General: Based on interview and review of documentation, on January 16, 2026, a three-year-old child was left outside on a bench without direct supervision for approximately two (2) minutes total when staff re-entered the classroom on two (2) separate occasions to retrieve the child’s shoes, socks, and coat. The outdoor area is enclosed but has a gate that could have been accessed by the child to exit the playground area. Staffing Requirements In Compliance
General: Based on interviews and review of documentation, a three-year-old child was placed outside alone to calm down on 01/16/26. Documentation and staff statements indicate the outside temperature at the time was approximately twenty-three (23) degrees Fahrenheit, with a reported windchill of ten (10) degrees Fahrenheit. The child remained outside for up to fifteen (15) minutes before calming down and being returned to the classroom. It should be noted that at certain times the child was not wearing a coat, shoes, or socks. Placing a child of this age outdoors in extremely cold conditions without continuous direct supervision compromised the child’s health, safety, and comfort. General: Based on review of documentation, the following was found: 1. One (1) child's enrollment information (DOE: 02/12/23) listed their preferred hospital as "any" instead of a specific hospital for treatment if needed. 2. One (1) child's enrollment information (DOE: 02/29/24) did not contain a contact telephone number for their preferred physician, nor their preferred hospital. 3. Of the ten (10) children's enrollment files reviewed, zero (0) contained a contact telephone number for their respective preferred hospitals.
General: Based on interview and review of documentation, one (1) staff member (DOH: 06/23/22) had a TB skin test available for review that had expired as of 08/17/25. The staff in charge indicated that the renewal test had been completed when many other staff went to complete their tests; however, the documentation was unavailable for review at the time of survey.
General: Based on review of documentation, a written policy that states the information taught in orientation will be implemented by each staff member was not available for review at the time of survey.
General: Based on review of documentation, the following was found: 1.) A staff member (DOH: 08/01/24) had TB results on file at the facility; however, the results were dated 08/07/24 which was after the staff was hired. 2.) General: Based on interview and review of documentation, the child care center failed to comply with regulatory requirements. An updated report from the Office of the State Fire Marshal was not presented for review at the time of survey. The most current report was dated 7/31/2020.
General: Based on observation, the child care center failed to maintain state regulations. A large container of Clorox Wipes, with a label on the outside that stated "KEEP OUT OF REACH OF CHILDREN", was found on a shelf directly beside the toilet in the Primary #1 Room. The wipes were accessible to children in care.
Based on observation and review of documentation, the child care center failed to maintain state regulations. On 08/03/2023, the surveyor made an attempt to conduct the annual inspection at the center. According to the provider's submitted records, it was listed in KICCS as being open year-round; however, the center was closed the day of the attempted inspection. A subsequent interview with the center director via telephone found the center closed down for a few weeks between the summer program and the start of the fall program. This closure was not reflected in their provider details and no notification was sent out stating the center would be closed. General: Based on Review of Documentation, one (1) staff person (hire date: 7/1/10) did not have a professional development plan on file at the time of the inspection.
Based on interview and observation, the facility did not seek prior approval from the DRCC office before implementing Toddler services. The surveyor asked the staff person in charge if a written request to add Toddler services had been submitted. The staff person stated she was unaware that a written request was required. The staff person stated there are five (5) children enrolled that are under the age of two (2) years old. She stated the children start at approximately eighteen (18) months. She stated the children started attending the facility on 8/18/2015. These children were observed by the surveyor at the time of this inspection.

General: Based on Review of Documentation, two (2) children's files (Enrollment dates: 1/20/15 and 1/28/15) had immunization certificates that were no longer valid as of 8/13/15 and 9/30/14.
General: Based on Review of Documentation, the minimum staff-to-child ratios and group size as required by the regulations were not posted. The staff person in charge indicated that since they were an accredited Montessori program they did not follow State regulations pertaining to ratios and group size but followed Montessori standards. However, during the inspection there was no documentation posted pertaining to Montessori ratios and group size.
General: Based on Observation, during a tour of the first classroom a container of Clorox wipes was observed stored on a shelf located over the toilet. The Clorox wipes were accessible to the children. In this same classroom there were numerous cleaning supplies stored in a low cabinet located over a low, child sized sink. The cleaning supplies were accessible to the children. The label on the Clorox Wipes and the other cleaning supples indicated to "Keep Out of the Reach of Children".
