General: Based on observation and review of documentation, a list of the children's medical conditions and names were posted on the wall, on an uncovered sheet within the Pre- Kindergarten Classroom. There was no written evidence to indicate that written permission had been granted to post the medical information.
General: Based on observation, the following items were found to be accessible to the children in care: Two-Year-Old Classroom: 1. Several diaper creams were stored in an unlocked cabinet. Three-year-old Classroom: 1. Bleach spray was stored on a low shelf. 2. Diaper changing creams and cleaning chemicals were stored on a low toy shelf. 3. Cleaning chemicals were stored under the sink. Pre-Kindergarten Classroom: 1. Several bottles of prescription medication, belonging to a staff member, were placed on a table within the Pre-Kindergarten Classroom. 2. A staff member’s soft drink was placed on a toy shelf within the Pre-Kindergarten Classroom.
General: Based on observation, the following was found: Younger Two's Classroom 1. An electrical outlet was not covered, near the art center. Pre-Kindergarten Classroom 1. An extension cord with four (4) electrical outlets was not covered accessible to children in care. Three-Year-Old Classroom 1. The toilet paper was stored on the back of the toilet basin.
General: Based on observation and interview, during a tour of the child care center, the staff-person-in-charge indicated that the walls between the existing Pre-Kindergarten Classroom and School-age classroom had been removed, and that these classrooms were consolidated into one class. Additionally, the wall separating the Older Three-year-old Classroom and Younger Three-year-old Classroom had been removed. The State Fire Marshal and DRCC were not notified prior to these changes being implemented.
General: Based on observation, a child within the Preschool Classroom became ill while at the child care center on 05/11/26. The child vomited, complained of stomach pain, and registered a low-grade fever of ninety nine (99) degrees Fahrenheit. The child was not isolated from his peers upon presentation of symptoms. The child remained at the classroom table with his peers while they finished their lunches. The staff-person-in-charge contacted the child’s authorized pick-up to remove him from the center.
General: Based on review of documentation, one (1) staff member (DOH: 01/26/26) had a negative tuberculin skin result on file that exceeded two (2) years and therefore was no longer current.
General: Based on observation, children in the One-Year-Old Classroom were placed in highchairs at 11:02 a.m., the staff-person-in-charge of the classroom cleaned the toys while the children were in the chairs. The children remained in the highchairs without an activity until 11:14 a.m., when the staff-member in the classroom started singing songs and doing fingerplays.

General: Based on observation, the following was found: Three-Year-Old Classroom 1. The sink water temperature did not exceed sixty five (65) degrees Fahrenheit when measured with a thermometer. Infant Classroom 1. The sink temperature did not exceed eighty (80) degrees Fahrenheit when measured with a thermometer.
General: Based on observation, several soiled pull-up diapers were stored in an uncovered trash can within the Three-Year-Old Classroom.
General: Based on observation, the mounted sink was separating from the wall within the Younger Two’s Classroom restroom.
General: Based on observation and interviews, an allergy care plan was not posted within the Preschool Classroom, where staff confirmed that multiple children within the classroom had food allergies.
