General: Based on interview, this regulation was not met. The center failed to report to the Division of Regulation Child Care an ambulance was called for a child in care who was experiencing respiratory distress on 08/15/25 after they could not reach the child's emergency contacts. It should be noted the child suffered from asthma and was on a prescription medication.

General: Based on observation, the surveyor entered the Three- to- Four-year-old classroom at 9:38 am and observed two (2) staff members. The second staff member left the classroom and did not return until 9:43 am. Fifteen students aged three to four were present in the classroom. The classroom was over ratio by three students for approximately five (5) minutes.
