General: Based on observation, the following was found: 1) A staff member's cellular phone was located on the bench of the playground where children were playing. The staff member was not within arm's reach of the phone. 2) In the Blue Classroom, a staff cellular phone was located on a low lying bookshelf, lower than the children's height and next to the classroom door. The children were lined up near the door to go outside, and the staff member was not near the phone. 3) In the Music Classroom, a staff member's water bottle and cellular phone were located on top of a two-drawer file cabinet in the far corner of the room. This was at a lower height than the children. 4) In the Red Classroom, there were two (2) bottles of shaving cream sitting on top of the children's artwork drawers. The label on the bottles stated, "Keep out of the reach of children." Next to those bottles, was a staff member's cup on a lower lying two (2) drawer file cabinet, and another staff member's bottle next to the shaving cream. 5) In the Yellow Classroom, there was a staff member's bottle located on a shelf within reach of the children.
General: Based on review of documentation, one (1) staff member (DOH: 07/27/11) did not have the required fifteen (15) hours of annual training. She had three (3) hours for the surveyed year. A review of ECE-TRIS verified this information.
General: Based on observation, the personnel file of one (1) staff member (DOH: 07/27/11) did not have an annual written performance evaluation for any years of employment.
General: Based on interview and review of documentation, the personnel record for one (1) staff member (DOH: 07/27/11) contained the most recent TB skin test dated 09/05/23, which was no longer current. Through interview with the staff member in charge, the surveyor learned this staff member had decided to substitute teach this year at the last minute and they were scheduling her to get her TB test.
General: Based on observation and interview, the following was found: 1) In the Green Classroom, there was one (1) ceiling tile with a water stain. The director indicated that condensation from the pipes caused this. In the same room, there was a corner of ceiling tile missing next to a light fixture. 2) In the Music Classroom, there was a ceiling tile corner, located in the middle of the classroom, with a water spot that had what appeared to be mold growing on it.
General: Based on review of documentation and interview, one (1) child (DOE: 08/18/25) did not have a preferred hospital listed, rather it stated "whatever is closest/most appropriate". The surveyor discussed the need for the provider to put telephone numbers for the rest of the files that had a hospital listed.

General: Based on Observation, it was found that parents take turns providing the snack food for the entire class each day. The foods being brought into the facility were found to be portioned out at the person's home and placed in individual bags. Since homes are not regulated and in compliance with the Department of Public Health's food safety standards and permits, this is not an acceptable practice. General: Based on Review of Documentation, it was found that the file for an employee hired on 1-18-11 contained the results of a negative TB skin test that was read on 8-13-15. This date is not within the two (2) year timeframe required by the regulation.
General: Based on Review of Documentation, it was found that the ECE-TRIS records and files for employees hired on 10-4-07 and 11-10-11 contained evidence of the Pediatric Abusive Head Trauma training being completed on 7-19-12. This date is not within the five (5) year timeframe required by the regulation. It was also found that the ECE-TRIS record and file for an employee hired on 10-4-07 contained evidence of only nine (9) hours of training being completed during the last completed training year. General: Based on Observation, upon arrival a child was observed walking down the hall to the bathroom unsupervised. The staff person in charge indicated she has spoken to staff about assuring children are supervised when going to the bathroom. Staffing Requirements
General: Based on Observation, at the time of the survey the staff person in charge had left her classroom and was observed walking down the hall. During a tour of that person's classroom there were two (2) adults in the room with eighteen (18) children ages four (4) to five (5) years old. An interview with the staff person in charge found that one (1) of the adults in the classroom was a volunteer. However, the staff person in charge did not have the results of a criminal records check and child abuse and neglect check on file. There was no documentation on file to verify the application for these records checks had been submitted. This volunteer was observed reading a story to the children. This volunteer counted in the staff to child ratios because the staff person in charge had left the room.
General: Based on Observation, at the time of the survey a child was observed leaving the red room and go down the hall to the bathroom unsupervised. An interview with the staff person in charge found that she acknowledged the child went to the bathroom unsupervised.
General: Based on Review of Documentation, a staff member hired (1/18/11) completed 13 (thirteen) hours of annual training, which does not meet the minimum training requirement.
General: Based on Observation, at the time of the survey there were eighteen (18) children ages four (4) to five (5) years old in the red room. There were two (2) staff with one (1) of those staff members being the staff person in charge. Upon arrival the staff person showed the surveyors the rooms and then went back to the classroom. However, throughout the course of the survey the staff person in charge was observed walking throughout the facility. One (1) of the surveyors went to the red room at several different times and found that there was one (1) staff person and eighteen (18) children ages four (4) to five (5) years old in the room. During the times the staff person in charge was observed out of the classroom after the initial walk through the staff person in charge was not working with the surveyors. During an interview with the staff person in charge the staff person in charge first indicated that she had to be with the surveyors. However, it was brought to her attention that she was only with the surveyor during the inital walk through which was the choice of the staff person in charge and after that she was supposed to be in the classroom. The staff person in charge then stated she has to leave the room in order to check on the other classrooms.

General: Based on Review of Documentation, the following was found: 1. One (1) staff file (hire date: 8/29/13) did not contain evidence of the six (6) hours of cabinet-approved training being completed within the first three (3) months of employment. 2. Six (6) staff files contained training certificates from the Art Museum at the University of Kentucky. There was no evidence to verify that the training hours were obtained from a cabinet-approved source. Therefore, the regulatory requirement was not met.

An interview with administrative staff during the 10/2/13 relicensure survey revealed that on 9/5/13 a four (4) year old child fell off the monkey bars on the playground and broke her arm. General: Based on Observation, during an inspection of first aid supplies a bottle of Rite Aid brand acetaminophen pain relief was observed in the box that contained the first aid supplies. The box was unlocked and kept on a shelf behind the door in a small unlokced office located in the hallway. General: Based on Observation, the facility failed to ensure an opening to the outside be protected against the entraince of vermin. During a tour of the facility, the surveyor observed a window open in the Blue Room (4's class) and the window did not contain a screen.
