General: Based on observation and review of documentation, the following was found: 1) The statement of deficiency and corresponding plan of correction from the 2024 licensure year was not posted for public inspection. The most recent inspection was dated 08/15/23. 2) The daily planned program (lesson plan) posted within the Infant 2 Classroom was dated for July 7th through July 11th. The posted plan was not current.
General: Based on observation, a plunger was stored by the toilet within the Two’s and Three’s Classroom and was accessible to the children in care.
General: Based on review of documentation, a staff member (DOH: 05/22/25) did not have evidence of a high school diploma, GED or Commonwealth Child Care Credential on file in the facility.

General: Based on observation and interview, this regulatory requirement was not met. Upon arrival, the surveyor toured the facility with the staff person in charge. Eighteen children (three-to-five-year-old's) and one staff person were observed present in the Pre-K Classroom at 8:03AM. The staff person in charge stated that the preschool teacher, who arrived to work at 8:00AM, would be pulling the four preschool children from the group and taking them to her classroom. The staff person in charge remained in the Pre-K Classroom for ratio purposes until the preschool teacher arrived in the classroom. Since the youngest child in the group was a three-year-old, the minimum staff to child ratio would need to be one staff person for twelve children. General: Based on interview, on 4/19/22, a two year old child was left alone in his classroom for approximately 10 minutes when his class went out to the playground. The child hid behind the door so the teachers were unaware he was still in the classroom. A parent called the center to let them know a child was alone in the classroom. The Director viewed the video footage of the incident to confirm what happened. The head counts and face/name sheet wasn't done correctly. Child was not hurt but shaken up. General: Based on review of documentation, the following was found: 1. Staff person hired 6/15/2020, did not have evidence of completing the required orientation training in the first three months of employment. 2. Staff persons hired 7/17/18 and 9/26/18, did not have evidence of completing the Pediatric Abusive Head Trauma training within the first year of employment. General: Based on observation, the following was found: 1.) In the two-year-old classroom, there were crayon marks on the walls. 2.) In the toddler classroom, there was an area of the wall that needed to be repainted. It could not be determined if there was a substance on the wall or if the paint was beginning to wear/rub off.
General: Based on observation, the following was found: 1.) There was a staff member's purse as well as a Starbucks coffee drink sitting on top of a shelf in the classroom of the Pre-K 1 classroom. These items were accessible to the children in the classroom. 2.) During a walkthrough of the kitchen, it was found that the door was propped open. In the kitchen, there was a large knife sitting out near the sink area.

General: Based on interview and review of documentation such as attendance records, it was determined that there were numerous times that the infant one and two classrooms, as well as the toddler one classroom were over ratio and over the maximum group size. Upon review of attendance records, it was found that there were seven days when the infant one classroom was over group size and ratio. On these days, the number of infants varied from 11 to 14 at any given time; therefore, not meeting the maximum of 10 infants per group. When reviewing the attendance records for the infant two classroom, it was found that there was no documentation indicating that the class was over group size; however, it was found that at certain times throughout the day, there would be up to nine children in the classroom. The attendance records did not indicate what teachers were present and at what time they arrived. Interviews with the infant two teachers indicated that the room was over ratio at times and had too many children with only one teacher present. Upon review of attendance records for the toddler one classroom, it was found that there were at least three days when this classroom was over group size and ratio by one child. During a tour of the Early PreSchool Room, the surveyor observed a good amount of tissue paper to be lying on the floor of the toilet room. The tissue paper was wet and water was also on the floor in the toilet room. The surveyor noted the class had just left the room to go to the playground. General: Based on review of documentation, the following was found: 1.) There was no record of six (6) hours of cabinet-approved orientation training on ECE-TRIS for three (3) staff members hired 6/4/18, 7/9/18, and 8/17/18. 2.) Three (3) staff members did not have records on ECE-TRIS evidencing completion of Pediatric Abusive Head Trauma Training within the past five (5) years. A staff member hired 3/5/18 last completed PAHT training on 5/14/12, a staff member hired 4/8/2002 last completed PAHT training on 1/3/14, and a staff member hired 1/19/16 last completed the PAHT training on 12/4/13. 3.) Two (2) staff members hired 6/4/18 and 2/3/18 did not have record of fifteen (15) hours of annual cabinet approved early care and education training on file for review. Surveyor observed a group of sixteen (16) children (two (2) year olds) return to their classroom from the playground during the visit. Upon entering the classroom, the staff person instructed the children to grab a book and sit on the carpet. During a tour of the preschool outdoor playground the surveyor observed a large trash can that was overflowing onto the pavement. The lid was balancing on top of the piled trash. The assistant director acknowledged the finding during the tour and stated, "I need to make sure our cleaning person knows that we have a trash can out here". General: Based on Observation, the Infant I room contained a refrigerator that was used to store bottles. The thermometer inside the refrigerator indicated a temperature of forty-nine (49) degrees Fahrenheit. A cold-storage facility used to store perishable food in a non-frozen state shall be forty (40) degrees Fahrenheit or below. In addition, the Infant 2 room contained a thermometer inside the refrigerator that indicated a temperature of forty-five (45) degress Fahrenheit.

General: Based on Review of Documentation and interview, this regulatory requirement was not met. One (1) staff person (hire date: 6/13/16) did not have educational documentation on file. The staff person in charge stated that this documentation was obtained; however, she was not able to locate it during the visit.
General: Based on Observation, the large wall vents in the two's classroom and the school age classroom were covered in dust.
General: Based on Observation, both sides of a pink playhouse on the playground was separated from the playhouse roof and was no longer stable.
General: Based on Review of Documentation, the following was found: 1. Staff person with hire date 5/7/15 had a criminal records check (CRC) dated 5/11/15. There was no documentation to verify that the CRC was submitted within the required time frame. 2. One (1) staff person (hire date: 11/17/15) had a child abuse/neglect check (CAN) that was submitted on 3/31/16; therefore, it was not submitted within the required time frame. This same staff person had a CRC that was dated 12/4/15. There was no documentation to verify that the CRC was submitted within the required time frame. 3. One (1) staff person (hire date: 10/20/15) had a CAN check that was submitted on 11/2/15; therefore, it was not submitted within the required time frame. 4. Staff person with hire date 7/5/16 had a criminal records check (CRC) dated 7/14/16. There was no documentation to verify that the CRC was submitted within the required time frame. 5. Record review and interview found that an employee hired on 7/13/16 has lived in Indiana within the last five (5) years. The staff person in charge stated that she has not submitted an Indiana background check for this employee. The staff person stated that she moved to Kentucky in May 2016. 6. One (1) staff person (hire date: 1/11/16) had California listed on her CAN check as a previous residence. The staff person in charge stated that she did not know when the staff person moved to Kentucky and indicated that she has not submitted a California background check for this employee.

General: Based on Interview, the infant room was exceeded the required staff to child ratio on 2/10/16. On that day there was one (1) staff person with seven (7) infants for approximately ten (10) minutes. The staff issue was the result of a staff person who did not follow the proper procedure for calling in. Also, during interview it was found that the younger infant room is over staff to child ratio during morning arrival time.
General: Based on Review of Documentation, the facility did not have an updated emergency preparedness plan that had been turned in to the local emergency management officials.
General: Based on Observation, the 2008 Chevy mini-school-bus, used to transport the children, had a left rear blinker that was not working.
General: Based on Observation, one section of the wooden fence on the back playground was leaning and when pushed the fence swayed back and forth.
General: Based on Observation, the back playground contained a sharp nail that was exposed on the wooden fence (in the leaning section of the fence). The exposed nail was within reach of a child.
General: Based on Review of Documentation, it was found that an employee with a hire date of 07/29/2015 did not have evidence of a TB skin test and an employee with a hire date of 05/07/2015 did not have a TB skin test completed until 06/26/2015.
Based on interview, facility staff reported that children are routinely moved to another classroom in order to maintain minimum staff-to-child ratios.
Based on interview there have been times the facility does not maintain staff-to-child ratios. Staff members reported that when they are over the minimum staff-to-child ratios they notify the management but it can be up to five (5) minutes before additional staff are sent or a child is moved to another classroom. It was also reported that on 4/7/15 one staff had thirteen (13) two and three year old child in care for over thirty (30) minutes. Programming
General: Based on Interview, it was found that children are moved from the Preschool classroom to the Pre-K classroom when/if there are too many children for one (1) staff person in the Preschool classroom. Staff stated that this happens almost daily and the same children are not always sent to the other classroom. Another staff stated that children are moved to the Preschool room from the Twos classroom if there are too many children for one (1) staff person in the Twos classroom. The staff person stated that his occurs two (2) to three (3) times a week.
General: Based on Interview, 1. Staff stated that on 4/7/15 she was alone with thirteen (13) children, ages three (3) years to four (4) years, for more than thirty (30) minutes. 2. Staff stated that a couple of weeks ago she was alone with fifteen (15) children, ages three (3) years to four (4) years, for over an hour. 3. General: Based on Observation, the refrigerator in the Infant Bitsy clasroom did not contain an indicating thermometer. The refrigerator was being used to store bottles of formula for the children in care.

This staff person was observed alone supervising fourteen (14) preschool children. 2. Staff person hired 6/17/14 had the results of a criminal records check dated 6/24/14. General: Based on Observation, a fish was observed in the two (2) year old room. An interview with the staff person in charge found that parental consent has not been obtained from the parents of the children who have access to the fish.
General: Based on Review of Documentation, the insurance card on the bus expired 4/1/13. An interview with the staff person in charge found that the bus was used to take children to and from school until school was out for the summer and it was used to transport the school age children on field trips this summer. The bus has also been used to transport children to and from school since school started last week. Before the end of the survey the staff person in charge found an envelope on the desk that had a new insurance card. However, this card had not been on the vehicle and the vehicle has been used to transport children.
Based on Review of Documentation, it was confirmed that a staff person hired on 1-2-09 did not have evidence of negative T.B. results completed every two (2) years on file in the facility. The most current documentation on file in the facility was dated 12-16-10. Crawler Room: Based on Observation, a seven (7) month old was found asleep in a crib with a boppy pillow. The staff person in the classroom stated that the child coughs a great deal if she is lying flat so a boppy pillow is used to help her rest better. General: Based on Observation, during a tour of Infant II there was an unlocked cabinet under the changing table that had a bottle of Johnson's Baby Shampoo without a lid and instructions to "Keep Out of the Reach of Children" on the back. There was a bottle of industrial strength glass cleaner with the words "DO NOT DRINK" in large bold letters down the side. It also indicated to "Keep out of the reach of children". A can of Barbosal shaving cream with the label indicating to "Keep out of the reach of children" and a jug of bleach were found in the same cabinet. Although this was found in an infant room some of the children are mobile making these items accessible. At the time of the visit the children in this room had been combined with another group for the morning. However, an interview with staff found that those children would be going to the classroom at some point during the day.
