General: Based on observation, in the two-to-three-year-old room the diaper changing table is set up against the wall next to the bathroom. When a staff member changes a diaper their back is turned towards the classroom and they cannot adequately supervise the children. General: Based on observation, the surveyor walked into the toddler room and said "Hello" and no one answered. The surveyor observed three toddlers in the room. When no one answered the surveyor made a few steps into the room and observed a teacher sitting behind the shelf looking at her phone. The teacher did not see nor acknowledge that someone was in the room. The surveyor stood next to the shelf looking at the teacher to make an eye contact but she did not notice the surveyor. After waiting for approximately one minute the surveyor "Hello" again. The teacher looked up, then the surveyor introduced herself. The teacher then got up and put her phone away. The staff member was not engaged with the children and was not paying attention to the children in her care. This could pose a potential safety risk to the children. General: Based on observation, it was found that the infants are diapered in a room located off the infant classroom. Due to the infants being changed in a separate room, the infant teacher cannot properly supervise the remaining infants in care. The staff in charge stated that the teacher in the toddler classroom would supervise the toddler and infant classrooms while the infant teacher is diapering a child by standing in the doorway of both classrooms, which are connected by a door. The staff in charge also reported that if the toddler teacher cannot supervise both during an infant diaper change, a floating teacher would come into the classroom to assist. Having the toddler teacher supervise both rooms does not provide adequate supervision when each child must be within sight and sound. General: Based on observation, the following was found: 1.) In the two-year-old classroom, there were numerous areas througout the room where the walls were dirty, missing paint, and had scuff marks on them. In addition, there was an area in the corner of the room where it appeared that the wall was cracking, which left paint and the inside of the wall exposed. Also, there was one ceiling tile that was cracked in the corner and was starting to break away from the rest of the tile. 2.) In the diaper changing room, there were areas of missing/chipped paint on the walls, as well as some scuff marks. 3.) In the infant classroom, there were areas where there were scuff marks on the wall. In addition, there was an area of the wall that appeared to have been drywalled, but was not painted over. This area was where the baby gate is located. Also, there were numerous ceiling tiles that were starting to tear and rip. 4.) In the toddler classroom, there were areas of missing/chipped paint on the wall, as well as some scuff marks. In addition, the sprinklers in the ceiling were extremely dusty. 5.) In the four-year-old classroom, there were areas of missing/chipped paint on the walls throughout the classroom. Also, there were some holes starting to form in the walls. 6.) In the pre-k room, there were several stained ceiling tiles.
General: Based on observation, it was found that in the two-year-old classroom, there was a carton of almond milk sitting out on a shelf. During the exit conference, the staff in charge reported that the children had just had breakfast and the milk had not been put back in the refrigerator yet. General: Based on review of documentation, the staff person in charge did not ensure that the center policies and procedures were implemented. The surveyor was provided with a copy of the staff policies, including a section under discipline which reads, “Do not subject children to harsh or physical discipline, Abusive language will NOT be used, nor will any method of discipline that is frightening. Do not associate discipline with rest or food.” The job description for a Teacher reads, “Responsible for supervision of a group in the bathroom.” Further, the policies state that mistreatment of a child at the center is grounds for immediate dismissal of a staff member. Multiple staff members reported going to the staff in charge about concerns regarding a staff person’s actions towards the children. Staff reported seeing this person handle children harshly and speak to them inappropriately. The procedures outlined in the policies were not implemented.
General: Based on observation and interview, the center did not maintain staff-to-child ratios in compliance with state regulations. The surveyor visited the facility on 2/26/2020, and observed 13 students with one staff member. The surveyor asked what ages the children were, and the staff person in charge of this group stated that the youngest in the group was age four years. The surveyor observed two children who appeared to be younger, and asked for their names. The surveyor determined through file review that the two children in question were age three years. The classroom was over ratio by one student. It was determined through staff interviews that the one-year-old classroom staff would have the infant room staff supervise both classrooms at one time, while diaper changes occurred in the adjacent diaper changing room. The staff member stated that the infant room staff would be responsible for up to ten children while diaper changes occurred. The infant room teacher would be out of ratio by up to five children while the one-year-old classroom staff completed the diapering. Several staff report that the center does not maintain staff-to-child ratios while staff take lunch breaks. Additionally, it was reported that on 2/25/2020, that two children were removed from the four-year-old classroom and “hidden” in the toddler bathroom to maintain the appearance of staff-to-child ratio compliance while a DCBS worker was present in the building. Staff members report that children are moved into classrooms for an age they have not met, however; the ratios for the youngest child in the group are not maintained. General: Based on Review of Documentation, staff rehired on 05/02/16 did not have the results of a child abuse and neglect check on file. The staff person was found working with children alone in the classroom. The director stated that the results should be back any day now.
General: Based on Observation, there was a section of duct tape covering a carpeted area of the infant classroom. A ceiling plastic light cover was broken, there was an area of water stains on the ceiling and a section of the rubber baseboard was missing in the Big Room. The director stated that the church is supposed to repair or replace that area of carpet soon. The director stated that the church is planning on replacing or repairing that section of carpet in the infant classroom. The director also stated that the churches baptismal is directory above where the water stains were found and the church is getting ready to replace the section of baseboard that is in disrepair.
General: Based on Review of Documentation, staff hired on 10/30/14, 07/31/06 and 03/28/15 did not have the fifteen (15) hours of required cabinet approved training on file. Staff hired on 08/23/99, 07/31/06, 05/22/12, 08/26/94, 08/19/02, 09/04/84 and 04/18/13 did not have the required pediatric abusive head trauma class. This information has been confirmed through ECE-TRIS. 345 - Driver Requirements Not Applicable 922 KAR 2:120. Section 12. Transportation. (18) A driver of a vehicle transporting a child for a center shall: (a) Be at least twenty-one (21) years old; (b) Complete: 1. The background checks as described in 922 KAR 2:110; and 2. An annual check of the: a. Kentucky driver history records in accordance with KRS 186.018; or b. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person. General: Based on Review of Documentation, one (1) staff file (hire dated: 4/27/15) has resided in the State of Ohio within the past year. There was not an out-of-state background check on file at the time of the inspection.
General: Based on Observation, at approximately 8:45 a.m. the surveyor observed the infant staff person leave the classroom area two (2) times to put away a diaper bag and a bottle. She went into the diaper changing area that is separated from the classroom by a half door. The staff person bent down for a few seconds to put away a bottle and had her back turned for a couple of seconds to put away the diaper bag. The staff person left five (5) infants ages six (6) months to twelve (12) months unattended in order to put away a child's belongings in the next room (diaper changing area). The surveyor asked the staff person how she manages diaper changes. She stated she peaks over the door to check on the other kids as she changes diapers. It is important to note the hand- washing sink is located in a restroom that is adjacent to the diaper changing area. The staff person would not be able to see the children at all when washing children's hands after diaper changes. She stated an additional staff person comes in to help her at noon. The staff person in charge stated there are typically two (2) people in the infant classroom but one of the staff called in sick. The staff person in charge put an additional staff person in the infant classroom during the inspection.

General: Based on Interview, it was found that on 6/9/14 (at approximately 7:40 AM) a staff member was informed by the parent of a three (3) year-old child that the child hurt her arm on the playground (the child told the parent this information) and the child was favoring the arm over the weekend so they were going to pick the child up from the daycare in the afternoon and take her to the doctor to have her arm checked. General: Based on interviews it was determined that the children in the two-year-old classroom would be left unsupervised while the staff member in this classroom completed diaper changes in the diaper changing room. The children were not visible to the staff person responsible for their care. Additionally, the surveyor conducted an on-site visit on 2/26/2020, and observed a staff member in the infant classroom changing a child’s diaper. There were four infants in the classroom who were not in this staff member’s line of sight while the diaper change occurred. Further, on 2/26/2020, the surveyor observed the three-year-old classroom children to be playing in the Art room, the Transportation room, and the Reading room. The staff member in charge of this group had positioned herself in the Art room, but was unable to view the all of the children in the two adjoining spaces. On 3/3/2020, the surveyor visited the facility at approximately 1:00 pm, and observed that the one-year-old classroom and the two-year-old classrooms were too dark to properly supervise the children as they slept. It was reported that the staff member would grab the children in an aggressive manner, mock the children with speech impediments, and roll the children into their mats at naptime while placing her leg across them to prevent the child from moving. This staff member forced a child to pick their feces off from the floor, and taunted the child afterward.
