922 KAR 2:120. Section 2. Child Care Services.
General: Based on observation and interview, at approximately 5:00 pm, the surveyor was reviewing paperwork in the elementary school's main office. There was one (1) staff member in the office with his ear buds in his ears as he was engaged in the required Pediatric Head Trauma training. The director was behind a closed door engaged in a large group meeting and not within range of voice. While reviewing paperwork, a child (DOE: 06/26/25) entered the office alone from the main hallway of the school. He approached the surveyor and handed her a behavior form. The surveyor asked the child why he was bringing that form to the surveyor to which he replied, "Ms. Mac told me to bring it" At that time, the surveyor walked with the child to the office the director was inside. She interrupted the director's meeting and informed her that the child had arrived at the office unsupervised. The director stated that there was a staff member in the office. The surveyor noted that the staff member in reference still had his ear buds on and was not aware there was a child in the office. In addition, the child had to walk down a long hallway to get to the office without staff present.
