Through a review of children's records, one child's record didn't have evidence of a date of admission.
Through review of three staff records the following was noted: 1. There wasn't evidence of staff observations. 2. There wasn't evidence of staff evaluations.
The program did not have a method of knowing where children were at any given time when the attendance record did not accompany the groupings of children when they went to the playground.
