General: Based on interview and review of documentation, there were several instances where the Child Supervision Record (CSR) was not completed in accordance with the childcare center’s policies on 10/20/25: 1.) A child was signed out of the Preschool B Classroom while the group was on the playground and was supposed to be transitioned to the Discovery Preschool Classroom. The staff member determined that the child would remain on the playground but did not document him as “present” on the CSR. 2.) It was found that one (1) student was not signed out for the day on the CSR, when the sign-in/ out sheet shows their guardian picked up the child at 5:00 PM. 3.) Children from the Pre-Kindergarten Classroom were transitioned to the School-Age Playground at 5:20 pm; however, this transition was not documented on the CSR sheet.

The child was supposed to be escorted to another classroom with a staff member who was leaving for the day; however, the child did not wish to go inside and the staff member left the child on the playground with the Preschool B Classroom. The staff member in charge of the Preschool B Classroom was unaware that the child did not leave the playground with the other staff member. The Preschool B staff member transitioned her students to the School-Age Playground at 5:20 pm, not accounting for the child who was left behind. The child remained on the Preschool Playground and was found by another staff member at 5:30 pm. The child was left unsupervised for approximately ten (10) minutes. General: Based on interview, it was found that on 05/07/25, five (5) children, ages two (2) and three (3), were able to get out of an unlocked gate on the playground while the teacher was tending to a hurt child by the slide. Four (4) of the children made their way down the grass toward the road, where the center bus is parked, when the teacher noticed the gate was open. The teacher was able to get all the children back into the playground quickly without injury. The staff member who located the child escorted them to the playground, where it was revealed that the staff member was unaware that the child was not with the group. General: Based on review of documentation, observation and interview, this regulatory requirement was not met. On 3/4/22, the facility reported that on 3/3/22 at approximately 11:30AM, a staff person allowed a three year old child to use the restroom alone and unsupervised in the Pre-K Classroom. An interview with staff revealed that the staff person was present with her group on the facility playground when the child had to use the restroom. The staff person opened the exterior door that leads to the Pre-K Classrooms and allowed the child to enter the building and use the restroom. Two maintance workers observed the child exit the bathroom and walk to the exterior door. The child was not able to open the door to go outside and rejoin his group. The child was not being supervised by a staff person at the time. It is important to note that a staff person was present with her group of children in the Pre-K #1 Classroom at the time; however, she was not aware that the child was in the restroom and the child was not visible from where she was sitting. The maintance workers alerted her to the situation and opened the door for the child so he could rejoin his group. An administrative staff person witnessed the incident and immediately escorted the two boys out of the classroom. The children were found by a parent at approximately 5:08 p.m. It should be noted that on 1/3/22, the surveyor entered the facility and observed an unsupervised preschooler in the hallway; the child was looking at an iPad. It is not known how long the child was unsupervised prior to the surveyor’s arrival.

General: Based on interview, the staff person in charge stated that she viewed cell phone footage of a four-year-old child on his stomach, on the classroom floor with a staff person on top of his back trying to get the child to put hand sanitizer on his hands. Interviews with staff found that the staff person tried for several minutes to get the child to wash his hands after going to the bathroom, but the child kept refusing to do so. The staff person ended up sitting on the floor with the child between her legs trying to put hand-sanitizer on his hands. Staff stated that the child fought with the staff person causing both the staff person and the child to fall over. Staff stated that the child ended up on his stomach and the staff person landed on top of him, which is when the cell phone footage was taken. General: Based on observation and interview, the child-care center failed to maintain staff-to-child ratios. The surveyor entered the child-care center’s office at approximately 10:31 a.m., and observed eight toddlers, a speech therapist, and a staff person. The speech therapist had the toddlers engaged in an activity. The surveyor found that the toddlers were one and two-years-old. The surveyor explained to the staff person that she was over-ratio by two children because the speech therapist was not considered a qualified staff person. The staff person stated that the staff person in charge had just left the office to take a couple of children downstairs. The staff person in charge returned to the office and explained that three teachers had not reported to work, and they were in process of opening the toddler classroom. At approximately 10:41 a.m., a school bus arrived, and the staff person exited the office to get a child off the bus, leaving the staff person in charge alone with the toddlers. The staff person in charge stated that an additional staff person was due to arrive at 11:00 a.m. to work in the toddler classroom. It should be noted, there were no additional staffing issues in the other classrooms.

General: Based on observation, interview and review of documentation, this regulatory requirement was not met. On 6/15/2021, a ten-year-old child was physically restrained by a staff member after he got upset and started to throw things in the school-age classroom. The child was removed from the classroom by his parent, who is also a staff member at the facility. The child tried to hit the staff member and bite her when he was removed from the classroom. Based on interviews, the parent gave permission for another staff member to restrain the child and was present in the hallway when it occurred. The staff member sat with her arms around the child (in the floor) and the child's arms were crisscrossed so he would not hit or hurt himself. The child was facing away from the staff member. Based on interview, the child was restrained for less than five minutes. The facility policy indicates that unacceptable child guidance includes the following: spanking, grabbing, pinching, or any physical punishment or restraint. Based on interviews on 6/15/2021, a 10-year-old-child was physically restrained by a staff member at the childcare center, which is against the facility policy. The child was found by another parent who was at the facility to pick up his/her child. That parent took the one-year-old to a classroom and explained where he was found. It was determined that he was in the hallway alone for approximately two (2) to three (3) minutes. DRCC surveyor noted that in the hallway where the child was found, there was a set of unblocked open stairs leading down to other classrooms and an unlocked front entrance was approximately twenty (20) feet from the hallway where the child was found.

General: Based on Observation, a tour of the facility was taken upon arrival and posted planned program of activities were reviewed as well as daily schedules. The daily schedule posted for the preschool room had 8:00 a.m.- 9:00 a.m. listed as breakfast and 9:30 a.m.-10:30 a.m. was listed as Circle Time/Small Group Time. There were no activities listed from 9:00 a.m.-9:30 a.m. In addition, the posted plan of activities just had March listed at the top and the then there was a line that said Week 1 and Week 2. There were no dates listed as to when these activities would take place. An observation of the classroom upon arrival found the room to be very chaotic. Children were observed running around the room, throwing things and some were crying. Containers of crayons had been dumped all over the floor. This was between 8:50 a.m. and 9:25 a.m. During a tour of the Pre-K room during this same time frame the classroom was found to be very chaotic with children climbing on the tables, throwing things and running around the room. It was found that all of the posted planned programs of activities in the facility did not have dates as to when the activities would be completed. The month of March was listed at the top. During an interview with the staff person in charge it was stated that she and the director are responsible for assuring staff implement what is on the lesson plan. The staff person in charge admitted that there have been issues in the preschool room with activities not always being implemented. The staff person in charge also stated that parents had come to her and the director with concerns regarding the programming in the preschool room. General: Based on interviews and review of documentation, the facility failed to provide for the health, safety, and comfort of a child. During interviews, it was found that on 11/11/15 a staff person observed another staff person spank a toddler on his bottom two (2) times. The staff person reported the observation to the Director immediately after observing the incident. The Director stated the staff person did not admit to spanking the toddler on his bottom, but she did admit to smacking the toddler's hands in order to get his attention. She stated the staff person demonstrated what she had done by smacking her own hands. Based on written documentation, the staff person was terminated for unacceptable child guidance. It is important to note that the toddler did not have any physical injuries as a result of the spanking.
General: Based on Interview, on 6/22/15 one (1) staff supervised nineteen (19) children alone. Based on interviews the children ranged in age from 2 years-old up to 8 or 9 years-old. Based on interviews the staff member was out-of-ratio for approximately 10 to 15 minutes. Based on interviews on 6/22/15 another staff was supervising infants up to 1 year-olds and this staff had six (6) or seven (7) children alone for approximately thirty (30) minutes. Based on interviews there were enough staff present in the building, but the staff were in the office area.

General: Based on Interview, the staff person reported that she was caring for approximately twelve (12) children on 1/8/14 when two (2) children started crawling and sliding underneath a gate that separates a storgage area and the other pre-school classroom. She stated that one (1) child slid unerneath the gate and entered the storage area and wedged himself between the wall and a shelf. She stated as this was occurring, a grandparent had entered the classroom and was asking the staff person questions about her grandson's belongings. She stated that as she was talking to the grandparent, another child slid underneath the gate and had entered the other classroom. The staff person stated that she was the only staff person in the downstairs area so she had to leave her classroom and leave the rest of the children unattended to go after both children who had slid underneath the gates. (This investigation was conducted on 12/5/13.) Based on a review of documentation it was found that staff document the time of arrival and departure for each child as they enter the classroom. However, the facility's written policy indicates that the parent is required to sign their child in and out using their full legal name on either the facility's "paper-based" roster and/or the facility's computer system. The facility does not assure parents follow this written policy that is in the parent handbook. The policy also requires that the parent make face to face contact with their child's teacher at the start and end of each day. Further interview found that on approximately four (4) different occassions a parent did not make face to face contact with the child's teacher because the parent did not escort their child to her classroom and then left her unsupervised in the hallway. Another parent found the child standing on the stairs on at least two (2) occasions and the child's teacher found the child outside of the classroom on at least two (2) additional occasions. The parent was not informed of the facility policy that she must make face to face contact with her child's teacher until after the child was found in the hallway approximately four (4) times.

Based on interivew it was found that on 11/27/13 a child in the Pre-K room scratched another child on the forehead while trying to take Legos out of the child's hand. Additional interviews found that on 11/6/13 this same child stabbed another child in the arm with a pencil. The child told staff he was giving that child a shot. Additional incidents with the same child occurred on 10/21/13 where a child was scratched in the face with a stick because another child told this child they were not "best friends." On 10/17/13 this child hit another child because the child "looked at him" and on 10/9/13 the child hit another child because the other child said, "Jake the dog." This child has been enrolled since 9/23/13 and has had six (6) documented incidents where another child was injured. General: Based on Interview, the facility failed to provide for the comfort of each child. On 11/21/12 at 4:25pm at six (6) year old child fell off of a slide on the playground and broke his arm. Staff stated during interview that there were no visible marks, bruising or swelling on the child's arm; however, staff stated that the child was not picked up by his parent until 5:15PM - 5:30PM and they indicated that the child's pain progressively got worse as he waited in the facility office. The staff person in charge described the child's behavior by stating, "he started crying loud/screaming prior to his dad arriving at the facility and his face was turning pale. On 7/19/18 the facility reported an incident that an administrative staff person witnessed between a staff person and a preschool child on 7/19/18 at approximatley 1:15PM. The staff person was placed on administrative leave as a result of the incident. An interview with staff and a review of documentation during the investigation found that the staff person had her upper body positioned over the child while the child was lying on his cot at naptime. An administrative staff person witnessed this when she walked in the preschool classroom, she heard the child crying and she noticed that the staff person was bent down close to the child's face saying something to him. The administrative staff person tapped the staff person on the shoulder and asked her to leave the room and take a break. The staff person stated in her written statement, "During naptime today I struggled getting a child down to nap. I walked away from him to help a friend to get down to nap. In the process I noticed the child up with a wooden train track hitting a friend on another cot with said toy. I immediatley went over to him and attempted to get him on his cot. During this process I leaned over his cot with one hand over his body and the other laying on the side closet to myself. As he was moving, my engagement ring scratched his face on the left side". General: Based on Observation, it was found that in the Head Start Immersion classroom, two (2) screws were protruding approximately 3/4" from the wall by the sinks. In the Head Start Immersion classroom and the Discovery Preschool classroom, it was found that the veneer was peeling away from the counters surrounding the sinks. The clear caulking around the sink on the right side of the counter in the Head Start Immersion classroom was found to have a black/green substance accumulated underneath it. The paint on the baseboards in the school age classroom, Infant A and Infant B was found to be peeling all around the classrooms. In the Pre-K classroom, a round metal wall plate around a pipe/faucet coming out of the bathroom wall was found to be rusted and broken, creating sharp edges. The pipe/faucet is located between the two (2) toilets and is approximately eight (8) inches off the floor.
