General: Based on review of documentation, the surveyor found no documentation of current commercial liability insurance with a minimum of $100,000 per occurrence. During interview, the owner stated that the insurance carrier would not issue a commercial liability insurance policy without the center's license number, and she would contact her insurance provider to have the policy issued as soon as possible.
