General: Based on interview and review of documentation, on 06/02/26 at approximately 11:00 a.m., a staff member left two (2) children, who were two-years-old, alone on the playground during a transition to the classroom. General: Based on review of documentation, the following was found: 1) A staff (DOH: 01/20/20) only had three (3) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 2) A staff (DOH: 06/22/22) only had ten (10) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 3) A staff (DOH: 04/10/23) only had eleven (11) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 4) A staff (DOH: 02/02/22) only had fourteen (14) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 5) A staff (DOH: 06/12/23) only had eleven (11) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 6) A staff (DOH: 07/25/23) only had eight (8) hours of early care and education training during the 07/01/24 to 06/30/25 training year. 7) A staff (DOH: 04/08/24) only had fourteen (14) hours of early care and education training during the 07/01/24 to 06/30/25 training year. General: Based on interview and review of documentation, the following was found: On 10/08/25, at approximately 3:15 p.m., the staff member in charge received a text message from a parent reporting that a child was alone in the classroom. The parent observed this through live camera footage on the Sprout About app. At that time, the two staff members responsible for the child were on the playground with eleven (11) toddlers, ages one to two years old. Upon receiving the message, the staff member in charge immediately notified the classroom staff that a child remained inside. One of the staff members went to the toddler classroom’s back door, observed the child playing, and called the child to come outside. The staff member in charge stated that both staff members failed to follow the required name-to-face attendance procedure. Immediately following the incident, the staff member in charge reported the event to her supervisor and to the Division of Regulated Child Care (DRCC). The supervisor reviewed the video footage and determined that the child had been alone in the classroom from 3:00 p.m. to 3:16 p.m. Both staff members responsible for the child were immediately placed on suspension and subsequently terminated. As a result of this incident the health, safety, and comfort of the child was compromised. Director Requirements Not In Compliance

General: Based on a review of documentation, a staff member (DOH 08/18/25) did not have an eligible determination status in the Kentucky National Background Check Service (KARES) system. The determination status was listed as “In Process.” The staff member was present during the inspection but left at the request of the staff-in-charge. During file review, it was found that three (3) staff members (DOH: 06/21/22, 08/22/22, and 10/03/22) did not complete the fifteen (15) hours of training that is due annually. A review of ECE-TRIS verified this information.
General: Based on review of documentation, the 2021 Ford vehicle used to transport the children did not have current monthly documented emergency drills. The last documented evacuation drill for this vehicle was from July of 2024. The 2012 Chevy vehicle used to transport the children did not have any monthly documented emergency drills in the binder. The form in the binder was blank. General: Based on interview and review of documentation, the child care center failed to maintain state regulations. During file and a KARES system review, it was found that three (3) current staff members (DOH: all three (3) 08/22/22), did not have an up-to-date background check completed at the time of the survey. The center director claimed the KARES system usually sends out a notification email to alert her that background checks that are approaching the five (5) year resubmission date for an employee is drawing near; however, she did not receive the emails for any of the aforementioned employees. However, on 10/08/25, two (2) staff members working in the Toddler Classroom failed to follow this policy. As a result, one (1) toddler was left alone in the Toddler Classroom from 3:00 p.m. to 3:16 p.m., while the staff members and eleven (11) additional toddlers were outside on the playground. General: Based on observation, the child care center failed to maintain state regulations. While the surveyor was doing file review in the School Age classroom, that was unoccupied at time, he could hear staff and children outside on the playground (it is located directly behind the classroom). At approximately 9:17 am, he overheard a staff member, that was outside, interacting with a child in care. The child was asking questions to the staff member, when she replied back in a loud, aggressive tone: "you need to just grow up", followed by "you are bugging me this morning". At that time, the surveyor walked to the window to see the staff member and the child sitting at a picnic table. Minutes later, the other surveyor came into the School Age classroom. The issue was discussed, and it was found the staff member is a teacher at the center (DOH: 03/01/24).
