General: Based on interview and review of documentation, record of a monthly fire drill having been conducted for the month of January 2026 was not available for review at the time of survey. The staff-in-charge indicated the drill was scheduled to be completed, but the center was closed due to weather conditions and therefore the drill was not conducted.
General: Based on observation, the following was found: Infant/Toddler Classroom 1. A green wall had several areas of chipped paint in a localized cluster. School-Age Classroom 1. An intake vent above the classroom library had a buildup of dust. 2. A vent above a classroom table had a buildup of dust.
General: Based on interview and review of documentation, the following was found: 1. A child in care (DOE: 11/11/24) had an immunization certificate available for review that had neither a date until which the record was current, nor a physician signature. 2. Two (2) children in care (DOE: 11/11/24 [x2]) had immunization certificates available for review that expired as of 05/31/25. The staff-in-charge indicated the parent had intended to renew the immunization records but had not yet done so.

General: Based on review of documentation, a written procedure detailing how the center would maintain supervision, ratios, and group size at various times of day was not available for review at the time of survey. General Administration In Compliance

General: Based on interview and review of documentation, a staff member (DOH: 06/03/25) did not have evidence of having completed CPR and First Aid training. The staff-in- charge stated that this staff member had completed the cabinet-approved six (6) hour orientation training, which does include a component that satisfies the CPR and First Aid training requirement; however, an error in the ECE-TRIS system did not record this information.
General: Based on observation and interview, a lesson plan was not posted nor available for review in the Preschool Classroom. It should be noted the classroom was not in use at the time of survey, though the staff-in-charge indicated it was used regularly.
General: Based on interview and review of documentation, a staff member (DOH: 10/30/97) who is the owner of the child care center and occasionally filled in to transport school-age children to or from local schools did not have a current CPR and First Aid certification and records indicated no evidence of prior certification in the staff member's personnel file. The staff-in-charge stated that this staff member acted primarily as the owner and was gradually becoming less involved in a hands-on capacity. The staff-in-charge stated the last time the staff member transported school-age children was in October 2025 to the local elementary schools, approximately two (2) days during a week the staff-in-charge was on vacation.
General: Based on interview and review of documentation, the following was found: 1. A staff member (DOH: 06/03/25) did not have evidence of having completed the cabinet-approved six (6) hour orientation training within the first three (3) months of hire. The staff in charge stated that the staff member had completed the training requirement, however, an error in the ECE-TRIS system did not record this information. 2. A staff member (DOH: 08/28/17) only completed twelve (12) of the required fifteen (15) hours of annual training for the year dated 07/01/24 to 06/30/25. 3. A staff member (DOH: 08/29/22) only completed fourteen (14) of the required fifteen (15) hours of annual training for the year dated 07/01/24 to 06/30/25. A review of ECE-TRIS confirmed these findings.

General: Based on interview and review of documentation, record of a vehicle evacuation drill having been performed for the month of January 2026 was not available for review at the time of survey. The staff-in-charge indicated the drill was scheduled to be completed, but the center was closed due to weather conditions and therefore the drill was not conducted.
General: Based on review of documentation, the following was found: 1. Six (6) children in care (DOE: 09/21/22, 05/15/23, 07/05/23 [x2], 02/10/25, and 06/02/25) did not have a contact telephone number for their respective preferred hospitals within their enrollment information. 2. A child in care (DOE: 02/10/25) did not have a preferred physician with contact information within their enrollment information.
General: Based on review of documentation, a written plan detailing the reunification procedures for infants and toddlers, children with chronic medical conditions, and children with disabilities with their guardian in the event of an emergency was not available for review at the time of survey. General: Based on observation, this regulatory requirement was not met. During the inspection there were 21 children present on the playground with three teachers. However, four of those children were two-years-old. The children on the playground ranged in age from two-years-old to eight-years-old. Due to the two-year-old children being on the playground, the maximum group size would be 20 children; therefore, the facility was over group size by one child. An uncovered electrical outlet was observed in the school-age room (basement) located next to an exterior door. 2. General: Based on observation and interview, this regulatory requirement was not met. The following was found: 1. The preschool room contained a vent on the lower section of the wall below the window in which a metal grate was bent and provided a large opening that could be an entrapment hazard for a child’s fingers. 2. The school-age room (basement) contained a leak on the soffit. The water was running down the wall and onto the floor, where the facility staff had placed several paper towels to catch the water on top of the carpet. The soffit and the wall were discolored due to the water leak. The staff in charge indicated that the water leak happened this morning and that a plumber was notified. The preschool room contained a small hole in the drywall that was partially covered by a piece of rubber trim, but part of the hole was still exposed. General: Based on review of documentation and interview, this regulatory requirement was not met. At the time of the survey it was found that Transportation Cabinet vehicle documentation could not be provided for the facility's 2013 Ford E350 van. The staff person in charge stated that this vehicle is used to transport children daily during the school year and during field trips in the summer.

General: Based on observation and interview, this regulatory requirement was not met. Upon arrival (9:06AM), the surveyor observed six children and one staff person in the infant/toddler classroom. The youngest child in the group was an infant, the minimum staff to child ratio would need to be one staff person for five children. The staff person in charge stated during interview that she forgot that the second staff person assigned to the group was scheduled off work for an appointment. The staff person working alone in the infant classroom stated that the group had been over ratio approximately 30 minutes because her sixth child arrived at approximately 8:35AM. General: Based on observation, this regulatory requirement was not met. During a tour of the preschool classroom the surveyor observed a staff purse positioned on top of a storage cart in the corner of the room and a second staff purse hanging on a hook in the same area. Both purses were accessible to the children present in the classroom.

General: Based on review of documentation and the Training Records Information System (TRIS), the following was found: 1. Two staff (hire dates: 9/3/19 and 2/15/18) did not complete the six hours of cabinet-approved training within the first three months of employment. A review of Division of Regulated Child Care files found that this regulatory violation was previously issued for these same two staff as a result of a renewal inspection conducted on 2/12/20. 2. Two staff files (hire dates: 2/15/18 and 3/3/18) did not contain evidence of the pediatric abusive head trauma training class being completed within the required time frame. A review of Division of Regulated Child Care files found that this regulatory violation was previously issued for these same two staff as a result of a renewal inspection conducted on 2/12/20. 3. One staff person (hire date: 10/30/97) had a pediatric abusive head trauma training class certificate that was dated 4/20/13; therefore, this training is no longer valid. General: Based on review of documentation, the surveyor found the following: 1. One staff person (hire date: 5/30/1995) had TB documentation on file that was no longer valid as of 1/4/2020. 2. General: Based on observation, the surveyor found the following: 1. In the two-year-old room, the surveyor observed a box fan, a vacuum cleaner, a broom, and a dustpan next to the hand-washing sink. Eight children were present in the classroom during the observation. 2. In the breakfast area, the surveyor observed a small stepladder propped against the wall. Seven children were present in this area eating breakfast during the observation. 3. In the restroom located in the preschool room, the surveyor observed a mop bucket and a mop. Based on Interview, it was determined that, while changing diapers in the Ones classroom, the staff have their back to the children. The children are not properly supervised while staff change diapers. The changing area, sink and cabinets are positioned in the middle of a large classroom. One half of the room is used as the Ones classroom and the other half is gated off and is used for storage. During the investigation a staff person lifted a child onto the changing table to undress the child and was facing the storage side of the room. The staff person's back was to the children and the classroom side of the room. There were no other staff present in the room. This follow-up inspection recorded no violations or advisories.
General: Based on Review of Documentation and ECE-TRIS records, it was found that a staff person hired 5/6/14 had only completed six (6) hours of cabinet approved training for the 2015-2016 training year. It was also found that this same staff person did not have documentation on file or in the ECE-TRIS system verifying the completion of one and one-half (1 1/2) hours of pediatric abusive head trauma training. A review of staff files and ECE-Tris records found that staff person hired 1/23/14 had not completed fifteen (15) hours of cabinet approved training for 2016-2017 and an additional staff person hired 10/23/14 had not completed fifteen (15) hours of cabinet approved training for 2015-2016. Further review of staff files and ECE-TRIS records found that two (2) staff with hire dates of 6/15/94 and 5/30/95 did not have evidence of having completed fifteen (15) hours of training for the 2015-2016 training year. During an exit conference with the staff person in charge it was acknowledged that staff had not completed all required training hours. General: Based on Review of Documentation, there was no evidence to verify that staff meetings are conducted with staff for 2016. At the time of the exit the director indicated she could only find documentation for staff meetings conducted in 2014.

One infant, identified as being five-months-old, was observed asleep in a bouncy seat that had been positioned inside of a crib. The child had a blanket on top of him that was positioned close to his face. One staff person (hire date: 9/3/19) had an "Application for Employment" in her file stating that she did not graduate high school and no other educational documentation was available; however, she was observed working alone with children in the preschool classroom during the visit. A review of Division of Regulated Child Care files found that this regulatory violation was previously issued for this same staff person as a result of renewal inspections conducted on 2/22/21 and 2/12/20. The director had indicated on the Plan of Correction connected to the 2/22/21 renewal that the employee is currently working on getting her GED and stated, "The employee will remain on staff at the center, but will not have any supervisory control over any children. She will be helping with cleaning and preparing/setting up for snacks and meals until she successfully completes her GED".

General: Based on observation, the surveyor found the following: 1. In the two-year-old classroom, the surveyor observed areas of chipped white paint around a window ledge. 2. In the breakfast area, the surveyor observed dirt and grime on the platform underneath the hand-washing sink. 3. In the preschool classroom, the surveyor observed a loose outlet plate that was not secured to the wall, leaving a gap between the wall and the outlet plate. The surveyor noted that the television was plugged into the outlet.

The staff person indicated that she puts it on daily in the morning for about twenty-five (25) minutes. General: Based on Observation, during a tour of the two-year-old room there were nine (9) children ages one to five (1-5) years old with two (2) staff. Three (3) of the children were under two (2) years of age. It was found through interview that the age of the youngest child who had not turned two (2) years old was twenty-one (21) months old. Interviews with staff and the staff person in charge found that there was not a written transition plan for each child and signed by the child's parent. General: Based on Interview, the facility failed to conduct, manage and document in writing staff meetings. The staff person in charge stated that staff meetings had been conducted; however, was not able to locate documentation during the survey.
General: Based on Review of Documentation, six (6) children (enrollment dates: 1/24/11, 1/4/11, 9/1/10, 10/17/10, 6/29/11 and 8/22/11) had immunization certificates that were no longer current.
