General: Based on review of documentation, current proof of liability insurance was not available during the inspection. The available copy was dated 11/19/23 to 11/19/24.
General: Based on review of documentation, two (2) staff members (DOH 07/22/23 and 07/23/23) did not have current evaluations on file.
General: Based on review of documentation, two (2) staff members (DOH: 07/12/24 and 05/29/24) did not have a proof of education in their personnel files.
General: Based on review of documentation, the following was found: 1) An earthquake drill was not documented as being conducted in the second quarter of this year. 2) On 09/15/25 it was documented that a fire drill and lockdown drill was simultaneously conducted at 4:10 pm. These drills require different actions and cannot be conducted at the same time.
General: Based on review of documentation, fire drills were not documented as being conducted in January and February of 2025.
General: Based on interview and record re the facility failed to notify the parent of an incident that resulted in a fractured wrist. A child fell on the playground at 11:07 am and complained of pain in her wrist. The staff members present indicated did not call the parent because they did not have a phone. General: Based on observation at the time of the inspection, all the children were combined into one (1) group. The children's ages ranged from five to eleven (5-11) years. There were thirty-eight (38) children and two (2) staff present. Therefore, the staff-to-child ratio and maximum group size were exceeded by eight (8) children. Based on observation at the time of the inspection, all the children were combined into one (1) group. The children's ages ranged from five to eleven (5-11) years. There were thirty- eight (38) children and two (2) staff present. It should be noted that during the observation, the two (2) staff members were working with all the children and it could not be determined which staff member was responsible for what children. A group shall be separately maintained in a defined area unique to the group and have a specific staff member assigned to be responsible for the group.
