The educator did not have the child specific posting for a child with asthma and a prescribed inhaler.
All of the children's records were reviewed. Seven children need an annual update form, three children need an updated physical, one child needs proof of a physical, three children need proof of lead testing and one child's enrollment packet did not include their date of enrollment.
The educator did not have the written parental consent to administer an (REDACTED) medication.
One of the inhalers on the premises did not have its original container with the original label affixed. The (REDACTED) on the premises was expired.

The emergency cards of one of the children in the first aid kit was missing a parent signature.
In the kitchen, several drawers allowed access to a lighter, charging wires longer than 6 inches, cork screws and knives. A garbage bag was open and accessible on the floor in the kitchen. In the outdoor space, the stairway did not have the required gate. There was a loose water hose and the fire place on the deck was uncovered.

The first aid kit was missing a thermometer, scissors, tweezers and two of the children's emergency medical cards.

In the bedroom the following items were accessible to the children, gloves, plastic bags, diaper cream, hand sanitizer and lotion. In the kitchen, several of the locks were broken allowing access to a pizza cutter, Vaseline, lotion, wires, and a lighter. In the outdoor space, sitting water was observed in several of the toys.
Four of the eight children's records were reviewed. Three children required an annual update form, one child required lead testing and one child's general permission form did not include the provider's name.

Provider and the children were observed outdoors without the first aid kit.
Several slides were observed to measure over 18 inches without a fall zone.
Provider was present with seven children without an approved assistant.
