One child was not signed in, and one child not signed out in infant/toddler room.
The black fence on the preschool playground had a gap between the building and the fence pole, causing an entrapment. The back left wooden fence panel was weathered, splintering, and chipping. Along side of the building on the preschool playground, the cement was cracking/breaking in areas causing jagged sharp cement in reach of the children. There was not an adequate amount of pebbles in the fall zone around the slide.

On the day of the visit two teacher certified educators were in the front room with 4 toddlers. A teachers assistant who is not certified was alone in the back right room with 2 toddlers and 1 infant.
This renewal - monitoring visit inspection recorded no violations or advisories.
The changing mat was beyond repair. It had multiple tares and was discolored.
Broom, mop, toilet brush and plunger were observed in the Infant/Toddler bathroom and a broom was observed in the infant area on the rug.
The fence on the preschool playground was wooden it had chipping wood, rotted wood and holes. The metal fence had large gaps on the bottom. The gate had a head entrapment.

During the visit, the fence on the deck was measured and could be a head entrapment. The slots in between the rails were between 3.5-5 inches.

The children’s sleeping materials were kept in-between stacked cots. Some blankets were touching others.
The program did not have a log of evacuation drills done for the year. The form provided did not have route taken.
A bottle was on the counter in the Infant/toddler room with no name on it.
