General: Based on observation, the following was found: 1) Several bucket seats at the sit-in activity table within the One-Year-Old Classroom were not equipped with the manufacturer’s seat belt. 2) A child’s feeding chair in the Lunchroom was not equipped with the manufacturer’s seat belt. 3) A teeter-totter on the School-Age playground had a cracked seat. 4) A sensory table on the School-Age playground was cracked in several areas. 5) A wooden bench on the School-Age playground was corroded and exposed a screw.

General: Based on observation, the following was found: Four (4)- to- Five (5)-Year-Old Classroom 1) A desk drawer within this classroom housed Ibuprofen and several containers of lotion. The warning label on these items read “Keep out of reach of children”. 2) An unlocked cabinet housed a container of Barbasol Shaving Cream and a container of perfume. The warning label on these items read “Keep out of reach of children”. Three (3)-Year-Old Classroom 1) A container of Twist hair lotion, a pint of latex paint, nail drying spray, and “Awesome” cleaner was stored in an unlocked cabinet and accessible to the children in care. 2) A child’s cubby had a container of Cutter Inspect Spray that was accessible to the children in care. 3) A child’s cubby stored two (2) containers of sunscreen that was accessible to the children in care. Pre-Kindergarten Classroom 1) An unlocked drawer within this classroom contained disinfectant wipes and a staple gun. These items were accessible to the children in care.
General: Based on observation, the following was found: 1) A blue and yellow climber on the School-Age playground was placed within two (2) feet of a swing set. The fall zone was not equal to the height of the equipment. 2) A green climber on the School-Age playground was approximately four (4) foot tall, however, the mulch only spread approximately three (3) feet out. The protective surfacing area was not equal to the height of the equipment.
General: Based on observation and interview, at 10:04 AM the surveyor entered the Two-Year-Old Classroom and observed one (1) staff member changing a child’s diaper. The staff member stated that she left her co-staff on the playground with eleven (11) children aged two (2). The surveyor observed one (1) staff member caring for eleven (11) children on the playground. The second staff member rejoined the group at 10:09 AM. General: Based on review of documentation, of the eleven (11) children's enrollment files reviewed, zero (0) contained a contact telephone number for their respective preferred hospitals. Additionally, one (1) child's file (DOE: 06/21/23) did not contain a telephone number for their preferred physician.
General: Based on observation and review of documentation, one (1) child was identified as having food allergies; however, an allergy care plan was not posted in the area where the food was served.

General: Based on observation, the child care center failed to maintain state regulations. During the survey, the following was observed: Pre-K Room 1.) In the Pre-K room underneath the window-mounted air conditioning unit the wall was in excessive disrepair. 2.) In the Pre-K room, a broken ceiling tile was observed. Girl's Restroom 1.) In the Girl's Restroom beneath the soap dispenser, an excessive amount of paint had been chipped away. An interview with the Director revealed the soap dispenser had recently been replaced and the former unit was much larger and left the unpainted area.
General: Based on observation, the child care center failed to maintain state regulations. A 2021 Ford Transit used to transport children from school to the facility daily did not have a fire extinguisher available in the vehicle. General: Based on observation, the radio in the One-Year-Old classroom was playing music that contained foul and profane language. The radio was playing “In Da Club” by 50 Cent during the inspection of the classroom.
General: Based on review of documentation, the child care center failed to maintain state regulations. Upon review of the children's files, the following was found: 1.) Four children’s files did not contain a preferred hospital. The dates of enrollment for these children are 04/05/21, 03/13/22, 9/16/21, and 04/07/21. 2.) One children’s file did not have parental signatures on any of the presented documentation. The date of enrollment for this child is 04/07/21. General: Based on observation, the child care center failed to maintain state regulations in regards to the health, safety, and comfort of children. The following was found: 1.) It was found that there were two (2) electrical wall outlets located in the dining area and Three's room. These outlets did not have safety covers and were in reach of children. 2.) In the dining area there was an uncovered thermostat used to regulate the heating and air conditioning. It was located directly beside the light switch, to the left of the entrance door. This was within reach of the children. 3.) In the dining area, there was a bucket of water with a light blue coloring, located on a stool in the upper right corner of the room. The bucket sat unattended during breakfast and for several minutes afterward. The bucket was within reach of children as they entered and exited the dining room. General: Based on observation, this regulatory requirement was not met. During the investigation, three infants were found in cribs along with multiple blankets, one in a bouncy seat set in the crib with a blanket over its head, and one on a play mat set in the crib.
General: Based on observation during the investigation in the infant room, two infants were found in cribs; one strapped in the bouncy seat that was set in the crib with blanket covering his face, and other on a play mat that was set in the crib. There is a diaper changing table in the two year old gross motor room, but there is not a sink; therefore, staff and children have to exit the room and enter the two year old classroom in the next room over to access a sink after diaper changes. General: Based on review of documentation and interview, this regulatory requirement was not met. The following was found: 1. One child had an enrollment packet on file; however, it was not signed by the child’s parent. 2. Two of the children’s files/enrollment documentation did not contain information for the family’s preferred hospital. 3. One child’s file/enrollment paperwork did not have physician contact information. 4. One child that was present at the facility on the day of the inspection did not have enrollment paperwork on file; therefore, none of the required information had been obtained. General: Based on observation, the following was found: 1.) In the school aged building, there were cleaners such as lysol wipes and windex being stored in an unlocked cabinet under the sink in the kitchen area. 2.) In the four and five year old classroom, there were staff members' purses and cups that contained liquid being stored in an unlocked closet.
General: Based on observation, the following was found: 1.) In the three year old classroom, there was a rug with different animals on it that was ripped and frayed around the edges. 2.) In the school aged classroom located in the main builidng, there was a gaming chair that was ripped leaving the padding underneath exposed. 3.) In the one year old room, there was a padded mirror on the wall in which the vinyl material was coming off. As a result, it left the padding underneath exposed. 4.) In the tots classroom, there was a white shelf that was in disrepair. This shelf was not smooth and had areas of chipped paint. 5.) In the tots classroom, there was a hard plastic child's pool that contained toys. This pool was very worn, warped around the edges, and no longer in good repair. 6.) In the two year old classroom, there was a cabinet door that was in disrepair and had duct tape on it.

General: Based on interview, the school age staff person stated that the children will sometimes go through the kitchen to get to the bathrooms which are located in the main hallway of the building. The surveyor noted Pine Sol and bleach sitting out on the kitchen counter. These items were accessible to the children. There is also a stove in the kitchen that the children would have to pass to get to the bathrooms. Premises Not In Compliance
General: Based on interview and observation, the director stated that right after Christmas they had to move out of the school age building due to a water leak behind the kitchen cabinets. She stated that the children were moved into the Fellowship Hall located in the church. She also stated that the children sometimes use two (2) classrooms upstairs. The Fellowship Hall and the upstairs classrooms are not a part of the facilities licensed space. Children were observed being cared for in the Fellowship Hall during the inspection.
General: Based on interview, the director stated that the school age building experienced a great deal of damage due to a water leak behind the kitchen cabinets. She stated that the building was gutted and they were having all new flooring put down and new construction was being done to the building. The director did not notify the cabinet prior to the start of the renovation. General: Based on observation, it was determined that in the Two (2) Year Old Room, the cabinet under the sink in the room was not locked. This cabinet contained a bottle of Ultra Joy Soap detergent and a bottle of Keystone Pot and Pan detergent. The cabinet was easily accessible to children in the room. In the Four (4) Year Old Room, a purple travel mug containing an unknown liquid was located on a table used by children. Children were sitting at the next table and the cup was easily accessible to children. Also in the Four (4) Year Old Room, a cell phone was located on the table being used by children. The cell phone was beside a student who was sitting at the table. The teacher was at another table with students for the duration of the time the surveyor was in the room. The cell phone was easily accessible to children.
General: Based on observation, it was determined the Two (2) Year Old Room had a fish in a small tank and did not have a written parental consent from each parent on file. The teacher stated she was not aware a consent form was needed. The children's files also did not contain any type of written consent form for the fish.
General: Based on review of documentation, there is no one on duty and present with children that is currently certified in Adult CPR from 7:00 a.m. to 7:30 a.m. and from 5:00 p.m. to 6:00 p.m.. There are school-age children present during these times. Also, the three (3) staff identified as van drivers that transport school-age children were not certified in Adult CPR. General: Based on Observation, during a tour of the infant room a plan of activities was posted on the window. There were no dates as to when these activities would be conducted. This was also found in the Pre-K room. During a tour of the 2's room the plannned program of activities with dates the activities would be conducted was not posted. General: Based on Review of Documentation, it was found through a review of seventeen (17) staff files that two (2) staff files did not have evidence of proof of education.
General: Based on Observation, there were large areas of grass and dirt under a green, curved metal climber on the large playground. There was a small amount of mulch that could be seen through the dirt and grass. On this same playground there was a tall metal slide sitting on dirt and grass with a small amount of mulch showing through. One (1) of the swings had only dirt underneath the area. General: Based on Observation, it was found that an unlocked drawer in the kitchenette of the School Age building contained a screwdriver, a box cutter and empty plastic bags. An unlocked cabinet in this same kitchenette was found to contain Smart Sense disinfecting spray and multiple cans of spray paint, each of which bore the label, "Keep Out of Reach of Children." The children walk through this kitchenette to get to the classroom areas.
General: Based on Observation, it was found that the arch monkey bars were not securely anchored to the ground.
General: Based on Observation, it was found that all climbers, slides and swings located on both playgrounds were positioned on very sparse mulch, grass and dirt. These items included five (5) swings, four (4) climber/slide combination units ranging from two (2) to three (3) feet in height, multiple metal automobile-shaped climbers, an arch jungle gym, six (6) feet tall monkey bars, a wave/arch jungle gym and a seven (7) feet tall metal slide. General: Based on Review of Documentation, it was found that out of seventeen (17) staff files reviewed, nine (9) staff files did not have evidence of the fifteen (15) hours of cabinet- approved early care and education training during each subsequent year of employment. The dates of hire are 12/21/2005, 09/06/2005, 11/10/2004, 09/10/1998, 11/10/1985, 09/05/2006, 11/10/2003, 09/10/2003 and 09/23/1997 for the employees lacking the training hours.
General: Based on Observation, it was found that in the lunch room where the children eat their lunch, there was an electrical box hanging out of the wall exposing wires.

General: Based on Observation, it was found that there were several areas of chipped paint on the walls in the afterschool classroom. There was found to be a hole in the wall by the closet in the two (2) year old classroom and a hole in the wall above the book area in the two (2) year old classroom. It was also found that there was paint chipping from the ceiling.
General: Based on Observation, it was found that there was no seating available for the children in the toddler classroom. Transportation
General: Based on Observation, it was found that the refrigerator handle in the afterschool classroom was broken. It was also found that face of a drawer was broke off on a cabinet in the two (2) year old classroom.
General: Based on Review of Documentation, it was found that the staff have not been provided with a current copy of the administrative regulations. The most current copy of the regulations were from 2008.
General: Based on Review of Documentation, it was found that a staff with a hire date of 05/08/2014 did not have evidence of six (6) hours of cabinet-approved orientation within the first three (3) months of employment. It was also found that staff persons with hire dates of 08/10/1997 and 09/10/2003 did not have the required 15 hours of cabinet-approved early care and education training during each subsequent year of employment.

General: Based on Review of Documentation, the director failed to supervise staff conduct to ensure implementation of program policies and procedures. Based on interview, one (1) staff member had placed a child in care in time out while standing and facing a wall. In the section labeled Discipline of Children, the written policy of the facility states, “at no time can a teacher put a child standing in time out with the child’s face/nose facing towards a wall.” During staff interview, a staff member reported that a child had been placed in time out while standing and facing a wall. Based on interview, one (1) staff member reported that socks had been placed on a child’s hands to prevent the child from hitting other children. The staff member reported that the socks had been placed on the child’s hands on more than one occasion. The staff member also reported that the socks are usually kept on the child’s hands for no more than fifteen (15) minutes at a time. The staff member stated that the socks were never placed on the hands of other children. The facility written policy states, “No child shall be teased, humiliated, frightened, ridiculed, or belittled at any time. No child shall be shamed.”
General: Based on Observation, the facility failed to maintain minimum staff to child ratio and group size. At the time of survey, ten (10) infant aged children were observed being cared for by one (1) staff member for approximately one (1) minute. The staff to child ratio for an infant classroom is one (1) staff member for five (5) children. In the toddler classroom, two (2) staff members were observed caring for thirteen (13) children which exceeds the minimum ratio. The children were one (1) year old. The maximum group size for one (1) year old children is twelve (12). In the outdoor play area, thirty-eight (38) children were found being cared for by three (3) staff members. The youngest child was three (3) years of age. The maximum group size for a child three (3) years old is twenty-four (24). Director Requirements
General: Based on Review of Documentation, staff members hired on 11-10-85, 6-4-10, 11-10-03, and 11-10-04 did not have documentation of fifteen (15) hours of annual training during the last full year of employment on file at the facility.

General: Based on review of documentation, the child care center failed to maintain state regulations. During the survey, the following was observed: 1.) An employee, (DOH: 12/21/2005), had failed to update their pediatric abusive head trauma training within the five (5) year regulation. It should be noted the training has since been updated. 2.) An employee, (DOH: 05/24/2024), has not yet completed their orientation training which is required within the first three (3) months of employment. 3.) An employee, (DOH: 09/06/2005), has not updated their pediatric abusive head trauma training within the five (5) year regulation. 4.) General: Based on interview and review of documentation, the child care center failed to maintain state regulations. At the time of survey, two (2) vehicles were presented for inspection, although one (1), a 2021 Ford Transit, was not previously inspected and approved by DRCC. Through review of the facility's paperwork, it was determined that no Change Request had been submitted to add this vehicle to the license. It should be noted that this vehicle does transport children daily from various elementary schools to the facility and is currently in use.

In the One-Year-Old Toddler room, a white electrical cord was hanging from the air conditioner to the wall outlet. The wire was hanging low and could be easily pulled/tugged on by children. General: Based on observation, the child care center failed to maintain state regulations. During the survey, the following was observed: Two's Room #1 1.) In the Two-Year-Old #1 room there were cobwebs and dust connecting from the air conditioning unit to the window. Two's Room #2 1.) The staff person stated that she had been the only teacher in the classroom that morning. General: Based on Interview, the facility failed to ensure that a child in care be provided with experiences that are developmentally appropriate for each child served. Staff members reported that a child in care is made to wear socks on the child’s hands to prevent the child from hitting other children. The staff members reported that no other children in the classroom are made to wear socks on their hands when they hit other children. The staff members stated that the child is usually made to wear the socks for no more than fifteen (15) minutes. Also, the staff members reported that a child in care was placed in time out while standing and facing a wall. The staff member reported that a sheet of paper was taped to a wall, with a red circle drawn in the middle of the paper. The staff member reported that a child in care was made to stand in time out while looking at the paper taped to the wall. The staff member reported that the child was made to stand for no more than five (5) minutes. The staff member reported that only one (1) particular child is made to stand for time out.
