General: Based on Review of Documentation, it was found that a staff person hired 2/15/17 had the results of a criminal records check conducted through Clear Investigative Advantage systems and not through the State Police or Administrative Office of the Courts. Upon arrival this staff person was observed supervising children alone in the hallway and bathroom. The director stated that there have been other times this staff person has been left alone. General: Based on Observation, during a tour of the classrooms it was found that there are small refrigerators in each room to keep items cold that are served to the children. It was found that none of the refrigerators had an indicating thermometer or other appropriate temperature measuring device. Two (2) staff files (hire dates: 4/15/11 and 8/13/07) contained results of negative T.B. tests that are no longer current. The tests were dated 4/25/14 and 4/17/14 which exceeds the two (2) year time frame. The staff person in charge acknowledged the finding during the exit interview.

One (1) staff person (hire date: 2/8/16) had a child abuse/neglect check (CAN) and a criminal records check (CRC) that were both submitted on 2/19/16; therefore, the documentation was not submitted within the required time frame. The staff person in charge acknowledged the finding during the exit interview.
General: Based on Review of Documentation and interview, this regulatory requirement was not met. The surveyor reviewed the emergency drill form labeled "2015 - 2016 school year" during the visit and found only one (1) documented earthquake drill (3/14/16) and only two (2) tornado drills (2/25/16 and 4/8/16). There was no evidence that drills had been conducted during the August - December 2015 timeframe. The form contained the following statement in the area where the earthquake drill was documented, "Do two (2) each year". The form contained the following statement in the area where the tornado drills were documented, "Do two (2) each year including one (1) in March". Based on interview and observation the music teacher does not work alone with the children, but she does work with the children every week for about twenty (20) minutes a week. The music teacher started in 8/2013 when school started.
General: Based on Review of Documentation, three (3) staff did not have a copy of a current evaluation on file. These three (3) staff were hired on 5/1/06, 11/28/12, and 4/25/12.

General: Based on Observation, during a tour of the bathroom located between room 222 and 221 a can of Lysol disinfectant spray was found in an unlocked cabinet over the sink. In the top drawer of the cabinet where the sink is located there was a roll of trash bags. In the drawer below the trash bags there were empty Ziplock baggies. Both of the items were accessible to the children. There was a tall step stool at the sink making the disinfectant spray accessible to the children. In the bathroom located between room 226 and 225 a large container of Clorox disinfectant wipes was found in an unlocked cabinet over the sink. There was a tall step stool at the sink making the Clorox wipes accessible to the children. Both bathrooms are used by the children and both items were located on the middle shelf. When children use the step stool at the sink it makes them tall enough to reach the items. The labels on both containers indicate to keep out of the reach of children. General: Based on Review of Documentation and ECE-TRIS records it was found that one (1) staff person hired 8/19/16 did not complete the required six (6) hours of orientation training until 12/1/16 which exceeds the three (3) month requirement to obtain the training. It was also found through record review and a review of ECE-TRIS records there was one (1) staff person with a hire date of 11/15/10 and one (1) staff person with a hire date of 8/13/14 who did not complete fifteen (15) hours of training for the 2015-2016 training year. General: Based on Observation, during a tour of Room 225 a bottle of Equate brand Sport Sunscreen was observed sitting on a shelf over the cubbies. The bottle of sunscreen was not labeled for an individual child and was not in a locked container or area. During a tour of Room 226 it was found that an unlocked container that had first aid supplies contained a bottle of Coppertone Sport sunscreen that was not labeled for an individual child and a tube of cortizone cream. General: Based on Observation, during a tour of Room 221 the children were eating snack. The children were served squeezable applesauce and a juice concentrate mixture. The snack did not meet the requirements for snack because only one (1) of the items met the requirement for snack. It was found through information received from the Division of School and Community Nutrition that the juice mixed from concentrate does not meet the requirement for 100% juice. In Room 222 snack was also observed. The snack served consisted of Cheez-its, marshmellows and the same juice mixture served in Room 221. This snack did not meet the requirements for snack. General: Based on Observation, it was found that in room 226 there was a fish tank with snails. An interview with the director found that the tank had contained a fish but is no longer in the tank and at some point the snails appeared. The director stated that the teacher had obtained permission for the fish but had not obtained parental permission for the snails.
General: Based on Observation, it was found that the facility sells milk for parents that wish to purchase milk for their children's lunch. The facility sells 2 percent milk. General: Based on Review of Documentation, it was found that two (2) children's files contained immunization certificates that were no longer current. The immunization certificates were dated 1/31/14 and 5/5/14. Both children had been enrolled in excess of thirty (30) days.
General: Based on Review of Documentation, it was found that six (6) staff members did not have current professional development plans on file. These staff were hired on 8/13/12 (two staff), 8/13/07, 3/15/10, 5/1/06, and 4/25/12.
