General: Based on observation, the surveyor found that a large section of the green sunshade on the playground was ripped/torn and part of the sunshade was observed to be slightly hanging down; therefore, the sunshade was not in good repair and the playground was not well maintained. During interview, staff-in-charge stated that a new sunshade has been ordered.
