General: Based on review of documentation, it was found during the survey that two (2) employees (DOH: 11/13/25 and 06/19/25) were left alone supervising children who were not determined to be eligible in the Kentucky National Background Check Service (KARES) at the time of survey. One staff (DOH: 11/13/25) was observed working in the Infant 2 Room alone with five (5) children ranging in age from 2-6 months-old during the renewal survey. The other staff member (DOH: 06/19/25) was observed working in another Infant Room alone with six (6) children ranging age from 12-18 months old.
