General: Based on observation, the following was found: Children's Bathroom next to Violets Classroom 1) A container of diaper cream was located on top of a low shelf, accessible to the children. General: Based on observation, a current daily planned program of activities (lesson plan) was not posted in the Lilies, Violets, Blue Stars, Tulips, or Daisies Classrooms.
General: Based on observation and interview, one (1) infant did not have an individual crib. Nine (9) infants were present at the time of the survey, but there were only eight (8) cribs. The surveyor asked the teacher how the infants napped. The teacher told the surveyor that the two (2) oldest infants used cots for nap time. One (1) of the infants using a cot was under the age of twelve (12) months.

General: Based on observation the following was found: Tulips Classroom 1) A large table was placed on its side with the legs protruding outward. In this position, the table did not appear to be balanced and it posed a safety hazard.
General: Based on review of documentation, no TB documentation was presented for three (3) staff members (DOH: 08/07/24, 03/31/25, and 04/07/25) who were observed caring for children during the inspection.
General: Based on observation, the following was found: Children's Bathroom across from office 1) Lysol cleaning wipes were located under the sink cabinet. The door on the cabinet was not locked.
General: Based on observation, the following was found: Gross Motor Room 1) Three (3) climbing cushions/mats had tears which exposed the inside padding. 2) One (1) floor mat in the corner of the room had a hole which exposed the inside padding.
General: Based on observation, the following was found: Violets Classroom 1) The changing table had an area of chipped paint and splintering wood. The area was within the reach of infants while being changed.
General: Based on review of documentation, a child (DOE: 03/12/24) did not have a current immunization certificate on file.

General: Based on observation and interview, during a tour of the Blue Stars Room, The Daisies Room and the Lilies Room lunch was observed. It was noted that milk was not a component for almost all the children's meals. However, cups of water or flavored drinks provided by the parents was served. Interviews with staff found the parents are required to provide lunch and drinks for their child. Further interview found some parents provide milk for their child. However, the facility does not supplement those children's lunches that do not have milk provided by the parent and water is given to those children whose parents do not send a drink item with their child's lunch. One of the staff indicated that if a parent provides milk it is stored in the refrigerator located in the employee break room or in the classroom if there is access to a refrigerator. An inspection of the refrigerator located in the employee breakroom found one carton of Lactose Free milk. An inspection of the refrigerator located in the Blue Stars Room found two cartons of Oat Milk. General: Based on observation and interview, a diaper change was observed in the Daisies Room. The staff member changed a toddler’s diaper and returned the child to play without washing the child's hands or prompting the child to wash their hands. It should be noted this room is in a larger room that has been divided down the middle with a half wall, creating two (2) separate classrooms. Access can be gained to the other side through a half door that leads into the Blue Stars Room. There is not a sink located on the Daisies side of the room. An interview with the staff member indicated that they have access to the bathroom located in the hallway or they can wash hands at the sink located on the other side of the half wall. General: Based on interview, the following was found: 1.) Some of the classrooms exceed staff-to-child ratio in the morning time when several children arrive at the same time. Staff report being over ratio for approximately one to two (1- 2) minutes while children are moved, or staff are brought in to correct the issue. 2.) During the week of June 10, two (2) staff members cared for fifteen (15) students aged one- to two-years-old for approximately thirty (30) minutes. General: Based on interview and review of documentation, the following was found: 1) A staff member (DOH:10/14/24) did not have a copy of a high school diploma, GED or other qualifying documentation on file at the time of the inspection. 2) A staff member (DOH: 12/16/24) did not have a copy of a high school diploma, GED or other qualifying documentation on file at the facility. The staff person in charge indicated the staff member (DOH:12/16/24) was supposed to have been at work at 7:00 am that morning but did not come to work nor did the staff member contact the facility to indicate they would not be at work that day. The staff member in charge indicated further, this staff members employment will be terminated.

General: Based on observation and interview, a diaper change was observed in the Daisies Room. It should be noted this room is in a larger room that has been divided down the middle with a half wall, creating two (2) separate classrooms. Access can be gained to the other side through a half door that leads into the Blue Stars Room where a sink is located. An interview with the staff member indicated that they have access to the bathroom located in the hallway or they can wash hands at the sink located on the other side of the half wall. Both options require the doorknob to be touched while entering or exiting the space, therefore, cross contaminating the doorknob and other surfaces. General: Based on observation and interview, during an inspection of the Violets Room, the following was found: 1) A staff member picked up an infant and positioned the infant in a Boppy Pillow that had been placed on the floor. The staff member then handed the infant a bottle. Although the infant was able to hold the bottle independently this regulation requires that a child be held while drinking from a bottle. 2) A bottle was observed on the floor beside an infant lying on their stomach. The surveyor asked the staff members in the classroom to whom the bottle belonged. One of the staff members indicated that the bottle belonged to the infant observed lying beside the bottle. The staff member indicated further that the infant had finished drinking their bottle. The surveyor asked the staff member where the infant had been fed their bottle. The staff member indicated the infant had been positioned in a Boppy Pillow placed on the floor. Although the infant was able to hold the bottle independently this regulation requires that a child be held while drinking from a bottle. Further interview with both staff members found it is common practice to position infants in Boppy Pillows placed on the floor. However, they also indicated that sometimes they will hold an infant while they drink from their bottle.

Staff members (DOH 08/16/23 and 03/20/23) had negative tuberculin skin results on file that exceeded two years, and therefore were no longer current. 2.) Staff members (DOH 5/13/24, 9/12/23, 8/12/23, 5/20/24, 2/26/24, 11/27/23, 10/20/23, and 12/4/23) did not have a negative tuberculin skin result, nor a physician’s statement clearing them of active tuberculosis in their personnel file.
General: Based on observation, the following was found: 1.) Staff member (DOH 03/20/23) did not have evidence of completing Cabinet- approved Orientation training, this training is required within the first 90 days of employment. 2.) Staff member (DOH 03/20/23) did not have evidence of completing Pediatric Abusive Head Trauma training, this training is required within the first year of employment. 3.) Staff member (DOH 03/20/23) completed five out of fifteen hours of Cabinet- approved early care and education training for the 03/20/23-03/19/24 training year. 4.) Staff member (DOH 03/20/23) completed one out of fifteen hours of Cabinet- approved early care and education training for the 03/20/23-03/19/24 training year. 5.) Staff member (DOH 03/20/23) completed nine out of fifteen hours of Cabinet- approved early care and education training for the 03/20/23-03/19/24 training year. The diaper changing pad used within the One-year-old classroom had a clothe covering, only non-absorbent material should be used on a diaper changing surface to ensure the pad is properly sanitized. The surveyor was unable to determine whether food safe conditions were maintained. 2.) The freezer in the Older Infant classroom refrigerator contained significant frost build-up.
