General: Based on Review of Documentation and interview, the surveyor found that the provider's dog was not currently vaccinated against rabies. The dog's rabies vaccination was due 8/18/15. The provider stated that she made an appointment for the dog to receive the rabies vaccination on Friday. It is important to note that the dog was not around the children during the inspection.
