General: The following observations regarding the health, safety, and comfort of the children at the center were made: 1.Two (2) children in the four and five-year-old room were observed bumping their bodies together while arguing over a chair as the teacher in the classroom stood in close proximity to the two (2) children while passing out food for lunch. The two (2) children continued to argue and bump each other for several seconds until one child pushed the other with force, causing him to stumble. The child who was pushed then moved to an empty chair at another table and sat down. During the entirety of the aforementioned exchange, the teacher in the classroom failed to redirect the children, comfort the child who was pushed, or acknowledge the situation in any way. 2. Three (3) toddlers in the one's classroom were observed sitting in highchairs without food or activities of any kind while the lights were off in the classroom and the two (2) staff members in the classroom sat on the floor in another section of the room with five (5) toddlers who were resting on cots. One (1) of the toddlers that was sitting in a highchair was crying and struggling to get out of the highchair while the two (2) staff members continued to sit on the floor with the other five (5) toddlers in the room. One of the aforementioned staff members stated that the toddlers in the highchairs were "waiting." 3. Electrical outlets in both the two's and four and five-year-old's classrooms were uncovered. In an interview, the staff member stated the children eat and are changed in the area. 2) Observation in the Transitioning Two's room revealed children failed to be safe as thirteen (13) children were hitting, smacking, snatching, throwing and slamming blocks as they sat at a table waiting for lunch to be served. The room exhibited general chaos for more than fifteen (15) minutes. 3) Observation in the Two's room failed to reveal programming while waiting for lunch to be served, resulting in children hitting and fighting with each other. 4) In the Fours classroom, a child ranging in age from four (4) years to five (5) years was observed lying next to an electrical outlet containing plugged in devices.
