General: Based on observation, the following was found: 1. There were two open windows in the Orange classroom. The windows did not have screens or any barrier to the outdoors. 2. There were two open windows in the Yellow classroom. The screens in the windows did not properly fit the opening, leaving a two-inch gap at the top of the opening. 3. The door to the playground was propped open for approximately 30 minutes. There was no screen or barrier to the outdoors. General: Based on Review of Documentation, it was found that the file for an employee hired on 9-1-08 contained evidence of a TB skin test that was read on 7-16-14. This date is not within the two (2) year timeframe required by the regulation.

General: Based on Observation, there were bowls of uncovered cheese cubes and fresh strawberries for snack in the refrigerator in the office.
General: Based on Review of Documentation, it was found that earthquake and tornado drills are being conducted and documented as one (1) drill. Therefore the date, time and the children who participated is only documented for one (1) drill.

General: Based on Review of Documentation, there was one (1) staff person hired as a substitute 8/29/12 who did not have evidence of having completed fifteen (15) hours of training for the 2013-2014 training year. It was also found that the results of a T.B. skin test for this same person exceeds the two (2) year time frame.

Also in this classroom, a wall-mounted plastic bag holder full of empty plastic bags was found to be mounted within reach of the children.
