General: Based on review of documentation and interview, the childcare center did not have the Kentucky Consumer Product Safety posting available for parents to view. The director stated that she threw away their last one and forgot to replace it.

General: Based on observation, the ceiling lights in the Dolphin, Shrimp, and Alligator Rooms contained deceased insects inside of them.
General: Based on observation, the following was found: Alligator Classroom 1. There was chipped paint on the windowsill. 2. The bathroom walls were covered in debris and dirt with some paint chipping. Shrimp Classroom 1. The bathroom was covered in dirt and debris. 2. General: Based on interview and review of documentation, this regulation was not met. The child care center failed to immediately report an incident of abuse when a staff member bit a one-year-old child on the leg. The incident occurred on the morning of 08/06/24 (Tuesday). The center reported the incident to the Department for Community Based Services (DCBS) on the afternoon of 08/09/24 (Thursday), but did not report the incident to the Division of Regulated Child Care (DRCC).
General: Based on interview, review of documentation, and observation, the center failed to protect the health, safety, and comfort of a one-year-old child in care. It was confirmed that a staff member bit a one-year-old child on the leg in response to that child biting another child within the classroom. As a result of this child being bit by the staff member, the child obtained a bruise on the leg.
General: Based on review of documentation, 1.) Staff member (DOH 07/25/23), did not have evidence of completing Cabinet-approved Orientation training; this training is to be completed within the first 90 days of hire. 2.) Staff member (DOH 07/25/23) , did not have evidence of completing Cabinet-approved Pediatric Head Trauma training; this training is to be completed within the first year of hire. 3.) Staff member (DOH 07/25/23,12/26/23), did not have evidence of completing Cabinet-approved CPR training; this training is to be completed within the first 90 days of hire. 4.) Staff members (DOH 01/01/79, 07/25/23), did not have the fifteen (15) hours of early care and education training recorded on ECE-TRIS for the July 1, 2023 – June 30, 2024, training year. General: Based on review of documentation, staff members (DOH 11/24/20, 01/01/79, 08/08/23), did not have evidence of a negative tuberculin skin result, nor a physician’s statement clearing this individual of active tuberculosis.
General: Based on review of documentation, it was found during the investigation that a staff member was rehired on 01/11/24 after leaving employment for approximately ten (10) months, and the last Kentucky National Background Check Service (KARES) was completed 03/28/23; therefore, this staff needed to complete a new KARES check. General: Based on interview, it was found during the investigation that a two-year-old child told his mother when she came to his classroom, that a staff member was mean and had "whooped his butt". The mother of the child then asked this staff member and staff member stated that she "tapped" him on his bottom, as he was playing in the blinds.
General: Based on observation and review of documentation, this regulatory requirement was not met. 1. A volunteer who started on 7/24/2023, was observed working alone with children in the Pre-K classroom. She stated that she cooks, but also gives staff breaks. A review of the Kentucky National Background Check Service (KARES), found the volunteer was listed as “Provisional” and the determination status listed as “In Process.” The staff member had not been fingerprinted; therefore, she did not have an “Eligible” for hire status. 2. The surveyor reviewed the staff members listed in KARES and found a staff member hired on 7/18/2023, was not listed. A review of the staff member’s employee file found she had an eligible for hire status dated 7/17/2018, which is past the five (5) year timeframe to have a new background check processed. This staff member was observed working alone in the infant classroom during the inspection. Employee Records Not In Compliance
General: Based on observation and interview, this regulatory requirement was not met. The surveyor entered the infant classroom at approximately 9:41 a.m., and observed six (6) infants and one (1) staff member. Furthermore, the surveyor observed four (4) toddlers alone in the adjacent toddler classroom (divided by a baby gate and cubbies). There was no staff member present. The infant staff member stated that she was “keeping an eye” on the toddlers because their teacher had stepped out of the classroom. She stated that she had been over-ratio for five (5) minutes. She stated that one (1) of the children in the infant classroom belonged in the toddler classroom. The toddler staff member returned to her classroom at 9:49 a.m. She stated that she had to step outside to take a call from her daughter’s doctor. The infant staff member sent one (1) of her six (6) children back to the toddler classroom to remedy the situation in her class. In short, based on observation and interviews, the infant teacher was over-ratio by five (5) children for approximately thirteen (13) minutes because she had supervisory authority of the toddler classroom while the toddler staff member stepped out to take a phone call. A three-year-old child was hit on top of the hand with a wooden ruler by a staff member on 07/18/23 due to the child's alleged misbehavior. The three-year-old child cried and held his hand. According to interview, the child's hand was red on the top for a little while and then it went away. The staff member was terminated from employment.

General: Based on observation during a tour of the Pre-K Room a medication request form with a child's name, date and type of medication was in a clear plastic holder mounted to the wall by the door. General: Based on observation in the Two-year-old Room, an uncovered trash can had been placed on the floor at one end of the diaper-changing table. Soiled pull-ups were observed in the trashcan. The lid to the trashcan had been placed on the floor at the other end of the diaper-changing table near the sink.
General: Based on observation during a tour of the facility, the following was found: 1) In the One-year-old Room, a black metal box containing various tubes of diaper ointment, Vaseline and a bottle of Equate Brand Night-Time lotion was found in an unlocked cabinet under the diaper-changing table. None of the items were labeled for an individual child. The box was not locked, making all of the items accessible to the children. 2) In the Infant Room a tube of Desitin diaper ointment was stored on the diaper-changing table between the diapering mat and the inside edge of the table. The ointment was not labeled for an individual child. An unlocked black metal box was found in an unlocked cabinet under the diaper-changing table. Inside the box were tubes of diaper ointment and a tube of Hydrocortisone cream that were not labeled for an individual child. The box was accessible to the children. There was a mobile one-year-old child present in the room at the time of the inspection. 3) In the Two-year-old Room, an unlocked black metal box was found in an unlocked cabinet under the diaper-changing table. Inside the box were a tube of Desitin diaper ointment and a bottle of Sensi-Care body lotion. These items were not labeled for an individual child. A clear plastic baggy that contained a clear jelly-type substance and a name on the outside of the baggy was also found inside the box. The surveyor asked the staff person if she knew what was in the baggy. The staff person stated she was not sure of the contents but thought the child's parent had indicated it was a type of medicated Vaseline. All medication is required to be in its original container and labeled for an individual child or adult. General: Based on observation during the follow-up inspection of the playground, the surveyor found a damaged section of the chain link fence. The top rail was bent and not attached to the gateposts, and the chain link material was bent and not attached to the top rail. The staff person in charge stated that a tree had fallen on the gate approximately a week ago. She stated that they were not allowing the children to use this portion of the playground. General: Based on observation during the follow-up inspection, the surveyor found the child-care center failed to correct the regulatory violation. The surveyor again found excessive overgrowth of weeds and vines protruding through the fencerow. Additionally, the surveyor observed debris along the fencerow. It is important to note, the staff person in charge communicated in the plan of correction that the weeds and vines were cut back on 8/29/2020.
General: Based on observation and interview during the follow-up inspection, the surveyor found the child-care center failed to correct the regulatory violation. The surveyor observed a can of Good Start infant formula with a child’s name written on top of the lid. The staff person informed the surveyor that the child was new, and she prepares his bottle. The surveyor found that the formula and the water are not pre-measured by the parent prior to the staff person mixing the bottle. It is important to note, the staff person in charge communicated in the plan of correction that this regulatory violation was corrected on 8/24/2020.
General: Based on observation, the child-care center failed to meet this regulation. The surveyor found excessive overgrowth of weeds and vines protruding through the fencerow. Additionally, there was standing water at the end of two slides.
General: Based on interview and review of documentation, the director failed to conduct recurring staff meetings. The staff person in charge presented written verification of only one staff meeting conducted on 2/24/2020. It is important to note, this regulation was cited during a renewal inspection performed on 8/6/2019.
Based on review of documentation and interview, the child-care center failed to meet this regulation. The surveyor reviewed lesson plans posted in the Infant, Pre-school, and Pre-K classrooms, and found limited information. For example, only one to two activities were listed on the lesson plans such as, "shells", "paint", “outside”, and "clean cubbies". Additionally, the lesson plan posted in the pre-k classroom was incomplete. Due to the limited amount of information, it could not be determined if the activities met the individual needs and developmental levels of each child. The staff person in charge stated in previous years, the regional program manager had accepted the lesson plans during the summer months. It is important to note, this regulation was cited during the renewal inspection completed on 8/6/2019. Premises Not In Compliance
General: Based on observation, this regulatory requirement was not met. The following was found: 1. At 10:10AM the surveyor entered the facility and observed the kitchen door propped open and food cooking on the stove. Twenty-one (21) children (two (2) - five (5) year olds) were observed playing in an adjacent classroom (four (4) year old classroom). It is important to note that you must walk through the four (4) year old classroom to get to the kitchen; therefore, the stove was accessible to the children. A staff person with a 10/25/04 hire date was observed working in the four (4) year old classroom and cooking/preparing food in the kitchen at the same time during the visit. 2. During a tour of the infant classroom, the surveyor observed a five-month-old child asleep in a crib with a bib around his neck. Employee Records Not In Compliance
General: Based on a review of documentation and interview, this regulatory requirement was not met. A review of staff files during the inspection found two (2) staff that did not have the date of employment listed in their file. The staff person in charge stated that she did not have their hire dates documented anywhere. She reviewed the date that one (1) staff person filled out her background check forms and then she asked the second staff person when her first day was. The staff person in charge then verbally identified the dates - 1/9/19 and 6/19/19 as the hire dates. However, the dates of hire were not maintained in the personnel files.

General: Based on Interview, it was determined this regulation was not met. During the course of the investigation the staff person in charge provided conflicting information to the DRCC representative, the Department for Community Based Services (DCBS) representative and a Crimes Against Children officer regarding how a four-year-old child received a circular bruise to her left leg. Initially the staff person reported the child was injured by hitting her leg against a desk. The staff person then reported that the child struck her leg against a box of paper while sitting on the floor. The staff person later stated that the child could have injured her leg while she was underneath the desk in the office.
General: Based on Interview, it was determined this regulatory requirement was not met. A child in care was involved in an incident in which she received a bruise on the thigh of her left leg. The child reported that a staff person at the facility had hit her with a red item she keeps in a drawer in the office. When the parents inquired into the cause of the mark, they were told the child hit her leg on a desk. The parents were not provided with an accident report describing how the injury occurred. It is important to note that a review of documentation found fifteen (15) other accident reports in the child’s file. The staff person in charge initially reported to the DRCC representative, the Department for Community Based Services (DCBS) representative and a Crimes Against Children officer that the child was injured by hitting her leg against a desk. The staff person then reported that the child struck her leg against a box of paper while sitting on the floor. General: Based on Observation, it was found that there were several areas of chipped paint on the walls in the Three (3) year-old classroom and in the Two (2) year-old classroom. It was also found that there was a substantial amount of dirt and dust build up on the walls behind the toilet used by the children in the Three (3) year-old classroom. It was found that there was some food stains on the wall in the Two (2) year-old classroom. It was found that there was a brown ceiling tile in the restroom used by the two (2) year-olds. It was also found that the black trim around the wall in the Two (2) year old classroom was pulling away from the wall.
General: Based on Observation, it was found that two (2) of the cribs in the Infant classroom were less than 12 inches apart.

General: Based on the sunscreen (blanket permission) forms presented for review, the surveyor found that the forms had a time-frame of administration listed as May 1, 2016-August 30, 2016. The form read: "The sunscreen must be applied prior to outdoor play and repeated according to the directions on the product label during water activities from May 1, 2016- August 30, 2016." General: Based on Interview, it was found that on 7/11/17 a two-year-old child was left on the playground for less than thirty (30) seconds. General: Based on Interview, the following was found: 1. It was stated that seven (7) two-year-old children were left unsupervised on 3/20/17 for an undetermined amount of time. In an interview with staff it was found that one (1) staff person left the Twos classroom to go to the restroom, leaving one (1) staff alone with seven (7) children that were eating breakfast. When the staff person returned from the restroom the children were alone in the classroom. The staff person stated that there were two (2) children on top of a toy shelf, one (1) child was still eating breakfast and the rest of the children were running around the room. It was stated that the other staff person returned to the classroom approximately five (5) minutes later. 2. On 4/12/17 it was reported that a staff person left eight (8) two-year-old children unsupervised for approximately one (1) to two (2) minutes. A staff person that was in the front office with a parent stated that she saw the staff person from the Twos classroom walk out the front door of the facility at 8:29 a.m. The staff person in the office stated that she finished with the parent then left the office to see who was supervising the children in the Twos classroom. She said that the other two (2) staff persons present in the building were cleaning up breakfast in the Threes & Fours classroom. She indicated that she immediately sent one of those staff persons to supervise the children in the Twos classroom. A staff person in the parking lot saw the staff person assigned to the Twos classroom walking to her car and asked her, "Where are you going?" The staff person responded, "I have to go get my T.B. test. Nobody is in my room. I just left." Staffing Requirements In Compliance

General: Based on Review of Documentation, it was determined that staff persons hired 1/12/15 and 12/8/14 did not have evidence of fifteen (15) hours of annual training on file for the last full year of employment. A review of ECE-TRIS records supports the information found during the survey.
General: Based on Observation, there were numerous areas of weed growth throughout the entire playground. The weeds were coming up through the mulch and around the equipment used by the children.
General: Based on Observation, the mulch on the playground was very sparce in most places and non-exitant in some areas. The mulch under the swings was vey thin and there was no mulch at the bottom of a plastic double slide unit.

General: Based on Interview, it was determined that the staff person in the Infant classroom was preparing formula in bottles for the children in care. The facility does not provide formula for all children and the powdered formula and water being prepared were not pre-measured by the parent before mixing.

General: Based on Review of Documentation, the facility's policy for reporting suspected abuse/neglect states, "Under no circumstances is any Educare employee to take any action before reporting their observations to the center Director or Assistant Director." This is not in accordance with KRS 620.030.

General: Based on Observation, all of the cribs in the infant room were within two (2) to three (3) inches of one another and there were children in some of the cribs.
General: Based on Observation, in the pre-k bathroom it was found the the floors and the baseboards around the toilets has a build up of dirt particles and grime. Also, the white caulking around the base of one (1) of the toilets was discolored to black in some areas.

General: Based on observation, the following was found: 1) In the bathroom located in the Pre-K Room the wall beside the sink and behind the toilets had a large area that was yellowed and contained a sticky residue. At the end of the day, they are sprayed again with soap and water and then sprayed with Lysol disinfectant. General: Based on observation and interview, the following was found: 1) In a closet that connects the One-year-old Room and the Three-year-old Room, a bottle of bleach and laundry detergent had been placed on the floor in front of the washer and dryer. A door to enter the closet is located in each classroom. Neither of the doors was locked at the time of the survey making the items accessible to the children. 2) In the One-year-old-room, a green spray bottle labeled bleach water had been placed on the counter of the sink used for handwashing. The counter is low and is easily accessed by the children. The staff person in the room confirmed through interview that the bottle contained a bleach water mixture. 3) In the Infant Room, a bottle of Goo Gone was found in a black metal box in the cabinet under the diaper-changing table. The cabinet and metal box was not locked. The label on the bottle of Goo Gone indicates to keep out of the reach of children. In the same cabinet, a bottle of Luster brand Oil Moisturizing Hair Lotion was found. The label indicates to keep out of the reach of children. There was a one-year-old child who was mobile in the room at the time of the survey.
General: Based on observation and interview during a tour of the facility, the following was found: 1) In the One-year-old Room, there was not a posted planned program of activities. A written plan with May written across the top was found on top of a shelf. An interview with the staff person in the room found she does not have a current written plan of activities. The staff person stated further that she is being trained on writing a lesson plan. Further observation in the room found there were no books accessible to the children. An interview with the staff person in the room found that books are kept in the closet. The staff person stated further, books had been read to the children the week prior. Stories and books should be a part of the daily planned program of activities. Books should be accessible to the children daily. It should be noted that at the time of the survey, the children in the room were observed walking around with baby wipes. Some of the children were sucking on the baby wipes. The staff person stated that she gives the children the baby wipes because they enjoy carrying them around. The staff person also stated they like to clean the toys with the wipes. It is not appropriate for baby wipes to be given to children of this age. It is also hazardous to allow them to suck on the baby wipes. 2) In the Infant Room, there was not a posted planned program of activities. The staff person stated she does not have a written planned program of activities. The staff person stated further they are just now starting to do lesson plans since the COVID requirements were lifted. The staff person also stated all of the books and soft toys are still in storage due to previous COVID requirements. It should be noted the facility has been open since June 2020. 3) At the time of the survey on July 8, a planned program of activities for June was posted on the door leading to the playground in the Two-year-old Room; therefore, the posted planned program of activities was not current. The staff person in the room indicated she does not have a current lesson plan.
General: Based on observation and interview, the following was found: 1) The sensory table located in the Two-year-old Room contained dried pinto beans. This is a choking hazard for children of this age. 2) In the Infant Room, a box of latex gloves was found in the unlocked cabinet under the diaper changing table. The oldest child in the room at the time was one-year-old and mobile. 3) In the One-year-old Room, plastic bibs with a cloth trim were observed hanging on a hook attached to the outside of the cabinet door located under the sink. The bibs hung in a manner where they touch each other. The staff person stated she washes the bibs in the sink with a rag and dish soap after each meal and then hangs them on the hook. The staff person stated further, the bibs are not designated for specific children. The bibs contained soiled and stained areas on the cloth portion. This is an unsanitary practice and method of cleaning and storage and allows for the cross contamination of germs. 4) In the Two-year-old Room, an unlocked black metal box used to store medication was found in an unlocked cabinet under the diaper-changing table. A clear, plastic baggy was found inside the box. The baggy contained an allen wrench and various other small metal parts. This was accessible to the children. In the bathroom located in the same room a toilet plunger had been placed on the floor beside the toilet and a toilet brush was found stored under the sink in the bathroom. 5) In the bathroom located in the Three-year-old Room, a toilet brush was observed stored under the sink. This is an unsanitary method of storage the toilet plunger and toilet brush and they are accessible to the children. It is also an unsanitary method of storage of materials used by the children. 2) In the Three-year-old Room, a corner closet contained shelves with books and other materials, a freezer full of food, boxes, a DieHard Brand battery charger and various other items. A pile of assorted items had been thrown on the floor in one corner. The items consisted of cans and bottles of sunscreen, soccer balls, backpacks, trash bags, paper and other items. An unknown black substance and dirt and grime were observed on the floor entering the closet. A Catchmaster Brand sticky mousetrap was also observed on the floor by a shelf.

General: Based on observation, a child in the Two-year-old Room was given a baby wipe to clean their hands after toileting. The child's hands were not washed with soap and running water.

A clear plastic baggy that contained a clear jelly type substance and a name on the outside of the baggy was found inside the box. The surveyor asked the staff person if she knew what was in the baggy. The staff person stated she was not sure of the contents but thought the child's parent had indicated it was a type of medicated Vaseline. The surveyor asked the staff person if the parent filled out a written request to administer the medication. The staff person stated the parent verbally told her to apply to the child's bottom during diaper changes.

The surveyor found that the formula and water are not pre-measured by the parent prior to the staff person mixing the bottle.

The surveyor toured the one (1) year old classroom during the visit and found that a daily schedule was not posted; therefore, the surveyor could not determine the sequence of events or the children’s basic routine. 2. Three (3) classrooms (three (3) year old room, two (2) year old room and one (1) year old room) had a schedule of daily activities posted that was dated July 2019; therefore, they were no longer current. The surveyor toured the three (3) year old classroom during the visit and observed two (2) white step stools at the hand washing sink, that contained embedded dirt and grime on the top surface. General: Based on observation and interview, this regulatory requirement was not met. The surveyor arrived at the facility at 10:10AM to conduct the inspection. As the surveyor walked up to the front entrance, she observed a staff person with a 10/25/04 hire date present in the facility kitchen through a front window. The surveyor entered the facility and observed one (1) staff person (hire date: 6/19/19) present with a group of seventeen (17) children (two (2) - five (5) year olds) in the front classroom (later identified as the four (4) year old classroom). The staff person with a 10/25/04 hire date was still present in the facility kitchen; however, she indicated that she was working in the four (4) year old classroom with the other staff person (hire date: 6/19/19). The staff person then stepped out of the kitchen and into the four (4) year old classroom with the second staff person. The surveyor then noticed that a small group of four (4) children were sitting at a table playing with playdough in the three (3) year old classroom adjacent to the four (4) year old classroom. There was no adult present with the group. The staff person with a 6/19/19 DOH walked into the room and instructed the children to join the rest of the group in the four (4) year old classroom and indicated that she had been watching them. Therefore, the total number of children in the care of one staff person (hire date: 6/19/19) when the surveyor entered the facility was twenty-one (21). The children playing with play dough in the three (3) year old classroom were not visible to the staff working in the kitchen and the four (4) year old classroom; therefore, adequate supervision was not provided. General: Based on review of documentation, this regulatory requirement was not met. Information on the lesson plan posted in the infant room did not adequately describe the activities that children would be involved in throughout the day. General: Based on observation, the facility failed to ensure that drinking utensils be clean and sanitary. During a tour of the one (1) year old classroom, the surveyor observed several reusable sippy cups that had been washed by hand in the classroom sink after breakfast and were found lying on a paper towel to dry on a classroom shelf. This process does not meet the requirement of the regulation because the items are not sanitized. The director acknowledged this finding during the exit interview and stated that she would instruct staff to send all cups to the kitchen to be washed after each meal. A letter from the DRCC office stating the facility had submitted an acceptable plan of correction in regards to an investigation conducted on 11/14/18 was framed and posted in the lobby. General: Based on observation, this regulatory requirement was not met. The surveyor toured the outdoor playground and found the following structures with inadequate fall zone surface material around them: 1. A large metal swing set had black rubber mats positioned directly under each swing; however, the surface around the swings was bare dirt with very sparse mulch. 2. A Little Tykes double slide (approximately three (3) feet tall) had bare dirt and sparse mulch around the entire structure. 3. A double metal slide had a black rubber mat positioned at the base of the slide; however, the surface around the entire structure was bare dirt and sparse mulch. 4. A very large blue and yellow metal structure with monkey bars, multi-level platforms, spiral pole and spiral slide had a rubber mat positioned at the base of the slide; however, the surface around the structure consisted of sparse mulch and bare dirt. 5. A purple dinosaur on springs had bare dirt and sparse mulch around the entire structure. The staff person in charge acknowledged the situation during interview and stated that the owner recently added mulch to a few areas and was in the process of adding more. Equipment Not In Compliance

The surveyor arrived at the facility at 10:10AM and found that a staff person with a 10/25/04 hire date was cooking food for lunch in the facility kitchen and was also assigned to a group of twenty-one (21) children (two (2) - five (5) year olds with another staff person (hire date: 6/19/19) in the four (4) year old classroom. Therefore, additional staff are not available during cooking hours to maintain staff/child ratios. The director arrived at the facility at approximately 10:24am. At this time, the staff person with a 6/19/19 DOH separated the two (2) year olds from the group and relocated them into their classroom at the back of the building. They had fifteen (15) children in their group; therefore, both of them would be required to stay with the group to maintain ratio requirements. During a tour of the outdoor playground, the surveyor observed a black cable lying on top of the ground in the corner of the children's play area. Both ends of the cable were buried underground; however, a large section (approximately six (6) - eight (8) feet long) was located on top of the ground and was accessible to the children. The staff person in charge stated that it has been buried underground several times; however, she indicated that someone keeps digging it back up.

General: Based on Interview, it was determined this regulatory requirement was not met. A child stated she was hit by a staff person while in the care of the facility. It was reported that the staff person struck the child with a “red object” kept in the office. The child was observed to have a red and yellow in color, quarter-sized bruise after coming home from the facility. It was reported that the child was having a temper tantrum and was removed from the classroom in order to calm down. Information provided describing the chain of events following the child being removed from the classroom was contradictory. Initially staff stated the child might have been injured by hitting her leg against a desk. It was then reported that the child struck her leg against a box of paper while sitting on the floor. Later it was reported that the child could have injured her leg while she was underneath the desk in the office. A report from the Department for Community Based Services indicated that there was enough information to confirm that the allegation that a staff person had abused/neglected the child resulting in the bruise to her leg.
General: Based on Interview, it was determined that this requlatory requirement was not met. The facility staff did not follow guidelines in reporting an incident with apparent injury. A child was found to have a bruised leg after being examined by parents. The child stated the facility director had struck her with a red item she keeps in her desk. The facility did not report any type of injury or notify parents that an injury had occurred. The facility director only told parents about the incident after they had called the facility to ask what had happened. The director stated to investigators the child had only had a temper tantrum and she did not complete an incident report. The director told the parents the child had struck her leg against a desk. The director had not completed an incident report, but offered to complete one after speaking to parents about the incident. A report of the incident or injury was never made to the Cabinet by the facility. The director told investigators the child struck her leg against a box of paper in her office floor and then in a separate interview, told investigators the child was underneath her office desk and could have injured her leg. General: Based on interview and review of documentation, the surveyor inquired about the daily plan of activities (lesson plan) in the three-year-old classroom and was directed to a posted July calendar. The only activity listed on the day of the inspection (July 31st) was “water play”. The surveyor noted that there was only one activity listed for each day of the month of July. For instance, one of the activities listed was just “eat popsicles” and another activity listed was “make trail mix”. The surveyor found that a July calendar was posted in each of the classrooms with only one activity listed for each day of the month. An example of an activity listed for the day in the two-year-old classroom was “paint pictures.” Based on the documentation, the surveyor could not determine what other types of planned activities are being offered throughout the day. In short, a variety of activities must be planned and offered daily. The staff person in charge indicated that she was told that a detailed lesson plan was not necessary during the Summer months.
