General: Based on review of documentation, four (4) staff members (DOH: 05/05/23, 11/12/24, 08/05/24, and 03/12/24) did not complete the required (15) fifteen hours of annual training for the 2024-2025 training year. A review of ECE-TRIS verified this information.
General: Based on observation, several ceiling tiles in the Preschool Classroom were water stained.
General: Based on review of documentation, one (1) child (DOE: 07/29/24) did not have a current immunization certificate on file. Immunization on file was dated 11/21/24 and was marked religious objection; however a Commonwealth of Kentucky Certificate of Religious Exemption form was not on file.
General: Based on observation, a couch in the Preschool Classroom in the library center had peeled vinyl and was not in good repair.

General: Based on review of documentation, two (2) staff members (DOH: 11/12/24, 12/20/24) did not have a current TB test or statement from a health professional on file.
General: Based on observation, the following was found: Spaceship Room 1) Two (2) holes in the bathroom wall. Dinosaur Room 1) One (1) ceiling tile had a water stain on it. Forrest Room 1) Chipped paint on the wall by the cubbies. Rainbow Room 1) Two (2) holes in the green bathroom wall.
General: Based on review of documentation, the following was found: 1) A staff member (DOH: 05/05/23) did not complete the required pediatric abuse head trauma training. This staff member has been employed for more than one (1) year. 2) A staff member (DOH: 11/07/22) completed pediatric abuse head trauma training on 07/22/19; however, did not renew the training due 07/22/24. A review of ECE-TRIS system verified this information.
General: Based on observation, the water table was placed on top of a small shelf in the Ocean Room, creating a hazard for children in care.
General: Based on review of documentation and interview, it was determined that on 06/06/24 the air conditioning unit responsible for cooling the One-year-old classroom was not adequately cooling a classroom, causing the room temperature to rise to uncomfortable conditions for the occupants.

General: Based on a review of the video footage recorded on 05/24/23 at approximately 9:27 AM, (the time stamp on the video was 8:27 AM, but it was off by an hour due to daylight savings time) it showed a staff member pick up a one-year-old child by one (1) arm while on the playground. The child was seen dropping to the ground and the staff member tried to pick her back up by one (1) arm. It appeared that the staff member was trying to get the child to stand up. This is an inappropriate way to pick up a child.
General: Based on observation, the weed guard within the older children’s playground was exposed under the slide and presented a tripping hazard. The surveyor was unable to determine the temperature of the refrigerators. General: Based on observation and review of documentation, a television was on in the Spaceship Room playing a video of "Kids Bop". A review of the posted daily schedule found this information was not documented, nor was it on the daily planned program of activities (lesson plan).
General: Based on observation, bathroom doors in the Spaceship Room were dirty with stains.
General: Based on observation, there was no toilet paper accessible to children in the green bathroom in the Rainbow Room. The toilet paper was stored on top of the paper towel holder.
General: Based on observation, there was no soap in the Spaceship Room bathroom.
