Individual Health Care Plan (IHCP)
While observing the educators with the children, one educator was observed outside laying down on the ground in the playground next to one child, while several other children where playing on the structures and swing. The educator was not observed interacting or engaging with all the children.
Three medications reviewed were expired. Benadryl, Ventolin and Albuterol.
Expired IHCP for seizures. Missing IHCP for child with [redacted]. IHCP for child with [redacted] did not have names of educators trained and who trained them. IHCP for child with Allergy to [redacted] expired, doctors IHCP was not complete.

The climbing structure does not have an adequate depth of impact absorbing material. The poured rubber is damaged and missing large pieces under the slide and around the climbing structure.

The playground had a poured surface. Some areas were broken and peeled up, which could cause a tripping hazard. There was a 6 foot velcro strap hanging from the climber, which could be an entanglement hazard.
The school has not provided a current IPM Plan yet.

Individual Health Care Plan (IHCP) - R Apr 2025
During the inspection of the outdoor play space it was learned that the poured ground covering has buckled in spot, creating a tripping hazard. There are places under equipment, that the poured ground covering is coming apart and places that the top layer has peeled off, leaving the integrity of the ground covering compromised.

During the visit it was learned that not all staff have current completed BRC checks on file.
On the day of the visit there was not evidence that the educator holds practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
There was a missing IHCP for a child who requires an Epi-Pen. Two of the IHCP's were missing doctor signatures and did not have an attached doctors form with all EEC required information.
The Health Inspection was expired.
During the review of Individual Health Care Plans (IHCP) it was learned that not all plans contained all the required documentation.
