An extension cord longer then 6 inches located next to the fort and an extension cord in the bathroom hallway were not secured to the wall and posed a potential safety hazard. 2) The bathroom had several stained ceiling tiles. 3) A mop and squeegee broom were easily accessible to children in the bathroom hallway.

A current children's records checklist for both preschool and school age was not available for review. [REDACTED] children's files were reviewed and the following was found: 1) Two children’s files were missing their date of admission. 2) One child's transportation plan form was blank. 3) One child's medication consent form was incomplete. 4) Parents signatures and dates on a child's records were over a year old. Records should be signed annually.
The director informed the licensor that the preschool and school age staff records checklist was not current. Please update and upload to the portal library. A sample of staff records were reviewed and the following was found: 1) A staff file was missing documentation of an interview, verification of two references, proof of a current physical, and evidence of two MMR's or proof of immunity. 2) Two staff files were missing the required number of professional development hours, bi-monthly staff observations, and annual staff evaluations.
A child under the age of 7 was observed returning from the bathroom unsupervised.
Chipping paint was observed on the metal heaters in the school age classroom.
The fire drill log had the following missing information: 1) There was no written documentation indicating that practice fire drills were conducted between 1/19-6/19. 2) The evacuation drill documentation for 8/19 did not include the time of day the drill was conducted. 3) The route used in 9/19 drill was not documented. 4) The 10/19 drill documentation did not specify the date the drill was conducted, the evacuation route used, and the number of children who participated in the drill. 5) The effectiveness of drills was not documented.

[REDACTED] children's files with medication were reviewed and the following was found: 1) One child's Individual Health Care Plan was over a year old and did not included all EEC required information. 2) One child with a chronic condition was missing an Individual Health Care Plan.
