A cellphone and a plastic tumbler filled with coffee was found on top of a brown bookcase near the entrance in the Big Kids #1 Room. 2. General: Based on review of documentation, a review of ten (10) children's files revealed one (1) file was missing an immunization certificate and the child has been enrolled for longer than thirty (30) days.
General: Based on observation and interview, there were forty (40) children from three (3) classrooms on the large playground with four (4) staff members. The children ranged in age from three (3) to five (5) years old. All children were intermingled and moved freely about on the playground. The maximum group size was exceeded by sixteen (16) children. General: Based on observation, interview and review of documentation, a child's allergy information was posted for public inspection in the Twos C classroom. Therefore, the child's personal medical information was not maintained in a confidential manner. Interview with the staff member revealed that the child's parent gave the center verbal permission to post the information; however, there was not documented evidence of the parent's consent. It was learned during interview that a two-year-old child had a long, jagged fingernail and the child accidentally scratched another child with the nail. The staff member indicated that she trimmed the long, jagged nail with child nail clippers. The staff member did not have permission from the parent to trim the fingernail. General: Review of daily attendance records and interview with the staff in charge confirmed that multiple children were signed in and out in advance, documenting times of arrival and departure for future dates; therefore, the written daily attendance records were not an accurate reflection of the time each child arrived and departed the center.
General: Based on observation, a container of baby powder and athlete's foot powder was observed on the changing table in the Ones B classroom while the changing table was in use and children were having their diapers changed. The staff members in the room claimed that the powder was not used on children and they did not know who provided the powder; however, fresh powder was observed on a child who was getting his/her diaper changed. Additionally, the diaper changing mat on the changing table in the Twos C classroom contained holes and rips, exposing the inner absorbent material. General: Based on observation, six (6) children in the Ones A classroom were placed in the bucket seat table and high chairs upon returning from the playground at 10:30AM. At 10:45Am, they were still in the seats, with no activities presented to them. The sole staff member worked to place cots throughout the room and was not engaging with the children. She stated that they were waiting for lunch. Additionally, seven (7) children in the Twos B classroom also waited a prolonged time while seated at tables and waiting for lunch, with no activity and minimal staff engagement. Two staff did not complete the 15 hours of cabinet-approved early care and education training hours between 7/01/2020 and 6/30/2021. A staff member hired on 6/08/2021, did not complete the required Orientation training. This staff member has been employed for more than three months. 3. General: Based on observation, this regulatory requirement was not met. The following was found: The white gate/fence between the large outdoor playground and the infant toddler outdoor playground contained a rusted spring that was sticking out (at the bottom of the gate) and it was not connected to the gate on one side. The spring was sharp on the end and was rusted.

The bathrooms adjacent to the 3-5-year-old room contained toilet brush scrubbers stored next to the toilets. 2. General: Based on observation, this regulatory requirement was not met. The two-year-old's bathroom contained a bottle of Lysol Advanced Toilet Bowl Cleaner and a can of Apple Of My Pie Glade air freshener under the handwashing sink. The label on the can indicated to "Keep out of the reach of children". The cabinet was not locked; therefore, the items were accessible to the children. The large (used by older children) and the small outdoor playgrounds (used by infants and toddlers) contained areas where the black weed barrier was exposed through the mulch causing a tripping hazard to the children. 2. The infant and toddler outdoor playground contained a dryer vent hole in the side of the building (located within reach of a child). The hole did not contain a cover and posed an entrapment hazard to a child’s hand or arm. 3.
