General: Based on review of documentation, staff member (DOH: 08/12/24) had a negative tuberculin skin result on file that was not dated. The surveyor was unable to determine when the skin test was conducted.
General: Based on review of documentation, the following was found: 1) Staff member (DOH: 01/09/23) completed twelve (12) out of fifteen (15) hours of cabinet-approved early care and education training for the 07/01/24 to 06/30/25 training year. 2) Staff member (DOH: 01/02/24) completed twelve (12) out of fifteen (15) hours of cabinet-approved early care and education training for the 01/01/25 to 06/30/25 (second year) training year. 3) Staff member (DOH: 07/25/24) completed ten (10) out of fifteen (15) hours of cabinet-approved early care and education training for the 07/25/24 to 07/24/25 training year. 4) Staff member (DOH: 06/01/17) completed twelve (12) out of fifteen (15) hours of cabinet-approved early care and education training for the 07/01/24 to 06/30/25 training year. A review of ECE-TRIS supports these findings.

General: Based on review of documentation, this regulation was not met. Six (6) staff were hired prior to having their TB results or a statement from a health care professional stating that the staff is free from active tuberculosis. Staff one (1) was hired on 09/28/23 but had their TB results on 10/04/23. Staff two (2) was hired on 08/01/22 but had their TB results on 09/02/22. Staff three (3) was hired on 03/10/23 but had their TB results on 03/23/23. Staff four (4) was hired on 10/11/23 but had their TB results on 10/13/23. Staff five (5) was hired on 08/28/23 but had their TB results on 09/01/23. Staff six (6) was hired on 08/18/23 but had their TB results on 09/10/23.

General: Based on observation, it was found that upon arrival to the facility, there were thirty (30) children playing on the playground. The children were ages three to five-years-old and were not separated by class. The director stated that the children from the two (2) classrooms are combined daily in the mornings so they can play together. She also stated that she was not aware of the group size regulation. General: Based on observation, it was found that plungers and toilet brushes were in the bathrooms of PK1 and PK2. These items were accessible to the children. It was also found that the children's jackets in PK1 and PK2 were overlapping on the hanging hooks, thus allowing the spread of communicable diseases. Employee Records Not In Compliance
General: Based on review of documentation, it was found that the files for six (6) employees did not contain evidence of a high school diploma, GED or Commonwealth Childcare Credential.
General: Based on review of documentation, it was found that the files for employees hired on 8-15-17, 3-1-18, 11-6-17, 8-15-18, 8-15-18 and 8-15-18 did not contain evidence of a TB skin test or screening being completed.
General: Based on review of documentation, it was found that the file for an employee hired on 8-11-17 contained evidence of Pediatric Abusive Head Trauma training being completed on 1-18-13. This date is not within the five-year timeframe required by the regulation. It was also found that an employee hired on 8-15-17 did not have evidence of the six- hour cabinet-approved orientation training being completed on file. This information was found by a review of the employees' files and ECE-TRIS records.
