In the kitchen, several drawers had safety locks that were not functioning. This allowed access to sharp knives, scissors, grater, plastic bags and plastic gloves. In the hallways a mop bucket with sitting water was accessible. In the outdoor space, two toys had sitting water accessible to the children. A large ride on lawn mower was accessible in the outdoor space.

In review of attendance records and children's records it was observed the educator has four children under the age of two enrolled. The educator cared for four children under the age of two from April 14, 2026 through June 04, 2026 and then June 17, 2026 through present. One educator must not care for more than 3 children under two years of age, including at least one toddler (15 months) who is walking independently. Additional children must be older than 24 months. During this visit, the educator was in compliance caring for four children, three children were under the age of two years old with one child being older than fifteen months and walking independently.
In the play area, a section of the wall was observed with chipped and peeling paint.
The educator has not been documenting daily attendance since opening their program in July 2025.
Two of the children did not have completed progress reports.
In the living room, the television stand drawer had medication accessible to the children. A speaker with a loose wire was also accessible in the living room. In the bathroom under the sink cabinet there were several cleaning products and personal hygiene items accessible to the children. In the kitchen there were several cabinets with cleaning products, a string of lights, and sharp utensils accessible to the children.

One of the outdoor play houses had dirt all around the home, one of the window wells did not have a covering, a stairway was accessible on a piece of equipment.

All of the children's records were reviewed. Four of the children need a date of admission completed on their enrollment packet, two children have several sections of the enrollment packet incomplete, three children need proof of physicals, vaccinations and two children need proof of lead testing.
The first aid kit did not include the children's emergency medical cards.
The educator did not have documentation of the fire drills reportedly conducted since the educator opened the program in July 2025.
The educator did not have sheets for the playpens.
