General: Based on Review of Documentation and interview, this regulatory requirement was not met. During the visit it was found that the provider’s liability insurance had expired. The insurance was from West Bend Mutual and it was dated 9/2014 - 9/2015. The provider stated that she had current liability insurance; however, was not able to locate current documentation during the visit. General: Based on Review of Documentation, and interview, the provider failed to meet this regulatory requirement. The rabies certificates for two (2) dogs (Sadie and Layla) were no longer current as of 4/30/14. The provider stated that both dogs were due for vaccinations. Posted Requirements
General: Based on Review of Documentation, the provider did not have a planned program of activities posted.
General: Based on Observation, a can of Lysol Disinfectant Spray labeled "Keep Out of Reach of Children" was observed sitting beside the sink in the children's bathroom.
General: Based on Observation, the following was found: 1. The surveyor observed the diaper changing area set up in the children's play room. There is not a sink in this room. The closest sink is located in a bathroom approximatley ten (10) - twelve (12) feet down the hallway from the children's play room; therefore, a sink is not immediately adjacent. 2. A tour of the bathroom used by the children found that there was no disposable hand drying material available.
