General: Based on observation, the bathroom electrical outlet located above the sink had only one receptacle covered. The other receptacle was not in use and accessible to children.
General: Based on observation, the following was found: 1) The bathroom linen closet contained a large supply of cleaning products and personal care items accessible to children. This included acne cream, Comet cleaning spray, Scrubbing Bubbles cleaner, and other miscellaneous items.

General: Based on observation, the bathroom vanity's light had one (1) fully working light bulb, one (1) slightly dim light bulb, and two (2) light bulbs with no light. This was the only light fixture in the bathroom.

General: Based on review of documentation, the following was found: 1) Two (2) children's files (DOE: 01/13/23 and 10/03/21) did not have physician information. 2) One child's file (DOE: 10/03/21) did not have a preferred hospital listed. 3) The three (3) files that did have a hospital listed did not have a phone number for the hospital.

General: Based on observation, the outdoor play area contained a broken wooden plank on the privacy fence. The broken plank was located on the side of the fence with Little Tykes riding toys in front of it. The broken fence plank left an opening in the fence that was approximately five and a half (5.50) inches, which poses a safety hazard to the children. General: Based on observation, the following was found: 1) There was a tube of Colgate toothpaste found in an unlocked drawer in the bathroom used by the children. The toothpaste was labeled on the back to "Keep out of the reach of children". The toothpaste was accessible to the children. 2) There were three packages of Polydent Denture Cleanser found in an unlocked drawer in the bathroom used by the children. The Polydent read "Keep out of the reach of children". The Polydent was accessible to the children.

General: Based on observation, the surveyor found two climbing structures in the backyard that posed a safety risk as follows: 1. There was no fall zone material surrounding a 6.5 ft. Step 2 Skyward Summit Rock Climber. The structure was positioned on grass. 2. A wooden climbing structure was dilapidated. Further, the tallest point of the structure was just inches away from touching the utility lines. It should be noted that children ages 35 months to four-years-old are enrolled in the child-care home. General: Based on observation and interview, this regulatory requirement was not met. On 10/4/19, at 11:10AM, the surveyor made a visit to the home and found that no one was present. The surveyor contacted the provider via phone while parked in the provider's driveway. The provider stated during the phone call that she was not providing child care on the day of the visit and indicated that she had taken her children to Boyd's Orchard since her older children were on Fall Break from school. The provider's hours of operation are listed as 6:00AM - 6:00PM (M - F) in the DRCC database. The provider stated that she forgot to report the temporary closure to the DRCC office.
