General: Based on Observation, and interview during the investigation, the facility failed to meet this regulatory requirement. 1. During the 5/6/13 visit the surveyor observed two (2) staff caring for twenty-eight (28) children in the band room which is adjacent to the cafeteria. Staff stated that this room is used daily with the children from 3:15PM - 3:30PM because they do not have access to the cafeteria until 3:30PM each day. The only equipment available to children in the band room is chairs. The surveyor observed some of the children using their personal electronic devices; however, most of the children were wandering around the room aimlessly with nothing to do. 2. An interview with staff during the investigation revealed that on 5/9/13 the school receptionist/secretary cared for five (5) of the children that are enrolled in this program in the school office for approximately thirty-five (35) - forty (40) minutes because the staff person in charge had thirty (30) children in her care and was the only staff person present. The only equipment available to the children in the school office during this time frame was their personal electronics.
