General: Based on interview and review of documentation, the following was found: 1. A child's file (DOE: 10/20/23) contained an immunization certificate that had expired as of 10/31/24. The staff in charge was able to identify in their records that an updated record had been turned it but it was unable to be located at the time of survey. 2. A child's file (DOE: 07/23/25) contained an immunization certificate that had expired as of 01/14/26. General: Based on observation, several of the vinyl nap mats in the Three-Year-Old Room were torn leaving the foam exposed. This does not allow the mats to be properly sanitized.

General: Based on observation and review of posted lesson plans, 1.) The surveyor entered the three-year-old classroom at 9:51 am and observed the students to be viewing the movie “Cars”. The viewing of this movie was not listed on the daily planned programming. 2.) Upon entry to the school-age classroom, the surveyor observed the students to be watching a video on You-tube. During a review of this staff persons file, it was found that a Child Abuse Check, as well as a National Sex Offender Registry check had not been completed on this staff member from the state of Indiana. General: Based on review of documentation, it was found that one (1) staff person was allowed to work at the center after it was determined that the staff person had a DCBS substantiation. This was found through the initial application process of the National Background Check Program. Based on record review it was found that the process was started on 07/02/2019 and the substantiation was noted on this date. General: Based on observation, the following was found: 1) There were two (2) bottles of Lysol cleaner found in an unlocked sink cabinet in the girls' hallway bathroom. 2) There were numerous empty plastic bags found in an unlocked sink cabinet in the girls' hallway bathroom. 3) There was a tube of Lysol wipes found on a low shelf in the Arts and Science room. These items were all accessible to the children.
General: Based on observation, the following was found: 1) There was urine found in one (1) of the toilets in the boys' hallway bathroom. 2) There was urine found in the potty chair that was on the floor in the girls' hallway bathroom. Hygienic Practices Not In Compliance
General: Based on review of documentation and interview, this regulatory requirement was not met. On 1/15/19 it was reported to the DRCC office that a two-year-old child came home from daycare on 1/14/19 with bruising around her right elbow. The director spoke with the staff that cared for the child on 1/14/19 and submitted a written statement to the DRCC office that stated, "A staff person stated that the child climbed on the shelf and before the child jumped off of it she tried to grab her on the arm because she was afraid she would have an accident. The staff person stated that she didn't mean any harm to the child. The child also didn't show any sign that she was hurt. The staff person did not notice any marks because the child was wearing a jacket". An interview with staff during the investigation revealed that the child frequently climbs on furniture and equipment in the classroom; therefore, the child has to be re-directed down multiple times a day. This style of re-direction did not provide for the child’s health, safety and comfort.

General: Based on observation the surveyor observed the following: (1) A ceiling air return vent located in the Toddler I classroom contained an accumulation of dust. (2) A ceiling air return vent located in the Toddler II classroom contained an accumulation of dust. (3) An overhead light fixture located in the TV room adjacent to the Toddler II classroom contained an accumulation of dust. (4) A smoke detector was heard beeping indicating a low battery in the Nursery. The director stated she was aware that the smoke detector needed a new battery. (5) The surveyor observed numerous dead bugs inside an unlocked cabinet underneath the hand-washing sink located in the Toddler I classroom. (6) The surveyor observed a cabinet door that was not secured and hanging from its hinges in the TV room located downstairs in the Preschool area.

General: Based on Interview, it was found that on 7/11/17 the mother of a three-year-old child reported to the facility that the child's sibling informed her he had seen his sister crying in the hallway and his sister stated that a staff person hit her. It was reported that the staff person pulled both children into the kitchen and asked the female child, "Did I hit you?" The mother reported that she observed a red mark on the child's neck that appeared to be a burise and that the child also reported to the mother that a staff person hit her. The mother took a picture of the mark and contacted the director facility the same day. The mother of the child also sent the picture to the director. On the morning of 7/12/17 the mother of the child, the director and the staff person in question had a meeting at the facility. During the meeting it was indicated by the staff person that she was trying to get the child back into the classroom and grabbed the female child by the neck, possibley too hard. At the time of the investigation on 7/13/17 during an interview with the director when asked why the facility did not report the allegation of abuse by a staff person it was indicated that through discussion with the owner it had been determined that they felt it was an accident. Interviews also found that staff are not educated on the facility's written policy for reporting abuse and neglect.

General: Based on Observation, there were white trash bags stored under the sink near the preschool room. In the kitchenette area there were plastic bags under the sink and in a drawer accessbile to children. There were plastic bags stored in the school age bathroom in an accessible location. Also, there was a container of bleach sitting just inside the kitchen. The kitchen door was propped open and children were observed walking past the open door.
General: Based on Observation, there were several areas of the building not in compliance. In the entry area near the preschool room there was one (1) stained ceiling tile. In the yellow fairy room there were areas of the wall that had been patched with spackle but had not yet been painted. In the cot storage area there were areas of purple and yellow peeling paint and there was a hole it the wall caused by the door knob. In the school age room there was peeling paint on multiple areas of the walls and debris on the floors. In the small room adjacent to the school age room there was a hole in the wall. In the area downstairs near the back door there were holes in the wall and the floor had dirt particles and debris. In the infant room there were areas of peeling paint on the walls and a piece of baseboard missing from the wall near the trash can. In the toddler room there was grime and streaks on the walls, espeically behind the sink. In the two (2) year old room there were holes in the wall and areas of chipped paint. Also, in the two (2) year old room there was a brown stain above one (1) of the windows.

General: Based on Observation, and interview, this regulatory requirement was not met. Upon arrival (4:40PM) the surveyor observed one (1) staff person and fifteen (15) children (age range: one (1) year - school age) watching a cartoon in the T.V. room on the main floor. Since the youngest child in the group was a one year old, the minimum staff to child ratio would need to be one (1) staff person for six (6) children and the maximum group size would need to be twelve (12) children. The facility exceeded the staff/child ratio by eight (8) children and the group size by three (3) children. The staff person that opened the door for the surveyor to enter the facility identified herself as the staff person assigned to care for the two (2) toddlers that were present in the group. It is important to note that this staff person was observed sitting in a chair in the hallway and talking on the phone when the surveyor arrived at the front door. Programming
General: Based on Observation and review of documentation, this regulatory requirement was not met. Upon arrival (4:40PM) the surveyor observed one (1) staff person and fifteen (15) children (age range: one (1) year - school age) watching a cartoon in the T.V. room on the main floor. A review of the posted daily schedule and lesson plan found that television viewing is not part of the daily schedule and/or plan; therefore, the T.V. viewing was not being used as part of an educational tool.

General: Based on Observation, the walls contained numerous gash marks and scuff marks in the pre-school classroom, art/science classroom, and infant classroom. 2) There were areas of peeling orange, blue, and yellow paint in the pre-school and school age classrooms located downstairs. 3) The walls in the school age library area contained black scuff marks. 4) A corner ceiling tile in the pre-school restroom located downstairs contained brown water stains.
General: Based on Observation, the surveyor observed a couple of loose trash bags underneath the hand-washing sink in the pre-school restroom located downstairs. The staff person in charge removed the trash bags during the inspection.
General: Based on Observation, the surveyor observed some food particles and a yellow stain on a pink gingham crib sheet in the infant classroom.

General: Based on Observation, the following was found: 1. The school age classroom had a clear plastic storage container with blocks that was observed in disrepair. The container was broken in several places and a large crack down the middle of the container had been covered with clear tape. This classroom also had a child-size dress in the dramatic play clothes that had a stain on the sleeve. 2. Four (4) toys requiring batteries were observed on the children's toy shelf in the preschool classroom. None of these toys contained batteries; therefore, they were inoperable. The plastic cover on the battery compartment was missing on two (2) of these toys. 3. During a tour of the toddler classroom the surveyor heard the radio/CD player randomly turn on and off. The CD that was playing did not play properly; therefore, the song was skipping when it came on. The surveyor observed one (1) nap mat that was torn down the seam in this classroom and nine (9) electronic toys such as Vtech Little Smart Phonics from A to Z, Barney Learning Laptop and Barney driving toy, that were inoperable because they did not contain batteries. The plastic cover on the battery compartment was missing on most of these items. This classroom also contained twelve (12) puzzle frames on the children's toy shelf that did not have any puzzle pieces and two (2) barn shape sorter toys with no pieces. 4. The two (2) year old classroom contained several electronic toys such as a Fisher Price musical turtle and a Song Max keyboard that were inoperable becasue they did not contain batteries, four (4) books with torn/missing pages, a shape sorter with no pieces, a child-size dress in the dramatic play clothes that contained several stains and a red child-size couch with torn vinyl. The Song Max keyboard also had four (4) missing pieces. 5. Seven (7) toys requiring batteries were observed on the floor and on the children's toy shelf in the infant classroom. None of these toys contained batteries; therefore, they were inoperable. The plastic cover on the battery compartment was missing on most of these items. General: Based on Observation, the facility failed to meet this regulatory requirement. The surveyor observed whole milk being served to all age groups during the survey. One (1) child present was identified as a one (1) year old child; however, the other eleven (11) children that were present were identified as being two (2) years old - school age.
General: Based on Observation, the facility failed to meet this regulatory requirement. Upon arrival (8:15AM), the facility's entrance door was locked. The surveyor rang the doorbell to obtain entrance into the facility. While standing outside the front entrance the surveyor observed a staff person working in a classroom with children at the end of the hall. The surveyor observed the staff person exit the classroom and walk a few steps toward the front door; however, once the staff person observed the surveyor standing at the front door she turned around and went back into the classroom. The surveyor then knocked on the front door. A couple of seconds later, the staff person came to the front door to open it for the surveyor. The surveyor followed the staff person back to the classroom and found that the staff person had left the four (4) children in her care (one (1) and two (2) year olds) at a table and a high chair in the room while she opened the door. The surveyor observed the staff person that was working in an adjacent classroom standing at a shelf that separates the two (2) classrooms. The staff person stated that she was supervising her group of eight (8) children (three (3) years old - school age) that were eating breakfast and also supervising the four (4) children in the adjacent classroom while the staff person exited the classroom to open the front door. The staff person had a total of twelve (12) children (age range: one (1) year - school age) in her care during this timeframe; therefore, staff-to-child ratios were not in compliance. Staff stated during interview that they were the only two (2) staff in the building at the time of the visit and indicated that the staff person in charge was "on a van run to pick up three (3) children". The staff person in charge arrived at the facility during the visit (approximately 8:30AM) and had another staff person with her. The staff person in charge stated that she had been to the grocery store. When the surveyor informed the staff person in charge of this regulatory violation, the staff person in charge stated, "The one (1) year old child and the child's older sibiling typically do not arrive at the facility until 10:00AM; therefore, I thought I had adequate coverage. I did not know those they were coming in early". The staff person in charge further stated that staff have been instructed to take children with them when they have to open the front door for a parent and/or visitor. The surveyor explained to the staff person in charge that all of the children were observed eating breakfast; therefore, this practice would be disruptive to their mealtime routine. Equipment

A leather couch and love seat in the school age classroom contained numerous tears in the vinyl surface on the arms, seat cushions and back area. General: Based on Interview, it was found that on 7/11/17 the mother of a three-year-old child reported to the facility that the child's sibling informed her he had seen his sister crying in the hallway and his sister stated that a staff person hit her. It was reported that the staff person pulled both children into the kitchen and asked the female child, "Did I hit you?" The mother reported that she observed a red mark on the child's neck that appeared to be a burise and that the child also reported to the mother that a staff person hit her. The mother took a picture of the mark and contacted the director facility the same day. The mother of the child also sent the picture to the director. On the morning of 7/12/17 the mother of the child, the director and the staff person in question had a meeting at the facility. During the meeting it was indicated by the staff person that she was trying to get the child back into the classroom and grabbed the female child by the neck, possibley too hard. At the time of the investigation on 7/13/17 during an interview with the director when asked why the facility did not report the allegation of abuse by a staff person it was indicated that through discussion with the owner it had been determined that they felt it was an accident. Interviews also found that staff are not educated on the facility's written policy for reporting abuse and neglect. General: Based on Observation, there were numerous scuffs, chipping and peeling paint, and areas where the drywall was exposed in the room labeled the Art Room. General: Based on Observation, the school age bathroom had small pieces of toilet paper or paper towels on the floor. The walls in this bathroom were also grimey, especially near the light switch. Also, there was no toilet paper in the preschool bathroom.
General: Based on Observation, the diaper mat in the infant room was torn.
General: Based on Observation, there were five (5) cribs in the infant room. The sheets in each crib were dirty with what appeared to be food spills or stains cauaased by drooling babies. The staff person in the classroom stated that the sheets are washed once per month. The director stated that the sheets are washed once per week.
General: Based on Observation, in the knight room there was one (1) broken slat on the vertical blinds. In the school age room the back of the couch was broken. In the kichenette area there was a broken folding chair, the seat was missing. In the infant room there was a dresser that was dirty with tape residue and grime. In the toddler room there was a shelf stacked on another cabinet that was very unsteady and could be easily shaken by hand. There was also a broken file cabinet in this room. In the two (2) year old room the kitchen set in the dramatic play area was covered in excessive crayon marks.
General: Based on Observation, there were uncovered electrical outlets in the preschool room and the yellow fairy room.
General: Based on Review of Documentation, one (1) staff person who was designated as a driver had a driving record that was completed on 4/17/15. Programming
General: Based on Observation, in the yellow fairy room there was a metal box hanging off the wall attached with wires. Also, in this room there were cobwebs in the can ceiling lights and bugs in the flourescent lights. In the school age room there were broken slats on the door leading to the furnace area/water heater area. In the room adjacent to the school age room there was a broken electrical plate cover and a electrical outelt completely missing a plate cover.
General: Based on Observation, the warm water in the school age bathroom did not have adequate pressure. When the faucet was turned all the way on the water still only trickled out.
General: Based on Observation, the children in the preschool room were sitting at the table doing an activity then starting washing their hands at 9:58 AM in preparation for snack. The children were sent back to the table where they continued to wait until 10:10 AM for snack. While the children waited they were observed touching toys, playing with other children, and one (1) child was sucking his thumb.
General: Based on Observation, a staff person in the preschool classroom served the children morning snack without first washing her hands.
General: Based on Observation, there were not enough child sized chairs for all of the children. One (1) child was sitting in the teacher's chair. He sat there for approximately thirty (30) minutes looking at books and doing flash cards with the teachers. Two (2) other children arrived in the classroom and more chairs were brought into the room from another classroom in the center, including one (1) for the child in the teacher's chair, Transportation

General: Based on Observation, the facility failed to meet this regulatory requirement. During a tour of the facility the surveyor observed that one (1) step in the stairwell that staff, children and visitors use to access the lower level of the building was is dis-repair. General: Based on Observation, the following was found: 1. The smoke detector on the main level of the facility beeped every few minutes throughout the surveys on 10/2/14 and 10/6/14. This usually indicates a low battery or a malfunction of the device. 2. The toddler classroom had a radio/CD player sitting on a shelf that was plugged into a low outlet approximately three (3) feet away from the shelf. General: Based on Interview, the facility failed to meet the regulatory requirement for sanitizing toys and other items that are considered mouth contact surfaces by a child not toilet trained. During interview the staff person in charge described the procedure used for cleaning toys by stating, "They are cleaned with bleach water daily". This procedure does not meet the regulatory requirement. Outdoor Play Area
General: Based on Observation, during a tour of the outdoor playground, the following was found: 1. A Step 2 climber with a green slide had two (2) steps that were in dis-repair. The first two (2) steps leading up to the slide platform contained numerous broken areas which created sharp plastic edges. 2. The base of a Vtech turtle riding toy was broken which created sharp plastic edges and a small plastic truck riding toy in the same area was broken in several places. 3. A Little Tykes structure on the younger children's side of the playground that had a blue swing seat attached was broken. The swing was observed sitting off-balanced because the plastic where the swing connected to a metal bar was broken. 4. A red slide on a multicolored climbing/slide structure was in dis-repair. The top and bottom surface of the slide contained broken plastic which created sharp edges on the surface where the children slide down. 5. A tricycle merry-go-round structure was found in disrepair. The seats were missing on all five (5) trikes and three (3) foot pedals were missing. It is important to note that this same regulatory violation was previously issued as a result of a relicensure survey conducted on 5/6/14. The director had indicated on a Plan of Correction submitted 6/10/14, "Contacted merry-go-round manufacture business office for the replacement of parts on 5/20/14. The preschool head teacher will inspect playground every Monday and report any damaged toy/equipment to director. She will also be in charge of cleanliness of equipmnet on playground. Director follows up every Tuesday to make sure nothing is damaged and that everything is in good condition and will repair damaged toys or throw away if cannot be fixed". The director identified a box in the facility office that contained replacement parts for the merry-go-round structure and stated that she had a couple people attempt to fix the structure with the replacement parts; however, they were not able to do the repairs. The director further stated that she has been in contact with the manufacture and indicated that when they have a representative in the area they will attempt to do the repairs for her.
General: Based on Review of Documentation, one (1) staff person (hire date: 10/6/14) that did not have educational documentation on file.
General: Based on Observation, this regulatory requirement was not met. During a tour of the school age classroom and kitchenette on the lower level of the building the surveyor observed a can of Lysol spray and three (3) spay bottles containing bleach water solution sitting on the counter by the sink. The staff person and serveral school age children were observed doing art work in the kitchenette area at the time; therefore, these items were accessible to the children.
General: Based on Observation, this regulatory requirment was not met. During a tour of the lower level of the building the surveyor found that the sink cabinet in the children's bathroom closest to the preschool classroom was in dis-repair. The face plate on the bottom portion of the cabinet was not securely attached to the cabinet; part of it was observed lying on the floor. It is important to note that during a second tour of the facility on 10/6/14 at 3:00PM the surveyor observed this face plate completely disconnected from the cabinet. The faceplate was observed lying on the floor with the screws exposed.
General: Based on Observation, this regulatory requirement was not met. During a tour of the school age rooms on the lower level of the building the surveyor observed a school age child walk into a small room adjacent to the main school age room to retrieve an item and come back out. The surveyor noted that the room was dark. A closer inspection of this space found that the ceiling lights did not work properly. Only one (1) light bulb provided a small flicker of light inside this room; therefore, this room was not well lit.
General: Based on Observation, this regulatory requirement was not met. During a tour of the kitchenette adjacent to the school age classroom on the lower level of the building the surveyor opened a microwave sitting on the counter and observed numerous live roaches crawling around inside the microwave. A closer inspection of the area found roaches nesting inside of an electrical outlet by the sink. General: Based on Observation, and interview, this reguatlory requirement was not met. The preschool playground contained a Step 2 play structure with a green slide (approximately three (3) feet tall) and a multicolored plastic structure with a red slide (approximately two (2) feet tall). The surface throughout the entire preschool playground consisted of concrete with pea gravel on top. The surveyor observed concrete exposed at the base of the green slide due to the sparse amount of pea gravel. This type of surfacing does not provide adequate cushioning for the children during falls. The regulations defines "protective surface" as loose surfacing material not installed over concrete. General: Based on Observation, a bathroom in the basement area did not contain single use hand drying material.
General: Based on Observation, a light fixture in the hallway in the basement contained a broken plastic covering. A section of the cover was missing and it contained several cracks.
General: Based on Observation, a wall under the window in the basement T.V. Room was found in disrepair. The wall contained approximately ten (10) holes from apparent damage from a futon sitting under the window.
General: Based on Observation, a bathroom in the basement area did not contain toilet paper.
General: Based on Observation, the following was found: 1. A dis-assembled crib dated January 2006 was being stored in a closet in the basement. There was no evidence that the crib met safety standards listed in 16 C.F.R. 1219 - 1220. 2. A tour of the infant classroom found one (1) crib that contained a sheet that was not clean. The sheet contained numerous spots/stains.
General: Based on Observation, a tricycle merry-go-round structure was found in disrepair. The seats were missing on all five (5) trikes and five (5) foot pedals were missing.
General: Based on Review of Documentation, the facility failed to meet this regulatory requirement. The facility had written emergency procedures, but no evidence that these procedures are practiced monthly by staff.
General: Based on Review of Documentation, it was found that the facility did not have a written annual plan for the child-care staff professional development. The professional development plans that were on site did not contain a date; therefore, it could not be determined if the plans were done annually.
General: Based on Observation, the following was found: 1. Three (3) stained ceiling tiles were observed in the preschool area in the lower level of the building. 2. The walls in the children's eating area in the lower level of the building contained numerous dark smudges around the tables and areas of chipped paint. 3. The bottom section of the window sill in the TV room in the upper level of the building contained numerous areas of chipped paint. 4. The door frame between the upper level TV room and the two (2) year old classroom was dirty with numerous scuff marks, smudges and areas of chipped paint.
General: Based on Observation, upon arrival (8:00AM), the surveyors found one (1) staff person caring for eleven (11) children in the two (2) year old classroom. The staff person identified the youngest child present as a five (5) month old and the oldest child as a four (4) year old; therefore, staff-to-child ratios were not in compliance. The surveyors asked if any other staff were present in the facility. The staff person stated yes and yelled-out to a staff person that he stated was down the hall. The surveyors proceeded down the hall to the infant classroom, opened the classroom door and observed a staff person asleep on the floor. The surveyors woke the staff person up and informed her of the understaffing situation.
General: Based on Observation, the diaper changing pad in the infant classroom contained a large tear in the vinyl surface.
General: Based on Observation, the facility failed to equip the 2006 Hummer H3 used to transport children with a fire extinguisher, a first aid kit and a device to cut the restraint system.
General: Based on Review of Documentation, it was found that five (5) staff members had TB skin tests on file that were no longer current. These TB skin tests were dated: 4/22/11, 4/27/11, 6/22/10, 4/6/11, and 4/6/11. The staff members were hired on 4/22/11, 4/28/09, 5/21/07, 12/17/08, and 3/5/09.
General: Based on Observation, there were no culture materials available to the children in the infant classroom.
General: Based on Observation, the sink drawer beside the toilet in the girls' bathroom contained numerous personal items such as deoderant, tampons, dental floss, hair bands, lip gloss, mouthwash and tubes of toothpaste. The drawer did not contain a lock; therefore, the items were accessible to the children.
General: Based on Observation, the toilet in the bathroom used by preschool children on the lower level contained urine; therefore, it had been used, but had not been flushed. Hygienic Practices
General: Based on Observation, the facility failed to provide appropriate equipment for napping. Upon arrival (8:00AM) the surveyors entered the two (2) year old classroom and observed eleven (11) children present that were all asleep. One (1) child was an infant, the other children were two (2) - four (4) years old. The infant that was identified as being five (5) months old, was observed sleeping in a car seat in this classroom. There were no cribs available in this classroom.
General: Based on Observation, during a tour of the preschool playground the surveyor observed a Step 2 rocking apparatus that was missing its handles. The surveyor also observed a Step 2 climber/slide structure on the preschool playground that was in dis-repair. The slide was not attached to the climber section of the structure. The slide was observed lying on the ground on the opposite side of the playground.
General: Based on Review of Documentation, and interview the facility failed to meet this regulatory requirement. The facility had a written plan for emergencies; however, there was no evidence that emergency procedures are practiced monthly by the staff that transport children. The staff person in charge stated that she was not aware of this regulatory requirement.
General: Based on Observation, it was found that four (4) of the cribs in the infant classroom had two (2) mattresses in them, thus creating a suffocation hazard and going against the manufacturer's specifications. During the survey, a three (3) year old child was observed to be eating popcorn while running and climbing on the playground. Children must be seated while eating.
General: Based on Observation, it was found that the weed guard was pulled through the mulch on the playground used by the younger children. This creates a tripping hazard. In addition, it was found that the pea gravel from the playground used by the older children had run over the barrier to the playground used by the younger children. Pea gravel is a choking hazard to children under the age of three (3) years.
