General: Based on interview, on 09/29/25, fifteen (15) children were being supervised by one (1) staff member for at least 1.5 hours from 3:30 pm until 5:00 pm. Staff advised that the youngest child in the room was thirty (30) months, and the oldest child was three (3) years old. Staff advised that there were not enough staff to operate and the child care center had to close the following week. General Administration Not In Compliance

General: Based on observation, interview and review of documentation, the surveyor was not presented with a personnel file for a staff member (DOH: 01/25/25) who was observed to be working in a classroom. During interview with staff-in-charge the surveyor was informed that there should be a file for the employee, but she could not locate it at the time. Therefore, staff-in-charge did not have documentation of background checks submitted through the Kentucky Applicant Registry and Employment System (KARES) for the surveyor to review. Based on review of the Kentucky National Background Check Service, the staff member did not have a completed background check; therefore, the staff person was hired prior to clearance for employment. During interview, staff-in-charge informed the surveyor that the staff member was hired on 01/25/25 and that a Child Abuse/Neglect Background check was completed; however, she was unable to locate the documentation. Staff-in-charge also confirmed that a background check was not submitted through the KARES and that she was unaware that she needed to submit a background check through KARES. The surveyor did not observe the staff person working alone with children.
General: Based on observation and interview, in the last stall of the girls' restroom located in the main building, the surveyor observed a training toilet that contained yellow substance that appeared to be urine. The training toilet was placed next to the commode. Therefore, the child care center failed to ensure that the training toilet was out of reach of other toilets, emptied promptly and disinfected after each use.
This investigation inspection recorded no violations or advisories.
General: Based on review of documentation, the surveyor found the following: 1. A child’s (DOE: 11/30/22) file did not contain an immunization certificate. 2. A child’s (DOE: 08/16/24) file did not contain an immunization certificate. 3. A child’s (DOE: 08/25/24) file did not contain an immunization certificate. 4. A child’s (DOE: 09/24/24) file did not contain an immunization certificate. 5. A child’s (DOE: 12/20/23) file contained an immunization certificate that was no longer current as of 04/17/24.
General: Based on observation, the surveyor found a staff (DOH: 07/31/23) file that contained documentation from KARES that the staff person was eligible for hire on 03/14/23. On the day of visit, the surveyor printed the child-care center’s KARES report and reviewed the names listed and noticed that the staff person was not listed under the child-care center’s roster and had been listed underneath the "Seperated" section on the child-care center's roster; therefore, the child-care center failed to update the staff person’s employment status. During interview with staff-in-charge, the surveyor found that the staff person mentioned was working today in Room 202 alone. Based on review of ECE-TRIS, one (1) staff person (DOH: 07/17/19) completed zero (0) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 07/01/23 - 06/30/24. 2. Based on review of ECE-TRIS, one (1) staff person (DOH: 03/20/23) completed zero (0) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 03/30/24 - 06/30/24. 3. Based on review of ECE-TRIS, one (1) staff person (DOH: 04/03/23) completed zero (0) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 04/11/24 - 06/30/24. 4. Based on review of ECE-TRIS, one (1) staff person (DOH: 04/20/23) completed zero (0) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 04/21/24 - 06/30/24. 5. The surveyor reviewed ECE-TRIS and found that the staff person had a first year start date of 03/15/23. 7. Based on review of ECE-TRIS, one (1) staff person (DOH: 08/08/23) completed zero (0) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 07/01/23 - 06/30/24. 8. Based on review of ECE-TRIS, one (1) staff person (DOH: 02/15/24) completed ten and half (10.5) hours of the required fifteen (15) hours of cabinet-approved early care and education training between 07/01/23 - 06/30/24. The surveyor reviewed ECE-TRIS and found that the staff person had a first year start date of 01/10/22. 9. Based on review of ECE-TRIS, the following staff (DOH: 04/20/23 and 07/10/23) files did not contain documentation of one and one-half (1 ½) hours of cabinet-approved pediatric abusive head trauma training; therefore, the training was not completed within one (1) year of employment. 10. Based on review of ECE-TRIS, the following staff (DOH: 04/20/23, 07/10/23, 12/18/23, 03/7/24, and 08/01/24) did not have documentation that Orientation was completed within the first three (3) months of hire. General: Based on review of documentation, staff-in-charge was unable to provide documentation of training or certification in CPR and First Aid for the two (2) individuals who were identified as bus drivers for their transportation services. The only documentation that could be provided for review were copies of the two (2) individual's drivers licenses.
General: Based on review of documentation, staff-in-charge could not provide a written record of transportation services provided in accordance with 922 KAR 2:120, Section 12. During interview, staff-in-charge stated she could not provide the documentation.
General: Based on review of documentation, the staff-in-charge could not provide personnel files on the two (2) individuals who were identified as bus drivers for their transportation services. The only documentation that was presented for review for the two (2) individuals was a copy of their driver’s licenses. Therefore, the child-care center failed to maintain personnel files as the files did not include the following: date of employment, proof of educational qualifications, documentation of compliance with tuberculosis screening, or the results of background checks. General: Based on review of documentation, staff-in-charge was unable to provide documentation to show the child-care center’s plan for ensuring staff properly perform duties related to transportation services. During interview, staff-in-charge confirmed she could not provide the documentation.
General: Based on multiple interviews, the surveyor learned that several of the child care center children were frightened/scared of a staff member because the staff member sits in the gym and yells at children in a loud, threatening tone. Through interview, the surveyor found that the staff person will also use an aggressive tone toward the children to discipline them and that the staff person becomes easily aggravated when working with the children. General: Based on review of documentation, the surveyor found that the child-care center did not have a personnel file for a staff (DOH: unknown) person. The surveyor found through multiple interviews that the staff person was the cleaning person for the church and the child-care center. The surveyor found through multiple interviews that the staff person would be present while children were at the child-care center. During interview with staff-in-charge, she stated that the staff person was technically hired through the church but would also work with the child-care center and that the child-care center did not have a file on the staff person at this time; therefore, the child-care center failed to obtain and maintain the proper documentation needed for the staff person.

General: Based on interview, the surveyor learned the following: 1. A child has run away from the area the class was in on multiple occasions. The teacher was able to catch the child before exiting the building. Therefore, the safety of the child was compromised when she was running through the child-care center. 2. A child had bitten multiple children multiple times; therefore, the health, safety, and comfort of the children was compromised. Staff-in-charge reported that she has been working with the child's parents with the biting issues, along with the other child that keeps running from the class at the child-care center.
General: Based on observation and interview with staff, the surveyor found the following: 1. While standing in the doorway of the Preschool Classroom at approximately 10:13 am, the surveyor learned through interview with staff that one (1) child had gone to the restroom alone. The Preschool Classroom was observed to be located on the second story balcony above the Gym. A railing was around the balcony. The restrooms are located on the first floor of the Gym on the opposite side from the downstairs stairwell door where the children would exit. The children pass by a set of exit doors in the Gym that lead outside when they go to the restroom. The surveyor observed the stairwell door located near the Preschool Classroom to open and a child came through the door alone at approximately 10:15 am. The child struggled to open the stairwell door. The surveyor checked the stairwell and found no staff in the stairwell. The surveyor observed two (2) classrooms in the Gym. Through interview with staff and review of the child's file, the surveyor learned the child was three-years-old and the children in the classroom ranged in age from three- to four-years-old. The surveyor did not observe a staff member standing on the balcony when going to the Preschool Classroom. Therefore, the child was not within scope of vision, range of voice, or provided with adequate supervision at all times. 2. The surveyor entered the upstairs stairwell located near the Preschool and Pre-K Classrooms at approximately 10:25 am and observed a child walking up the stairwell alone. The surveyor followed the child back to the Pre-K Classroom, which consisted of children ages three- to five-years-old. Through interview with staff, the surveyor learned the child was four-years-old and had gone to the restroom alone. Staff reported the children will go down the stairwell to the restroom and come back up the stairwell on their own. Staff reported if the children are in the restroom for over three to five (3-5) minutes, they will call for another worker to check on the child because the younger children like to play in the restroom. Staff reported the classroom also takes breaks and will go to the restroom together as a class. The surveyor observed two (2) classrooms in the Gym. The surveyor did not observe a staff member standing on the balcony. Therefore, the child was not within scope of vision, range of voice, or provided with adequate supervision at all times. 3. Through interview with staff, the surveyor learned at approximately 9:00 am on the day of the surveyor’s visit, a three-year-old child in the upstairs Preschool Classroom came out of the classroom and walked past the stairwell located near the classroom and around the balcony to the back stairwell. Staff reported the child went into the stairwell alone and a staff member went to meet her at the downstairs door and took her to the restroom. Staff stated the child reported she could not open the bottom stairwell door closest to her classroom; therefore, the child went to the other door while the staff member remained in the classroom. Staff reported the child was not scared. Through interview with staff, the surveyor learned the children in the Preschool and Pre-K Classrooms normally use the first stairwell, go into the restroom alone, and then return to their classroom. Staff reported that most of the time a teacher will stand on the balcony and look over as the children go to the restroom. Therefore, the child was not within scope of vision, range of voice, or provided with adequate supervision at all times. 4. Through interview with staff, the surveyor learned a three-year-old child who was a runner, would want to run to the ramp or the hallway during the morning. Staff reported this child would push the chairs out of the way in the Gym and then run through the breezeway door, up a ramp, and down the hallway. A staff member would go after the child and keep their eyes on the child. Therefore, the child was not provided with adequate supervision at all times. It is important to note that the surveyor measured the distance from the upstairs Preschool and Pre-K Classrooms to the stairwell door to be approximately 37.74 feet; the distance from the second story stairwell door to the first-floor stairwell door, approximately 35.48 feet; and the distance from the first-floor stairwell door to the restroom, approximately 60.52 feet. The surveyor observed there to be two (2) flights of stairs with nine (9) steps each. The surveyor measured the distance from the second story stairwell door located near the Preschool and Pre-K Classrooms to the back stairwell door to be approximately 151.13 feet and the distance from the second story back stairwell door to the first-floor stairwell door to be approximately 37.6 feet. The Fire Marshal’s report presented for review at the time of the survey was no longer current as it was dated 7/15/2019. General: Based on observation and interview, the surveyor found two boppy pillows and two blankets in a crib. The staff person stated that a 10-month-old child sleeps in the crib. She stated the child is required to sleep in a propped position due to having reflux. The surveyor found that the child-care center did not have a waiver or written request from a health professional that would require the child to sleep in an alternate sleeping position. General: Based on review of documentation, it was found that two staff members did not have proof of education on file at the facility. It was found that both of these staff members had graduated high school and had documentation that they were currently enrolled in high school, but no high school diploma was in their file after they had graduated. General: Based on observation, the following was found to be accessible to children in care: 1.) In the preschool classroom, there was a staff member's purse sitting on a dehumidifier. In addition, the staff member's food and drink was sitting on the teacher's desk along with her cell phone and a charging cord that was plugged into the wall. 2.) In an unlocked closet in the preschool classroom, there were disinfectant wipes accessible. 3.) In the front hallway leading to the classrooms, there were numerous buckets of paint, primer, and stain on the floor.
General: Based on observation and interview, this regulatory requirement was not met. Information on the dry erase board in Room #100 (preschool room), did not adequately describe the activities that children would be involved in throughout the day. The board only listed the letters and numbers that would be reviewed for the week of 10/28/20 – 11/02/20. The staff person working in the classroom stated that she did not have a lesson plan and pointed to the bulletin board as the only list of activities available. General: Based on observation, this regulatory requirement was not met. The following was found: 1. During a tour of Room 101 (toddler room), the surveyor observed a bottle of liquid soap positioned on one side of the children's handwashing sink and a bottle of hand sanitizer positioned on the other side of the sink. The hand sanitizer was accessible to the children; therefore, not a safe area to be stored. 2. A radio was positioned on top of a dramatic play kitchen set in Room 101 (toddler room). The radio was plugged into an electrical outlet on the wall beside the play structure and approximately two feet of the cord was hanging loose; therefore, accessible to the children. 3. A large fan was mounted near the ceiling in the infant classroom and the fan was plugged into an electrical outlet near the floor. Several feet of the cord (approximately three - four feet) was hanging loose and was accessible to the children.
General: Based on review of documentation, this regulatory requirement was not met. A review of staff files found that a staff person hired 9/24/18 had the results of a T.B. test read on 4/8/19. This date is after the staff person was hired and not prior to employment. There was no other documentation to verify that a T.B. test had been completed prior to employment.
General: Based on observation, this regulatory requirement was not met. In room 101 there was a tube of Desitin diaper ointment in the locked medication box that was not labeled for an individual child. In this same box there was a bottle of Walgreen brand children's pain relief inside a clear ziplock baggy. The outside of the baggy had the name of a child written on the side. However, the bottle of medication on the inside did not contain the child's name. Outdoor Play Area Not In Compliance
General: Based on observation, this regulatory requirement was not met. 1) In Room 103, which is a one-year-old room, the posted menu had raisins listed as a component for morning snack. The surveyor asked the staff person in the room if raisins are served to the children. The staff person confirmed that she does give the children raisins. Raisins are considered a choking hazard for children of this age. 2) In Room 202 there was a broken outlet cover on the wall by a basket of books. 3) In the Brown Bear Room there was an outlet that did not have a plug in the receptacle. General: Based on Interview, the director stated that she gave out confidential information to some church members regarding a child who attended the facility. She stated that on one other occasion she yelled out confidential information into the hallway from her office desk regarding the same child who attended the facility.
General: Based on Review of Documentation, it was found that a staff person hired 9/30/15 lived in the state of Ohio within the last five (5) years. However, there was no documentation on file to verify that the results of a criminal records check from the state of Ohio had been obtained. General: Based on Observation, during a tour of the two to three (2-3) year old room there were several areas on the walls that had chipping and peeling paint. In the 4's room the baseboards had an accumulation of dirt and grime.
General: Based on Observation, during a tour of the infant room an infant was observed wearing a teething necklace. An interview with the staff in the room found that there is another child in the two-year-old classroom who also wears a teething necklace. These beaded necklaces are a choking hazard. General: Based on observation, the surveyor was inspecting the girls' restroom at approximately 9:55 a.m. when a staff person entered the restroom with two (2) toddlers ages seventeen (17) and nineteen (19) months old. The surveyor noted that the diaper changing table was positioned against the wall at the entrance of the restroom. As the staff person started changing the female toddler's diaper, the male toddler ran around the corner to the hand-washing sink. He climbed up a two (2) step step stool and placed his arms on top of the countertop and proceeded to swing his body back and forth (his feet were not touching the top step of the step stool). The toddler kept trying to reach for the water faucet. The surveyor got the toddler down from the step stool two (2) times. After the staff person finished changing the female toddler's diaper, she then picked up the male toddler to change his diaper. The female toddler also ran around the corner and climbed up the step stool. The staff person could not see either toddler when they were at the hand-washing sink due to the positioning of the diaper changing table. The staff person stated that the diaper changing table was not always in the restroom. She stated that due to recent water damage, some of the classrooms had to be moved temporarily and the only place to change diapers is in the restroom.
General: Based on observation, the surveyor found a bottle of Bio-Bowl cleaner on top of a side table located in the girls' restroom. The bottle had a label that read "Keep out of Reach of Children". During the exit interview, the staff person in charge stated that the cleaner had been left by cleaning personnel from the night before. She stated that the cleaner had been removed from the restroom. (2) The surveyor observed a spray bottle labeled "Bleach Water" on a shelf in the 2/3 classroom. The bottle was within reach of the children in care.
General: Based on Review of Documentation, in the Lightening McQueen room there were five (5) children present but only four (4) had been signed in with an arrival time.
General: Based on Review of Documentation, a staff person hired 4/24/13 had a TB skin test dated 4/29/13 and a staff person hired 2/23/13 had a TB skin test date 5/4/12.
General: Based on Observation, the children in the Lightening McQueen room had their hands washed in preparation for snack but played with toys before sitting down at the table. The staff person encouraged the children to get a book while they waited.
General: Based on Observation, of the Dinsey Babies room an infant was unsupervised while in a crib in the sleeping area. A infant was taken from a swing and placed in a crib at approximately 9:32 AM. The staff person then changed another infant's diaper. While at the changing table the infant in the crib was visible to the staff person. However, the diaper change was finished by 9:38 AM and the staff person went back to the middle of the play space and interacted with other infants. When the classroom observation was over at about 9:43 AM the infant was still in the crib and not visible to the staff in the room.
General: Based on Interview, a one (1) year old child was restrained in a high chair as discipline for biting. The high chair was also used to restrain the child when the staff person was busy with diaper changes as a means to prevent the child from biting.

A large hole was found beside the outside gate to the outdoor play area. The hole was approximately one (1) foot across and approximately six (6) inches deep. The hole is in the direct path of children walking to the outdoor play area. Also, the underpinning of the annex building had been removed, exposing the under part of the building. This area is located directly across from the outdoor play area used by the children, and is accessible to children walking to the outdoor play area. The height of the exposed under part of the building is approximately two (2) feet tall. The exposure is tall and wide enough for a child to enter the under part of the building, which is supported by several cinder blocks.

The infant was also sucking on a pacifier. Interviews with staff found that there was not a medical statement on file indicating that the infant is required to sleep in the "Boppy Pillow". Staff also indicated that the infant's mother works at the facility and that the mother put the child in the "Boppy Pillow" and pulled the blanket up around the infant's face. The American Academy of Pediatrics practices the policy that blankets be kept away from an infant's face who is sleeping and that items such as pillows and stuffed animals not be placed in an infants crib.
General: Based on observation and interview, the surveyor found that the first and third toilets located in the restroom next to Room 105 were unable to be flushed. Staff-in-charge stated that the toilet handles had to be wiggled in order for the toilet to flush. The surveyor informed the staff-in-charge that, when testing multiple toilets throughout the child care center some toilets would flush after wiggling the handle; however, the two (2) toilets remained nonfunctional. During interview, staff-in-charge immediately apologized and stated that she was horrified by the finding and that her staff knew better than to leave the diapers on the floor. Through interview with staff, the surveyor learned a staff member grabbed a three-year-old child a “little too hard,” when trying to catch the child while she was running. Staff reported this left a pink/red mark that looked like an “Indian Rug Burn” or friction on the child’s right forearm. Staff reported the mark was not bright red and faded. 2. Upon entry into the child-care center, the surveyor observed children at the restroom located in the Gym. Children were observed to run past the surveyor to the stairwell door located on the opposite side of the Gym. A staff person was observed to yell at the children “Did I tell you to go in there?” Staff-in-charge reported she heard the staff person and was going to address it with them. Through interview with staff, the surveyor learned the children were approximately three- to four-years-old. 3. Through interview with staff, the surveyor learned a staff member asked staff if “anyone wanted to play catch because they were getting ready to throw a child over the banister”. Staff reported the staff member and a three-year-old child were on the balcony above the Gym at the time of the statement. Staff reported the child was having a “meltdown” and the staff member was frustrated. Staff reported they thought the staff member made the statement as a joke. Staff-in-charge stated the staff member was given a verbal warning. 4. The surveyor observed an eight-month-old child in the Nursery to be placed in a highchair with no buckles. The surveyor observed four (4) highchairs that did not contain buckles. Through interview with staff, the surveyor learned the highchairs have not contained buckles since approximately July 2023. 5. The surveyor observed a red half circle table that contained seats with buckles in the Mobile Infants Classroom; however, two (2) of the seats were observed to not contain buckles. The table measured approximately thirteen and one-half (13 ½) inches from the floor. 6. Through interview with staff, the surveyor learned that children in the Mobile Infants Classroom are not always buckled when seated at a red half circle table. Staff reported they will buckle new children when they are seated at the table. Staff stated the children will crawl into the seats to play or eat and crawl back off the table or will crawl up to the middle of the table and crawl back off the table. One (1) staff reported they did not know the children needed buckled because the table was low to the ground. General: Based on observation, the surveyor found a red five (5) gallon bucket of On An’ On High Solids Metal Interlock Floor Finish placed at the bottom of the stairwell by the exit door that leads into the Gym. The label on the bucket read, “Keep out of the reach of children,” “Children can fall into bucket and drown,” and “Keep children away from buckets with even a small amount of liquid.” During interview with multiple staff, the surveyor learned that children in the Preschool and Pre-K Classrooms walk down the stairwell alone, go into the restroom alone, then return to their classroom. Staff reported that a staff member was supposed to stand on the balcony and look over as the children go to the restroom. General: Based on interview with staff, the surveyor learned timeout consists of sitting a table for three to five (3-5) minutes for one-year-old children and older. Staff reported timeout had been utilized with a one-year-old child in the Mobile Infants Classroom. The surveyor learned the child was twenty-months-old after review of the child's file. Through interview with staff-in-charge, the surveyor learned timeout is utilized for children who are twenty-two (22) months up to school age children. Staff-in-charge stated the length of timeout is based on the child’s age and was used as a last resort. The child-care center's Discipline Policy states," If timeout is used, it is one (1) minute per age of child. If the age of the child is under twenty-four-months, time out is not used."
General: Based on observation and review of KARES, the surveyor found that the KARES report for the child-care center did not contain an accurate record of staff who are employed in the child-care center; therefore, the child-care provider failed to maintain the employment status of each child-care staff member who has submitted to a fingerprint- based criminal background check by reporting the status using the NBCP web-based system. A staff member yelled at a child in the Nursery who was crying and said, “Put a sock in it.” Staff reported this has been told to the child multiple times. Based on review of the child’s file, the surveyor learned the child turned one-year-old on 03/12/24. 2. A staff member yelled at a child in the Nursery who was crying and said, “Be quiet” and “There’s nothing was wrong with you.” Based on review of the child’s file, the surveyor learned the child turned one-year-old on 03/12/24. 3. A staff member yells at children ranging in age from one-year-old to school age at the child-care center. Staff reported the staff member yells at the children if they are misbehaving, not listening, running, spitting, playing rough, arguing, hitting another child with a toy, or to stop. 4. Through multiple interviews with staff, the surveyor learned children who misbehave in the classroom are sent to a staff member's office at the child-care center. The staff member yells/screams at the children, yells at the children, and scolds the children who are in the office. The staff member screams and uses a loud stern tone stating, “You don’t do that,” or “Don’t hit your friends.” The staff member uses a loud, firm voice, scolding the children, repeating over and over what the child had done.
General: Based on observation, the surveyor found the following in the Nursery: 1. A loose pink blanket was observed to be placed on a child that was asleep in a swing/bouncer. The child was observed to be wearing a bib. Through interview with staff, the surveyor learned the child had been asleep for approximately ten (10) minutes and was eight-months-old. The surveyor observed staff to remove the child from the swing/bouncer. 2. A child was observed to be asleep in a crib and swaddled with their arms restricted. Through interview with staff, the surveyor learned the child had been asleep for approximately twenty (20) minutes. Based on review of the child’s log, the surveyor found the child went to sleep at 9:55 am. The surveyor observed the child to wake up at approximately 10:47 am while the surveyor was in the classroom. Through interview with staff and review of the child’s file, the surveyor learned the child was one-year-old. 3. A child was observed to be asleep in a crib and swaddled with their arms restricted. Based on review of the child’s log, the surveyor found that the child went to sleep at 10:37 am. The surveyor observed the child asleep at approximately 10:47 am. The children in this room are one-year-old and not toilet trained. 3) An interview with the staff person in Room 105 found that toys placed in the children's mouths are sanitized at the end of the day by spraying them with Lysol. The staff person also stated that sometimes the toys are put in the dishwasher. The children in this room are two-years-old and not toilet trained. 4) An interview with the staff person in Room 107 found that toys that are placed in the children's mouths are sanitized at the end of each day using bleach. All of the items were accessible to the children. 4) At the time of the survey both doors leading into the kitchen were observed open. In an unlocked cabinet under the sink there was a bottle of Consume Odor Eliminator, Lime Away and A Blue Glo Superior High Shine cleaner. General: Based on Observation, the surveyor observed the following items within reach and accessible to the children in care: (1) One (1) quart of Valspar Satin White paint was observed on top of the children's cubbies in the upstairs hallway. (2) Two (2) gallons of Valspar and Sherwin Williams brand paint were observed on top of the children's cubbies within the five-year-old classroom. The staff person in charge stated the facility had recently been painted. It is important to note the paint was removed during the inspection. (3) The surveyor observed an over-the-door pocket shoe organizer hanging from the bathroom door in the five-year-old classroom. The surveyor observed a container of Clorox disinfectant wipes, hard surface glass cleaner, loose black trash bags, and a bottle of Mist Air Freshener. These items were within reach of the children in care. (4) A bottle of Coppertone Sport Sunscreen was observed on top of a plastic storage bin next to the staff person’s desk in classroom #204. There were eight (8) children ages three-to-four years old present in the classroom.
General: Based on Observation, the surveyor observed a stained flower cushion in the dramatic play area located in classroom #101. General: Based on Review of Documentation, it was found that Brom -PSE DM was administered to a child on 3/29/17 at 11:50 a.m. and 5:00 p.m. On 3/30/17 the medication was administered at 7:45 a.m. and at 2:45 p.m. A review of the medication form found that the parent only signed for 3/29/17 and put the instructions to administer every four (4) to six (6) hours.
General: Based on Observation, it was found that the statement of deficencies and plan of correction posted was dated 4/29/14. General: Based on Observation, upon arrival at the facility a walk through was conducted. When the surveyor entered the two to three ( 2-3) year old room there was one (1) staff person and four (4) children. The staff person had two (2) children in the bathroom and was changing one (1) of the children's diaper on a changing table located inside the bathroom. There were two (2) children in the classroom area. The door to the bathroom was open but due to the shape of the room and the location of the changing table the staff person could not see the two (2) children and supervise them while she changed the child's diaper. The two (2) children in the classroom were in an area of the room that could not be seen unless the staff person was in the classroom. Although the staff person could hear the children she could not see the children in the room at all times. General: Based on Observation, during a tour of the two to three (2-3) year old room the staff person in the room was observed sitting at a desk putting tape on file folder games. At this time there were three (3) children in the room. One (1) child was sitting at the table with no activity. Another child was rolling around on a cot that had been placed near the book area and the other child was wandering around the room not engaged in any activity. Therefore, the hand-washing sink was not immediately adjacent to the diaper changing area. The staff stated that the classroom used to be the dining room. They stated they moved into the area approximately one (1) month ago because their classroom had water damage. The surveyor asked what they do for hand-washing. A staff person stated that she uses a baby wipe or sprays soapy water onto her hands and then wipes her hands with a paper towel. She stated that she uses baby wipes to clean the infants hands. The surveyor found that the children present were ages five (5) months to sixteen (16) months old. The surveyor found that three (3) children present were over twelve (12) months old; therefore, they would need to have their hands washed with soap and running water.

The staff person in the kitchen stated that the 2% milk was used for children ages two (2) years old and older. The staff person in charge stated that she was unaware that 2% milk was not supposed to be served.
General: Based on observation, the surveyor observed one (1) ceiling tile located in the Pre-K classroom that had a brown colored water stain. The staff person in charge stated that the ceiling tile had been replaced once before. She stated that they have extra ceiling tiles available in order to replace water stained tiles as needed.
General: Based on observation, the surveyor was inspecting the girls' restroom at approximately 9:55 a.m. when a staff person entered the restroom with two (2) toddlers ages seventeen (17) and nineteen (19) months old. The surveyor noted that the diaper changing table was positioned against the wall at the entrance of the restroom. As the staff person started changing the female toddler's diaper, the male toddler ran around the corner to the hand-washing sink. He climbed up a two (2) step step stool and placed his arms on top of the countertop and proceeded to swing his body back and forth (his feet were not touching the top step of the step stool). The toddler kept trying to reach for the water faucet. The surveyor got the toddler down from the step stool two (2) times. After the staff person finished changing the female toddler's diaper, she then picked up the male toddler to change his diaper. The female toddler also ran around the corner and climbed up the step stool. The staff person could not see either toddler when they were at the hand-washing sink due to the positioning of the diaper changing table. The staff person stated that the diaper changing table was not always in the restroom. She stated that due to recent water damage, some of the classrooms had to be moved temporarily and the only place to change diapers is in the restroom.
General: Based on review of documentation and interview, the surveyor found that one (1) staff person (hired on 5/26/15) did not have documentation of a criminal records check on file. The staff person in charge stated that a criminal records check had been requested for the staff person. The staff person in charge tried to locate the document online but the thirty day time frame to print the document had elapsed. It is important to note that this staff person was not observed working alone with children during the inspection.
General: Based on review of documentation, three (3) staff files (hire dates: 5/26/15, 3/29/16, and 5/23/16) did not contain proof of education. The staff person in charge stated that one staff person had just graduated from high school and another staff person was still in high school.
General: Based on Review of Documentation and the Early Care and Education Training Records System (TRIS), seven (7) staff (hire dates: 2/15/10, 5/10/13, 2/12/15, 3/11/15, 4/15/15, 4/20/15, 5/26/15) have not completed the Pediatric Abusive Head Trauma training as required by the regulations. One (1) staff (hire date: 4/15/15) had documentation on file that she had registered to take the six (6) hour orientation training on 7/7/15; however, there was no record on file or in TRIS that the staff person completed the training.
General: Based on review of documentation, two (2) volunteer bus drivers had driving records checks that were requested on 5/22/14. Driving records checks are required to be completed annually. Programming
General: Based on observation in the infant classroom, the surveyor found that the diaper changing table located in the classroom was positioned against a wall and adjacent to a door that leads into the kitchen. The surveyor did not observe a hand-washing sink in the classroom. The surveyor found that the closest hand-washing sink was located in the kitchen behind the door. The staff stated that the classroom used to be the dining room. They stated they moved into the area approximately one (1) month ago because their classroom had water damage. The surveyor asked what they do for hand-washing. A staff person stated that she uses a baby wipe or sprays soapy water onto her hands and then wipes her hands with a paper towel. She stated that she uses baby wipes to clean the infants' hands. The surveyor found that the children present were ages five (5) months to sixteen (16) months old. The surveyor found that three (3) children present were over twelve (12) months old; therefore, they would need to have their hands washed with soap and running water.
General: Based on observation in the infant classroom, the surveyor found that the diaper changing table located in the classroom was positioned against a wall and adjacent to a door that leads into the kitchen. The surveyor did not observe a hand-washing sink in the classroom. The surveyor found that the closest hand-washing sink was located in the kitchen behind the door. General: Based on review of documentation, one (1) child's file (enrollment date: 9/24/14) out of ten (10) files reviewed contained an immunization certificate that was no longer valid as of 5/4/16.
General: Based on review of documentation, three (3) children's files (enrollment dates: 10/1/12, 3/14/13, and 9/24/14) out of ten (10) files reviewed did not have the parent's preferred hospital information listed. Also, three (3) children's files (enrollment dates: 1/13/14, 3/1/16, 2/23/16) did not include the physician's phone number.
General: Based on Review of Documentation, the daily activities plan posted in the Nemo room was dated April and the plan posted in the Disney Babies room was not dated.
General: Based on Review of Documentation, there were five (5) children's files reviewed that did not include the name of the preferred hospital. Enrollment dates for these children were 2/9/14, 9/29/14, 3/3/15, 3/14/13, and 1/30/12.
General: Based on Review of Documentation, the emergency preparedness plan on file at the center had been submitted on 5/16/13. There was not a plan submitted by December 2014.

General: Based on Review of Documentation, there were three (3) staff who did not have enough training for their current training year. A staff person hired 2/15/10 had two (2) hours. A staff person hired 2/24/14 had two (2) hours. A staff person hired 9/9/13 had thirteen and a half (13.5) hours.
General: Based on Observation, in the bathroom there were two (2) areas where the drywall had been patched but was still unpainted. Also, in the preschool room there were excessive stains or spots on the carpet and dead bugs in the ceiling lights.
General: Based on Review of Documentation, and interview a staff person hired 4/14/15 was permitted to work alone on one (1) or two (2) occasions without the results of the child abuse and neglect check. The check was returned to the center requesting additional information, not with the results. The staff person worked worked alone in the infant room before the mistake was noticed. General: Based on Observation, this regulatory requirement was not met. Upon arrival (9:25AM) the surveyor entered the infant classroom with the staff person in charge and observed two (2) infants sleeping in the infant sleeping room without an adult present. The staff person in charge and the surveyor walked through this room to talk to the two (2) staff that were working in the adjacent infant room. The staff person in charge moved an empty crib that was positioned in the doorway between the two (2) infant rooms to gain access to the room where the two (2) staff were working. The surveyor observed three (3) children present in the room with the two (2) staff. The two (2) infants that were asleep in the adjacent room were not visible from where the two (2) staff were positioned; therefore, adequate supervision was not provided. Staffing Requirements
General: Based on Observation, the facility failed to meet this regulatory requirement. Upon arrival (9:25AM) the surveyor entered the infant classroom with the staff person in charge and observed an eight (8) month old child asleep in a swing and an eleven (11) week old child asleep in a Bouncy seat that had been placed in a crib. The eight (8) month old had a blanket positioned loose over his entire body from head to toe. Part of the blanket was being used to prop a bottle in the child's mouth. The eleven (11) week old child had a fleece blanket positioned loose over the majority of the infant's body. The blanket came up to the child's nose. General: Based on Observation, the facility failed to ensure that bottles be labeled. At the time of survey, a child in the two (2) year old classroom was observed walking around the classroom with an unlabeled bottle of milk. The staff member stated that the parent of the child dropped the child off with the bottle.
General: Based on Observation, the facility failed to ensure that food be protected from contamination. At the time of survey, two (2) plastic bowls of pretzels were delivered to the one (1) year old classroom for morning snack. The pretzels were uncovered and exposed. The pretzels remained uncovered for approximately six (6) minutes before being served to the children.
General: Based on Review of Documentation, the facility failed to provide a copy of the Kentucky Child Care Regulations to child care center staff. The copy of the administrative regulations in the facility was dated 07/23/2010, therefore, no longer current.
General: Based on Observation, the facility failed to post a planned program of activities for the one (1) year old and school age classrooms.
General: Based on Review of Documentation, the facility failed to maintain proof of educational qualification for employee hired on 11/14/2013. The facility failed to maintain a criminal records check for volunteer with start date of 06/21/2013. The staff member in charge reported that the volunteer will sometimes drive the bus that transports a child in care. The staff member in charge reported that a criminal records check had not been completed for the employee, but the employee has not been left alone in the presence of a child in care.
General: Based on Review of Documentation, the facility failed to maintain a statement from a health care professional that the individual is free of active tuberculosis or copy of negative tuberculin results every two (2) years. Employee hired on 02/11/2013 had a tuberculosis skin test that had been read on 04/19/2012. Employee hired on 03/05/2012 had a tuberculosis skin test that had been read on 02/09/2012.
General: Based on Review of Documentation, the facility failed to complete an annual check of the Kentucky driver history record for volunteers with start dates 06/21/2013 and 08/01/2011. Also, volunteer with start date 06/21/2013 had a Kentucky Driver License with expiration date of 02/26/2014. The staff member in charge reported that volunteer with start date 06/21/2013 had transported children earlier in the month of April on a field trip. The staff member in charge was unaware if the volunteer had obtained a valid Kentucky Driver License. Programming
General: Based on Observation, the facility failed to ensure that toxic cleaning supplies be inaccessible to a child in care. In the one (1) year old classroom, a bottle of Lysol wipes was found on the floor, next to diaper changing area. The manufactory label on the back of the bottle read, “Keep out of reach of children.” In the toilet room used by the preschool and pre-kindergarten classrooms, a bottle of SpanSan Disinfecting and Deodorizer was found on the floor under the hand washing sink. The manufactory label read, “Keep out of reach of children.” Also in the toilet room used by the preschool and pre-kindergarten classrooms, a bottle of Fast and Easy Hard Surface and Glass Cleaner was found in one of the showers across from the toilet stalls. The bottle’s manufactory label read, “Keep out of reach of children.” On the van used to transport children in care, the following items were found on the floor, next to the steps of the van: one (1) bottle of STP Power Steering Fluid, trash bags, and one (1) bottle of Fast and Easy Hard Surface and Glass Cleaner. The manufactory labels on the bottle of Fast and Easy Hard Surface and Glass Cleaner, and the STP Power Steering Fluid read, “Keep out of reach of children.”
General: Based on Observation, the facility failed to ensure that a sink used for hand washing be equipped with hot and cold running waster that allows for hand washing and be equipped with hot water at a minimum temperature of ninety (90) degrees Fahrenheit. The portable sinks in the preschool and pre-kindergarten classrooms did not have adequate water pressure for running water. The hot water temperature of the sinks measured at seventy (70) degrees Fahrenheit. The sinks did not provide enough water for hand washing purposes.
General: Based on Observation, the facility failed to ensure that the diaper changing area be free of any items not used for diapering. At the time of survey, a cell phone was obeserved stored on the diaper changing table in the one (1) year old classroom.
General: Based on Observation, the facility failed to maintain a protective surface for equipment used for sliding. The surveyor measured approximatley twenty-two (22) inches of space between the end of a slide and the protective mulch surface.
General: Based on Observation, the facility failed to ensure that crib mattress are covered with a clean tight fitted sheet. Four (4) mattresses in the infant classroom were covered with loose fitted sheets. The sheets had ripples in the fabric and gathered in several places.
General: Based on Interview, the facility failed to ensure that bottles are prepared by parents. The staff member in the infant classroom stated that the teacher will measure and prepare dry infant formula provided by parents. The staff member reported that the bottles are prepared in the infant classroom, and that some parents do provide the dry formula mix.
General: Based on Review of Documentation, the facility failed to maintain a current immunization certificate for children in care enrolled on 07/26/2012 and 09/27/2013. The immunization certificate for child enrolled on 07/26/2012 had an expiration date of 02/28/2014. The immunization certificate for child enrolled on 09/27/2013 had an expiration date of 04/24/2014.
General: Based on Review of Documentation, the facility failed to maintain a written record for each child identifying contact information to enable a person in charge to contact the child’s family physician. Children enrolled on 01/31/2012 and 05/01/2013 did not have a record of a family physician. The facility also failed to maintain a written record for each child identifying contact information to enable a person in charge to contact the family’s preferred hospital. These children had been enrolled on 08/06/2012, 01/31/2012, 09/05/2013, 01/09/2013, and 06/06/2013. The facility failed to maintain a written record for each child of authorization by the parent for the child care center to seek emergency medical care for the child in the parent’s absence. These children had been enrolled on 01/09/2013, 05/01/2013, and 06/06/2013.
General: Based on Review of Documentation, the facility failed to maintain a written record of quarterly practiced earthquake drills and tornado drills detailing the time, and children who participated in the drills. The time and children were not recorded for an earthquake and tornado drills conducted in March 2014.
General: Based on Review of Documentation, the facility failed to maintain a written record of monthly fire drills detailing the time, and children who participated in the drills. The time and children were not recorded for a fire drill conducted in January 2014.
